Intracranial Pressure.

Elevated intracranial pressure (ICP) is a critical condition that requires rapid diagnosis and intervention to prevent severe neurologic damage and mortality. While invasive ICP monitoring remains the gold standard, it carries significant risks in cases of coagulopathy, bleeding diathesis, or in unstable patients. Additionally, there is a risk of infection, particularly in immunocompromised patients. Transcranial ultrasonography offers a valuable, non-invasive bedside tool for indirectly assessing elevated ICP in critical care settings. There are 3 primary modes of detecting elevated ICP using

Raised Intracranial Pressure.

Raised intracranial pressure is a relatively common problem facing the clinician treating neurocritically ill patients. It is a leading cause of death in patients with intracranial pathology. There is a lack of controlled clinical trials evaluating most of the therapies currently available for raised intracranial pressure. The basic pathophysiologic and clinical principles of raised intracranial pressure are discussed and the major treatment options are presented. Patients with raised intracranial pressure should be evaluated immediately with particular attention to airway and hemodynamic stat

Intracranial hypertension. Advances in intracranial pressure monitoring.

Research is continuing to find improved methods for monitoring intracranial pressure as well as the effect of intracranial hypertension on the patient's recovery. Each of the methods of monitoring has its advantages and disadvantages and the use of one method over others is usually the preference of the neurosurgeon. It is the responsibility of the nurse to be aware of the strengths and weaknesses of each device in order to strengthen patient outcome. Again, the major benefits of the bolt are a low infection rate and easy insertion. Problems associated with its use of which the nurse should be

Intracranial flow patterns at increasing intracranial pressure.

In the course of a pilot study, changes in intracranial pressure were compared with the transcranial Doppler findings of the middle cerebral artery. The cases of five patients were discussed who developed dissociated brain death in spite of intensive therapeutic measures. The studies showed that changes of the intracranial pressure influenced the flow patterns considerably: at increasing intracranial pressure (decreasing cerebral perfusion pressure) a progressive reduction of the systolic and above all diastolic flow velocities and finally a pendular flow occurred. These changes could be recor

Surgical treatment of elevated intracranial pressure: decompressive craniectomy and intracranial pressure monitoring.

Surgical techniques that address elevated intracranial pressure include (1) intraventricular catheter insertion and cerebrospinal fluid drainage, (2) removal of an intracranial space-occupying lesion, and (3) decompressive craniectomy. This review discusses the role of surgery in the management of elevated intracranial pressure, with special focus on intraventricular catheter placement and decompressive craniectomy. The techniques and potential complications of each procedure are described, and the existing evidence regarding the impact of these procedures on patient outcome is reviewed. Surgi

[Monitoring of intracranial pressure].

An account of the significance of intracranial pressure is followed by a review of the various sites in which it can be recorded (spinal canal, lateral ventricles, subarachnoidal spaces, operation cavity, other sites). Monitoring of ventricular pressure is discussed in detail; this deals with the classic site and its technique has become part of clinical practice. The utilisation and interpretation of the data in the form of pressure levels and their changes in the course of time are also discussed. The clinical indications for monitoring are noted.

Intracranial pressure and veins.

This review summarizes the relationship between intracranial pressure and the venous system as far as ultrasound methods are concerned. Although the pressure in cortical veins is tightly and linearly coupled to the intracranial pressure, venous flow velocities in the basal veins are not. However, venous flow velocities reflect better the local cerebral blood flow than measurements in arteries. Therefore, they correlate better with clinical outcome in head trauma and subarachnoid hemorrhage. Transorbital sonography with measurement of the optic nerve sheath diameter and optic disk elevation has

Management of Intracranial Pressure.

PURPOSE OF REVIEW: Intracranial pressure (ICP) can be elevated in traumatic brain injury, large artery acute ischemic stroke, intracranial hemorrhage, intracranial neoplasms, and diffuse cerebral disorders such as meningitis, encephalitis, and acute hepatic failure. Raised ICP is also known as intracranial hypertension and is defined as a sustained ICP of greater than 20 mm Hg. RECENT FINDINGS: ICP must be measured through an invasive brain catheter, typically an external ventricular catheter that can drain CSF and measure ICP, or through an intraparenchymal ICP probe. Proper recognition of t

Elevated intracranial pressure management.

The treatment strategy for increased intracranial pressure (ICP) management includes decisions on head and body position, hypothermia, ventilation, anesthetics, osmotic drugs and surgical procedures. We can treat sudden increases in ICP using osmotic agents, some anesthetics and short episodes of mild hyperventilation. Propofol seems to be suitable for sedation of the increased ICP patients. Surgical decompression of the cranium seems to improve the outcome of the younger patients (below 50 years old), especially children.

Managing elevated intracranial pressure.

PURPOSE OF REVIEW: Elevated intracranial pressure is one of the major deteriorating factors in patients with intracerebral lesions. Therefore, every year many experimental and clinical studies are performed to identify the best method for managing elevated intracranial pressure in head-injured patients. The current review summarizes the most important recent findings for the treatment of increased intracranial pressure. RECENT FINDINGS: The currently most discussed treatments of elevated intracranial pressure are the use of hypertonic saline, which seems to be equal to mannitol, the use of hy

Intracranial pressure in craniostenosis.

In this study, intracranial pressure (ICP) was recorded with an epidural sensor for periods of 12 to 24 hours in 92 cases of craniosynostosis. Pre- and postoperative recordings were performed in 23 patients, and 55 children underwent preoperative psychometric testing. The ICP was found to be normal in one-third of the cases, was obviously elevated in one-third, and was borderline in one-third. Waves of increased ICP were recorded during rapid eye movement (REM) sleep. After surgery, ICP decreased progressively and returned to normal in several weeks. A significant statistical relationship was

Fever and intracranial pressures.

The effect of fever upon intracranial pressures was determined in the rabbit and cat. In the unanesthetized rabbit and cat, cerebrospinal fluid (CSF) pressure was measured via direct cannulation of the lateral cerebral ventricle. Intracranial pressure (ICP) was measured in the rabbit by a subarachnoid screw technique. In all cases, intravenous administration of bacterial pyrogen extracted from Salmonella abortus equi resulted in significant differences from controls in physiological variables measured during the initial "chill" phase of the fever. There was an increase in body temperature, a f

Intracranial pressure and intracranial volume in children with craniosynostosis.

Intracranial volume and intracranial pressure have been measured in 66 children with craniosynostosis, 48 boys and 18 girls. The premature fusion of skull sutures is assumed to restrict skull growth and predispose to elevated intracranial pressure. Thirteen children (20 percent) had raised intracranial pressure and demonstrated a significant restriction of skull growth. In this series, volume measurement alone, however, did not serve as a reliable predictor that the intracranial pressure was raised.

Intracranial Compliance Assessed by Intracranial Pressure Pulse Waveform.

BACKGROUND: Morphological alterations in intracranial pressure (ICP) pulse waveform (ICPW) secondary to intracranial hypertension (ICP >20 mmHg) and a reduction in intracranial compliance (ICC) are well known indicators of neurological severity. The exclusive exploration of modifications in ICPW after either the loss of skull integrity or surgical procedures for intracranial hypertension resolution is not a common approach studied. The present study aimed to assess the morphological alterations in ICPW among neurocritical care patients with skull defects and decompressive craniectomy (DC) by c

Effects of positive end-expiratory pressure on intracranial pressure in dogs with intracranial hypertension.

Positive end-expiratory pressure (PEEP) is used to improve oxygenation in patients with the adult respiratory distress syndrome. Nevertheless, this treatment may increase intracranial pressure (ICP) and be detrimental to certain neurosurgical patients. This clinical situation was simulated by administering PEEP to dogs with normal and elevated ICP. Increases in PEEP increased ICP in all animals. However, the presence of intracranial hypertension diminished the increase in ICP seen at a given level of PEEP. Cerebral perfusion pressure also fell less in the presence of intracranial hypertension

Arterial blood pressure vs intracranial pressure in normal pressure hydrocephalus.

OBJECTIVE: To characterize the association between arterial blood pressure (ABP) and intracranial pressure (ICP) in idiopathic normal pressure hydrocephalus (iNPH) patients, and its impact on outcome of shunt surgery. MATERIALS AND METHODS: We analyzed all 35 iNPH patients whose ABP and ICP were recorded simultaneously during 6 years (2002-2007). The static and pulsatile pressures were averaged over consecutive 6-s intervals; the moving correlations between ICP and ABP (static and pulsatile) were determined during consecutive 4-min periods to explore time-related variations. RESULTS: Neither

Critical closing pressure during intracranial pressure plateau waves.

BACKGROUND: Critical closing pressure (CCP) denotes a threshold of arterial blood pressure (ABP) below which brain vessels collapse and cerebral blood flow ceases. Theoretically, CCP is the sum of intracranial pressure (ICP) and arterial wall tension (WT). The aim of this study is to describe the behavior of CCP and WT during spontaneous increases of ICP, termed plateau waves, in order to quantify ischemic risk. METHODS: To calculate CCP, we used a recently introduced multi-parameter method (CCPm) which is based on the modulus of cerebrovascular impedance. CCP is derived from cerebral perfusi
