CASE REPORT: Diagnostic and therapeutic considerations in the case of a 57-year-old man with multiple iris cysts in both eyes.
COMMENTS: Small and hidden from view, most primary iris cysts of the iris pigment epithelium remain clinically silent. Larger cysts may be noted on examination, but often remain difficult to visualize due to their location. By providing a high-resolution view of the iris and ciliary body, ultrasound biomicroscopy is a useful adjunct to the clinical examination in distinguishing primary cysts of the iris pigment epithelium from solid uveal neoplasms.

PURPOSE: Metastatic cancer to the iris is rare, and the best approaches to clinical recognition and treatment of this neoplasm are not widely known. We reviewed a series of patients with metastatic cancer to the iris and elucidated the clinical variations, diagnostic approaches, and treatment of this neoplasm.
METHODS: The files of the Wills Eye Hospital Ocular Oncology Service were surveyed, and cases of metastatic cancer to the iris were extracted for further analysis. General data, systemic data, tumor characteristics, diagnostic methods used, treatment, and prognosis were tabulated.

Microcysts of the iris pigment epithelium have been described in association with diabetes mellitus, systemic mucopolysaccharidoses. Menkes's syndrome, and in neonates. Our study covers 68 cases obtained at necropsy. We specifically examined the iris pigment epithelium for vacuolation. We found that microcysts are more widespread than previously thought. In our series 57.3% of the cases reviewed by the light microscope showed microcysts. The authors present their experience with the evaluation and follow-up of 62 patients with primary cysts of the iris, discuss their clinical and pathological features, and propose a simple classification for these lesions. Orbital defects can result from a myriad of underlying diseases and injuries. Treatment of malignant neoplasms and maxillofacial trauma are common reasons for orbital exenteration. Recently, a growing number of cases of orbital defects have been reported due to mucormycosis in patients with COVID-19. In this report, we describe a technique to achieve ideal iris orientation in the fabrication of a silicone orbital prosthesis using custom-made eyeglasses as a jig. This method allows for easy orientation and reorientation, enabling precise iris alignment and enhancing the aesthetic outcome.

Feline diffuse iris melanoma (FDIM) is a common ocular neoplasm, typically affecting older cats, characterized by progressive hyperpigmentation and potential malignancy. This study evaluated the diagnostic value of iris biopsies in distinguishing FDIM from benign iris melanosis. We present the case of a 56-year-old man who developed a neoplasm of epithelioid histology in his anophthalmic left orbit 21 years after he underwent enucleation for a spindle cell iris melanoma. The recurrent tumour was managed by orbital exenteration. Neither further recurrence nor metastasis was diagnosed over a 5-year follow-up period. This case, along with five other similar cases in the literature,1-3 emphasises the importance of long-term follow-up after treatment of iris melanoma.

A neoplasm of the ciliary body extending from the equator to the iris and the anterior chamber was observed in an 18-year old girl. Because the visual acuity was lost and the clinical diagnosis was uncertain the eye was enucleated.
