Volkmann ischemic contracture in a newborn.

Congenital Volkmann ischemic contracture is a very rare condition in which a neonate presents skin, muscular and nerve lesions due to increased intracompartment pressure and subsequent ischemia, probably due to extrinsic intrauterine compression. In this age group, there are only about 50 reported cases and a specific cause is unknown. The authors describe the case of a newborn who presented with bullous and ulcerated skin lesions and nerve palsy of his forearm at birth, evolving to subcutaneous and muscular necrosis and contracture. Two surgeries were performed and the baby began a daily phys

Ischemic contracture in the lower limb.

Twenty cases of ischemic contracture in the lower limb demonstrate that the age distribution may be higher than that of patients with similar lesions in the upper limbs. Non-traumatic and traumatic causes are encountered equally frequently. Pain is one of the most important symptoms associated with long standing lesions. The ischemic lesion develops most frequently and severely in the anterior compartment of the leg. The functional prognosis is poor when both nerves are affected and when the deformity is severe.

Congenital Flexor Contracture of Ulnar Digits Masquerading as Volkmann Ischemic Contracture.

Congenital flexion contracture of ulnar digits is a rare entity with few cases reported in medical literature. This condition is often misdiagnosed as Volkmann ischemic contracture as both have similar presentation. The patient history, physical examination, radiological investigation, and intra-operative findings can differentiate these 2 conditions clearly. A 14-year-old female presented to a tertiary care hospital with flexion deformity of the left long, ring, and little fingers without neurological deficit since she was 3 years old. Patient had decreased handspan with difficulty in graspin

Nerve Lesions in Volkmann Ischemic Contracture.

Volkmann ischemic contracture (VIC) is a late sequela of acute compartment syndrome and consists of extensive muscle necrosis, fibrosis, contracture, and variable degrees of neural dysfunction. The outcome depends on successful restoration of muscle and neural function. The timing of surgery is often determined by the development of contractures and is delayed in the interest of observing some spontaneous recovery and infarct maturation. This period of observation may be detrimental to nerve function with gradual formation of scar tissue and worsening constrictive neuropathy. Early interventio

Congenital Contracture of the Ulnar Digits and Its Differentiation From Ischemic Contracture.

PURPOSE: To describe the clinical features, radiologic findings, differential diagnosis, and surgical treatment of a congenital flexion deformity of the middle, ring, and little fingers. The cause of the condition is the aberrant origin of the flexor digitorum profundus, leading to a congenital contracture of the ulnar digits. METHODS: We reviewed 8 patients with congenital contracture of the ulnar digits. The mean age at the time of surgery was 14 years. An examination revealed a flexion contracture of the middle, ring, and small fingers. Plain radiographs, 3-dimensional computed tomography,

Volkmann Ischemic Contracture Due to Traditional Practice.

An 11-year-old boy presented with three days' history of blunt trauma to the left shoulder due to a fall to the ground. The boy was taken to the traditional practitioner, who applied a very tight bandage over a piece of cartoon on the upper left humerus, which resulted in progressive pain and swelling of the whole left upper limb and impairment of movement and sensation. The tight bandage was removed after 48 h and fasciotomy was performed at Aljamhuri Hospital in Taiz, Yemen. The child was brought to our hospital after the family was informed about the need for left forearm amputation. We per

Congenital Volkmann ischemic contracture: a case report.

Congenital Volkmann ischemic contracture is a rare condition in which a neonate presents with an ulcerated forearm, necrotic subcutaneous tissue, nerve palsy, and muscular necrosis. We report a case of a 1-day-old infant who presented with an ulceration of the left forearm noted at delivery combined with decreased motor function of the left distal forearm. Histologic examination of the ulceration revealed findings of subcutaneous fat necrosis of the newborn. To our knowledge, this is the first case for which the findings of subcutaneous fat necrosis have been seen histologically.

Volkmann's ischemic contractures and blowout fractures.

Volkmann's ischemic contractures have long been recognized by orthopedic surgeons as a sequela of increased pressure within osseofascial muscle compartments. We present evidence that the same mechanism is a cause of fibrosis and contracture of extraocular muscles following orbital blowout fractures. Surgical treatment of a specific, recognizable type of blowout fracture is proposed.

A Case of Volkmann's Ischemic Contracture - A Case Report.

INTRODUCTION: The Volkmann's ischemic contracture (VIC) of the forearm is a sequel of the compartment syndrome of the forearm. We present a case of VIC treated by the Max page operation with a good functional outcome. CASE REPORT: We report a case of 14-year-old boy with fingers flexion contracture, decreased hand mobility and pain on his left hand. He had a history of left forearm fracture and that was treated by a traditional bonesetter with herbs and cardboards 6 months back. Later he started to have pain and deformity of left hand for which they came to the BGS Global Institute of Medical

Functional reconstruction of ischemic contracture in the lower limb.

OBJECTIVE: To discuss the method of functional reconstruction of ischemic contracture in the lower limb and propose a classification protocol for ischemic contracture in the lower limb based on its severity and prognosis. METHODS: A total of 42 patients with ischemic contracture in the lower limb were included in this study. According to different types of disturbance and degrees of severity, surgical reconstructions consisting of nerve decompression, tendon lengthening or transfer, intrinsic foot muscle release and sural-tibial nerve anastomosis were performed in every patient. RESULTS: Pos

Volkmann's ischemic contracture due to soft tissue injury alone.

Fifteen upper extremities, in 14 patients in whom incipient or actual Volkmann's ischemic contracture was present, were seen in a 5 year period. Nine patients were stuporous due to drug overdose and had laid on the extremity; two had received a recent injury of main arterial trunks; two had sudden severe compression; one with chronic myelogenous leukemia had each arm involved at different times in a bizarre autoimmune response causing massive swelling. No patient had a fracture or dislocation. Pain and tenderness, loss of sensibility, resistant muscle contracture, and rock-hard muscle compartm

Congenital Volkmann-Lesser ischemic contracture of the upper limb.

Neonatal vascular compromise to limbs has been associated traditionally with perinatal injury of the brachial plexus, fracture of the clavicle or humerus, or iatrogenic causes. Congenital Volkmann's ischemic contracture is an exceptional etiology of ischemic limb in the newborn. Fewer than 10 cases had been described in the literature. The authors report a newborn presenting at birth with partial bluish discoloration of the right forearm. The clinical picture and laboratory studies lead to the diagnosis of congenital Volkmann-Lesser ischemic contracture.

[Clinico-roentgenological characteristics of the changes in ischemic Volkmann's contracture].

In the article is presented the study of clinical aspects and roentgenograms of 91 children with Volkmann's contraction. The causes of contraction with 46 patients were supracondylar fractures of arm and with 32--forearm bone fractures. All fractures were associated with massive injures of soft tissues, some of the persons have injures of brachial artery. Tardy or incompletely dissected circular plaster bandage turned out to be a factor of complication. All patients demonstrated typical clinical aspects of acute circulatory disturbance of extremity, in some of them with skin and subcutaneous f

Ischemic contracture of the foot and ankle: principles of management and prevention.

A variety of clinical presentations can be encountered following compartment syndrome of the leg and foot. Deformity and functional impairment in the foot and ankle secondary to ischemia are determined by: 1) which leg compartments have been affected and to what degree extrinsic flexor or extensor "overpull" is exhibited, 2) degree of nerve injury sustained causing weakness or paralysis of extrinsic or intrinsic foot and ankle muscles, 3) which foot compartments have been affected and to what degree intrinsic "overpull" is exhibited, and 4) degree of sensory nerve injury leading to anesthesia,

[Ischemic contractures of muscle and nerve lesions (author's transl)].

Reporting 14 own cases symptomatology and treatment of the common ischemic syndromes of the extremities (Volkmann's contracture, thumb adduction contracture respectively contracture of the intrinsic hand muscles and anterior tibial syndrome) including the regularly concomitant nerve lesions are discussed. Edema and compression beyond the primary ischemia are essential factors in pathogenesis of nerve and muscle lesions. The electromyographic examination is helpful in diagnosis, prognosis and treatment of the severe sequelae of nerve and muscle. Since late diagnosis yields poor therapeutical re

Management of Volkmann's Ischemic Contracture: Case Series of 32 patients.

INTRODUCTION: Volkmann's contracture condition is of high prevalence in our population and is linked to therapeutic faults. The treatment and its results are determined according to the severity of the lesions. METHODS: This retrospective study was performed in three centers and was conducted over 30 years (1987-2018); it included 32 patients. The disabilities of the arm, shoulder and hand (DASH) score and the Weber test were used to evaluate the functional outcome looking at mid and long-term results. RESULTS: Thirty-two patients were treated for Volkmann's Ischemic Contracture (VIC). The a
