Isaacs syndrome: A review.

Isaacs syndrome is a peripheral nerve hyperexcitability (PNH) syndrome that presents as continuous motor activity. Clinical findings include cramps, fasciculations, and myokymia. Electrodiagnosis plays a key role in diagnosis by demonstrating after-discharges on nerve conduction studies, and fasciculation potentials, myokymic discharges, neuromyotonic discharges, and other types of abnormal spontaneous activity on needle examination. Etiopathogenesis involves the interaction of genetic, autoimmune, and paraneoplastic factors, which requires a broad-ranging evaluation for underlying causes. Ini

Isaacs' syndrome in pregnancy.

Isaacs' syndrome is a rare neuromuscular disorder of continuous muscle fibre activity resulting from peripheral nerve hyperexcitability. Symptoms commonly include myokymia (muscle twitching at rest), pseudomyotonia (delayed muscle relaxation), muscle cramps and stiffness. It is caused by voltage-gated potassium channel dysfunction and may be inherited or acquired. Treatment commonly includes anticonvulsants, immunosuppressive therapy and plasma exchange. To date only two cases of Isaacs' syndrome in pregnancy have been reported. We present a case of maternal Isaacs' along with a review of the

Isaacs syndrome associated with polyarteritis nodosa.

Isaacs syndrome (IS) is a rare peripheral nerve hyperexcitability disorder, often associated with an underlying disease. We report a clinical case which appears to be a previously unreported co-occurrence of IS and systemic polyarteritis nodosa, diagnosed in a young woman presenting with subacute back pain, weight loss, fasciculations, diaphoresis, tachycardia, micturition disorder, constipation, cutaneous lesions and mesenteric vasculitis on MRI. The patient achieved clinical remission following treatment with intravenous cyclophosphamide and corticosteroids.

[Isaacs' syndrome. Report of three cases].

We report two females, and one male with Isaacs' syndrome. The patients presented with clinical myokymia activity, muscle cramps, delayed relaxation, and muscle hypertrophy and increased sweating. Needle electromyography in several muscles showed generalized continuous motor unit discharges, myokymic discharges, and normal nerve conduction studies. Muscle biopsy showed type two fiber atrophy. Treatment with carbamazepine was effective in two cases and prednisone in one.

[Isaacs syndrome: report of a case].

The authors report a case of Isaacs syndrome observed in a 40 years old woman. The clinical picture included progressive muscular weakness, dysphagia, dysphonia, dyspnea and increased perspiration. Fasciculations, facial myokymia and pseudomyotonia were observed on physical examination. Electromyographic study at rest revealed continuous electrical muscle activity. Muscle histochemistry showed type II fibers atrophy and an ultrastructural study of the gastrocnemius muscle disclosed marked cysternal dilatation of the sarcoplasmatic reticulum. An excellent clinical response was observed with the

Autoimmune aetiology for acquired neuromyotonia (Isaacs' syndrome).

Neuromyotonia is a rare disorder of unknown cause in which hyperexcitability of peripheral motor nerves leads to incapacitating muscle twitching, cramps, and weakness. We investigated an antibody-mediated mechanism for neuromyotonia in a 24-year-old man with a 7-year history of severe disease unresponsive to pharmacological treatment. Two periods of plasma exchange each produced almost complete disappearance of symptoms for 2-3 weeks, and a highly significant decrease in recorded neuromyotonic discharges. Injection of the patient's plasma or purified IgG into mice significantly enhanced in-vit

Late-onset seropositive Isaacs' syndrome after Guillain-Barré syndrome.

Acquired neuromyotonia, or Isaacs' syndrome, has been described in combination with a variety of other autoimmune disorders; however there has never been a report of seropositive Isaacs' syndrome in a patient with a history of Guillain-Barré syndrome (GBS). Both conditions involve antibody-mediated autoimmune effects on the peripheral nervous system, although the clinical manifestations are quite different. We present a man who experienced an episode of GBS at the age of 21 and subsequently developed Isaacs' syndrome at the age of 24 which was positive for anti-voltage-gated potassium channel

[Case of Isaacs' syndrome associated with Hashimoto disease].

We report a case of Isaacs' syndrome associated with Hashimoto disease. A 26-year-old woman, who had a past history of Hashimoto disease, complained of involuntary movements and muscle cramp in lower extremities. On examination, myokymia was seen in lower extremities. Myokymia was observed even during sleep, and worsened by exercise or bathing. The antibody against voltage-gated potassium channel (VGKC) was positive. Myokymic discharges were recorded with needle EMG in lower extremities. The patient was diagnosed as having Isaacs' syndrome. Isaacs' syndrome tends to be associated with some oth

Finger flexion resembling focal dystonia in Isaacs' syndrome.

We describe a patient with a 5-month history of gradually progressive painless flexion of the left ring finger associated with cramps in both thighs. She has severe chronic obstructive pulmonary disease and was on salbutamol. Serum anti-voltage-gated potassium channel antibodies was positive. Electromyography showed generalized neuromyotonia and myokymic discharges. The cramps were partially relieved by phenytoin. We would like to highlight that finger flexion resembling dystonia can be a presenting sign of Isaacs' syndrome.

Isaacs' syndrome associated with chronic inflammatory demyelinating polyneuropathy.

We report the first case of Isaacs' syndrome in which an inflammatory demyelinating neuropathy was documented histologically. For 9 months, the patient developed slowly progressive weakness, muscle spasms and stiffness, fasciculations, and myokymia in the arms, which were unmodified by sleep. Nerve conduction studies showed multifocal motor conduction block, abnormal dispersion phenomenon, and abnormal sensory and mixed nerve conduction. Needle electromyogram showed continuous motor unit potentials at rest with bursts of rapid-firing discharges which were unaffected by spinal anesthesia but di

[Isaacs syndrome: long-term improvement with intravenous polyvalent immunoglobulins].

A 35 year-old man developed a syndrome with muscle cramp, myokimia, generalized, fasciculations, excessive sweating, sleep disorders and severe impairment. It was a syndrome of continuous muscle fiber activity--or Isaacs syndrome--with central disorders (this may be called "Maladie de Morvan"). Previous reports have suggested that Isaac's syndrome might be an autoimmune disorder. Moreover, high doses intravenous immunoglobulins were given resulting in a substantial improvement six months after the onset of this treatment.

Isaacs' syndrome with muscle hypertrophy reversed by phenytoin therapy.

A 16-year-old boy was seen for severe episodic muscle cramps and generalized myokymia, consistent with Isaacs's syndrome. Bilateral calf hypertrophy (46-cm calf circumference) and ankle areflexia were noted. He was treated with phenytoin sodium, 300 mg/day. Within three months there was marked decrease in myokymia, total relief of cramps, return of ankle reflexes, and 6-cm reduction in calf circumference. We suggest that the excess muscle activity in Isaacs's syndrome may be responsible for the associated phenomena of muscle hypertrophy and areflexia.

Thymoma in the offspring of a patient with Isaacs syndrome.

Thymoma is a diagnosis most often made in adults, and is exceedingly rare in children. The diagnosis of a thymoma is often times linked to an autoimmune process, and the association of thymoma and myasthenia gravis in children has been previously reported. We present the first description of a child of a parent with Isaacs syndrome (congenital or acquired neuromyotonia) presenting with a thymoma. In our patient, an evaluation of underlying disease led to a presumptive diagnosis of congenital Isaacs syndrome. This case demonstrates that for the rare child diagnosed with thymoma, evaluation for
