[Intraoperative awareness: a true or false problem?].

The authors base their remark on a few cases of awareness which have been reported in their hospital and on the history a patient who experienced intraoperative awareness during cesarean section. Attention is focused on the ethical, social and medico-legal aspects of this phenomenon. Awareness may severely affect the patient's psychic equilibrium with the consequent onset of severe neuroses leading to the possibility of legal action and claims for compensation. Clinical experience has shown that the use of halogenate anesthetics, even in low concentrations, may help to avoid this phenomenon. C

Intraoperative awareness and recall during total hip arthroplasty.

A case of intraoperative recall and awareness in a patient with sickle cell disease who had received midazolam, 10 mg at induction of anesthesia, is reported. The patient was on opioids which may also have caused tolerance. Although the use of midazolam may have been judged to provide adequate amnesia, this was clearly not the case. Upon analysis of the reasons for recall, we predicted from a two-compartment model that the estimated blood level was 88 ng/mL-1; this level would have provided no amnesia. This case illustrates that recall can occur with the use of midazolam, especially if levels

A retrospective study of intraoperative awareness with methodological implications.

BACKGROUND: Awareness during general anesthesia is a problem receiving increased attention from physicians and patients. Large multicentered studies have established an accepted incidence of awareness during general anesthesia as approximately 1-2 per 1000 cases or 0.15%. More recent retrospective data, however, suggest that the actual incidence may be as low as 0.0068%. METHODS: To assess the incidence of awareness at our institution, we conducted a review of adult patients undergoing surgical procedures over a 3-year period. Information on awareness came from entries of "Intraoperative Awar

Long-term posttraumatic effects of intraoperative awareness in children.

BACKGROUND: The consequences of intraoperative awareness in children have received little empirical attention to date. Previous studies suggest that children are less likely to be distressed than adults in the short term, but long-term consequences have only been studied retrospectively. AIM: The aim of this study was to investigate posttraumatic stress symptoms in children who have experienced an awareness event. METHODS: In this prospective cohort study, children who were found to have experienced intraoperative awareness in our previous study were approached for assessment for posttraumat

Awareness during general anaesthesia--implications of explicit intraoperative recall.

Anaesthesia awareness (AA) is postoperative recall of events experienced under general anaesthesia. Most frequently patients remember an auditory perception, the feeling of motor function lost, pain, helplessness, anxiety, panic, impending death. The prevalence of awareness in nonobstetric and noncardiac surgical cases is 0.1%-0.2%. The prevalence is higher in cardiac surgery, obstetric and major trauma cases. According to the results of many studies light anaesthesia is the most common cause of the AA. Posttraumatic stress disorder appears in 33%-56% of patients who experienced awareness duri

Bispectral index-guided versus standard anaesthesia monitoring for intraoperative awareness prevention.

Although bispectral index (BIS) monitoring provides an objective measure of hypnotic depth, its routine use to prevent awareness remains debated due to conflicting evidence and cost considerations. Therefore, it is of interest to evaluate BIS-guided versus clinically guided sevoflurane anaesthesia in 200 adults undergoing elective surgery over 60 minutes. No definite intraoperative awareness occurred in either group, confirmed by modified Brice interviews up to 30 days postoperatively. BIS monitoring reduced sevoflurane consumption by 24% (1.7 ± 0.4 vs 2.2 ± 0.5 MAC-hours; p < 0.001) and accel

Intraoperative awareness--a three year prospective study using bispectral index monitor (BIS).

Patients who experience awareness under surgery may suffer from the post-traumatic stress disorder (PTSD) with its long-lasting psychological damage. In addition, there are also media attention and legal consequences. In spite of understanding its causes, it is still occurring worldwide. This prospective study was conducted to determine the incidence of awareness using the bispectral index monitor (BIS) when its causes are eliminated. There were 2328 patients admitted to the study. Their ages ranged from 14-104 yr (mean 38.6 yr). All patients were interviewed in the postoperative period. There

Intraoperative Ionizing Radiation Exposure Awareness and Associated Morbidity in Neurosurgery: A Nationwide Survey.

BACKGROUND: Neurosurgeons often use radiation to visualize blood vessels and implants intraoperatively. However, high exposure to radiation increases one's cancer risk. This study aims to investigate intraoperative ionizing radiation exposure awareness and associated morbidity among neurosurgeons. METHODS: An anonymized 30-question survey about their intraoperative radiation exposure, protective measures, radiation knowledge, and any conditions that can arise from protracted radiation exposure was disseminated to 3344 American Association of Neurological Surgeons members. RESULTS: A total of

Intraoperative awareness--comparison of its incidence in women undergoing general anaesthesia for caesarean section and for gynaecological procedures.

BACKGROUND: Intraoperative awareness (IA) is diagnosed when patients can recall their surroundings or an event related to the surgery that occurred while they were under general anaesthesia. The female gender and Caesarean section are considered to be contributing factors. The aim of the present study was to analyse the frequency of IA in patients undergoing general anaesthesia either for Caesarean section or gynaecological procedures. METHODS: ASA I and II women were included into the study. Patients were randomly allocated to 4 groups: A, B and C included patients qualified for elective gyn

Raising Awareness of Intraoperative Diagnostic Challenges to Prevent Misdiagnosis and Overtreatment: Laparoscopic Management of Rare Cotyledonoid Dissecting Leiomyoma Mimicking Ovarian Tumour.

Cotyledonoid dissecting leiomyoma (CDL), also known as Sternberg tumour, is a rare variant of leiomyoma that can be easily mistaken for a malignant neoplasm on clinical and radiological examination, posing a diagnostic challenge for clinicians. Background: Although the tumour can extend to neighbouring organs, it typically does not invade them and is considered benign. Therefore, it is essential to recognise and differentiate this leiomyoma variant from other malignancies to avoid misdiagnosis and overtreatment. Methods: This report depicts a unique case of CDL misdiagnosed as an ovarian tumou

BIS Versus End-tidal Anaesthetic Gas Concentration-guided General Anaesthesia for Prevention of Intraoperative Awareness in Elective Surgery: A Randomised Controlled Trial.

BACKGROUND: Awareness during general anaesthesia (GA) is a rare but serious complication, affecting 0.15%-0.2% of patients, with potential psychological consequences such as post-traumatic stress disorder (PTSD). Bispectral index (BIS) monitoring, which uses electroencephalography (EEG)-derived metrics to assess anaesthesia depth, has shown promise in reducing intraoperative awareness, though its universal adoption remains limited due to mixed evidence. PURPOSE: This study aimed to compare BIS-guided and end-tidal anaesthetic gas concentration (ETAG)-guided techniques in a randomised controll

Seeing is believing: increasing intraoperative awareness to scattered radiation in interventional procedures by combining augmented reality, Monte Carlo simulations and wireless dosimeters.

PURPOSE: Surgical staff performing image-guided minimally invasive surgical procedures are chronically exposed to harmful ionizing radiation. Currently, no means exist to intraoperatively depict the 3D shape and intensity of scattered radiation fields or to assess the body-part exposure of clinicians. We propose a system for simulating and visualizing intraoperative scattered radiation using augmented reality. METHODS: We use a multi-camera RGBD system to obtain a 3D point cloud reconstruction of the current room layout. The positions of the clinicians, patient, table and C-arm are used to bu

Effect of perioperative benzodiazepine use on intraoperative awareness and postoperative delirium: a systematic review and meta-analysis of randomised controlled trials and observational studies.

BACKGROUND: Benzodiazepine use is associated with delirium, and guidelines recommend avoiding them in older and critically ill patients. Their perioperative use remains common because of perceived benefits. METHODS: We searched CENTRAL, MEDLINE, CINAHL, PsycInfo, and Web of Science from inception to June 2021. Pairs of reviewers identified randomised controlled trials and prospective observational studies comparing perioperative use of benzodiazepines with other agents or placebo in patients undergoing surgery. Two reviewers independently abstracted data, which we combined using a random-effe
