BACKGROUND: Irreversible electroporation (IRE) is a novel tumor ablative therapy technique, using electric fields to induce apoptosis in target tissues. Whether these electric pulses of high field strength can cause cardiac damage and/or ablation-induced arrhythmias is unclear.
OBJECTIVE: The purpose of this study was to systematically evaluate the safety of electrocardiogram (ECG)-gated IRE with regard to cardiac side effects.
METHODS: In all patients, 12-lead ECG and signal-averaged ECG (SAECG) recordings were performed before and after IRE and 24-hour Holter recording on the day of the IR

Irreversible electroporation, as a nonthermal therapy of prostate cancer, has been used in clinic for several years. The mechanism of irreversible electroporation ablation is thermal independent; thus, the main structures (eg, rectum, urethra, and neurovascular bundle) in prostate are spared during the treatment, which leads to the retention of prostate function. However, various clinical trials have shown that muscle contractions occur during this therapy, which warrants deep muscle anesthesia. Use of high-frequency bipolar pulses has been proposed to reduce muscle contractions during treatme

The treatment of prostate cancer (PCa) requires approaches that eliminate malignant tissue while preserving essential urinary and sexual functions. Irreversible electroporation (IRE) represents a targeted ablative strategy that disrupts cellular membrane integrity through precise electrical pulses, demonstrating the capacity to ablate cancerous foci while sparing adjacent neurovascular bundles and sphincter mechanisms. This structured review examines the mechanistic foundations of IRE technology, evaluates current clinical evidence regarding safety and efficacy, and explores promising applicat

BACKGROUND: The effectiveness of electrochemotherapy of tumors (ECT) and of irreversible electroporation ablation (IRE) depends on different mechanisms and delivery protocols. Both therapies exploit the phenomenon of electroporation of the cell membrane achieved by the exposure of the cells to a series of high-voltage electric pulses. Electroporation can be fine-tuned to be either reversible or irreversible, causing the cells to either survive the exposure (in ECT) or not (in IRE), respectively. For treatment of tissues located close to the heart (e.g., in the liver), the safety of electropora

Here, we present a precision oncologic approach for localized prostate cancer (PCa) in a 66-year-old man. Overcoming the limitations of conventional MRI in delineating tumor extent and multifocality, preoperative 68 Ga-PSMA PET/MRI precisely defined the index lesion and excluded metastases. This diagnostic workflow was directly translated into therapy through PET/MRI-US fusion-guided irreversible electroporation (IRE). The procedure targeted the tumor accurately while sparing surrounding functional anatomy. Significant PSA and phi decline post-IRE confirmed therapeutic efficacy. This case demo

BACKGROUND: Irreversible electroporation (IRE) is a non-thermal ablation technique using high-voltage, low-energy pulses to induce cell membrane perforation and cell death. As an emerging therapy, IRE has gained increasing application in local tumor treatment, with Nanoknife being the most widely used device. Despite its relative safety compared to traditional therapies, potential adverse reactions still merit the attention of clinicians.
METHODS: We analyzed all IRE-related adverse event reports in the Manufacturer and User Facility Device Experience (MAUDE) database, focusing on event types

Chimeric antigen receptor natural killer (CAR-NK) cell therapy has emerged as a promising immunotherapeutic modality with potent cytotoxicity and a favorable safety profile. However, its therapeutic efficacy is often limited by poor infiltration into tumors and the profoundly immunosuppressive tumor microenvironment (TME). In hepatocellular carcinoma (HCC), one of the leading causes of cancer-related mortality, this suppressive TME severely compromises the function of CAR-NK cells. To overcome this limitation, we developed a combinatorial strategy integrating irreversible electroporation (IRE)

Progression of transitional cell carcinoma (TCC) in dogs often leads to urinary obstruction. This observational pilot study aimed to evaluate the safety and efficacy of irreversible electroporation (IRE) balloon therapy for the palliative treatment of TCC with partial urethral obstruction. Three client-owned dogs diagnosed with TCC causing partial urethral obstruction were enrolled. After ultrasonographic and cystoscopic examination, IRE pulse protocols were delivered through a balloon catheter device inflated within the urethral lumen. After the procedure, the patients were kept overnight for

High-frequency irreversible electroporation (H-FIRE) is a minimally invasive local ablation therapy known to activate the adaptive immune system and reprogram the tumor microenvironment. Its predecessor, irreversible electroporation (IRE), transiently increases microvascular density and immune cell infiltration within the surviving non-ablated and non-necrotic tumor region, also known as the viable tumor region. However, the impact of pulse electric field therapies on lymphatic vessels, crucial for T-cell fate and maturation, remains unclear. This study investigates how sub-ablative H-FIRE (SA

Irreversible electroporation (IRE), a non-thermal focal therapy, employs high-frequency electrical pulses to create tumor cell membrane nanopores, inducing immunogenic cell death. By triggering damage-associated molecular patterns, IRE activates dendritic cells (DCs) to prime systemic cytotoxic T lymphocyte (CTL) responses. However, IRE-activated CTLs predominantly accumulate in perivascular regions, limiting their infiltration into the tumor parenchyma. Oncolytic viruses (OVs) complement IRE by converting the "primed" but spatially restricted CTL response into a robust, intratumoral immune at

Electroporation has birthed two massive parallel universes of applications: medicine and food processing. Despite operating on the same fundamental physics, these fields fail to crosspollinate, limiting their advancement. The classical "aqueous pore" model and threshold-based definitions fail to explain the highly complex, multiparameter realities of molecular transport, cell death mechanisms, and tissue-level physiological responses. The fields often struggle to translate in vitro findings-where even varying cell lines yield conflicting results-into predictive clinical and industrial outcomes

Focal treatment has emerged as a precision-oriented alternative to whole-gland treatment approaches for localized prostate cancer. It aims to selectively destroy clinically significant tumor foci while preserving surrounding structures that are essential for urinary continence and sexual function. Irreversible electroporation (IRE) is a relatively new and increasingly investigated focal treatment modality. By delivering short high-voltage pulses, IRE creates permanent nanopores in cell membranes and induces cell death predominantly through non-thermal mechanisms. This theoretical advantage - ti

Pulsed-Field Ablation (PFA) is a non-thermal technique for the treatment of cardiac arrhythmias. Predictive numerical models capable of reliably predicting lesion size and shape remain limited. We introduce a biophysics-grounded and inherently anisotropic model for cardiac PFA, derived by homogenization of a microscopic electroporation model. Unlike standard approaches relying on nonlinear tissue conductivities and macroscopic electric-field-based ablation criteria, our framework naturally incorporates cellular geometry and orientation through the local transmembrane voltage, which provides a

Electroporation has evolved from a membrane-permeabilization method into a versatile therapeutic platform for intracellular delivery, locoregional tumor intervention, and bioelectrically regulated treatment. Depending on pulse intensity and duration, electroporation operates in two distinct modes: reversible electroporation (RE), which transiently permeabilizes the plasma membrane to enable delivery of nucleic acids, proteins, and small molecules while preserving cell viability, and irreversible electroporation (IRE), which causes permanent membrane damage for non-thermal tissue ablation. BACKGROUND: Electroporation-Based Treatments and Therapies (EBTTs) - electrochemotherapy, irreversible electroporation, pulsed field ablation, and gene electro-transfer - use short, high-voltage electric pulses to permeabilize cell membranes, enabling non-thermal tissue ablation and enhanced local drug delivery. Although EBTTs have been used clinically for over three decades, primarily in oncology, their adoption remains limited compared to thermal ablation modalities. In contrast, cardiac pulsed field ablation (PFA) has seen rapid adoption since the first FDA device approval in December 2023.
