Ehrlichiosis in Kentucky.

Ehrlichiosis is a recently described tick-borne disease characterized by headache, fever and chills, leukopenia, thrombocytopenia, and transaminase elevation. The presentation is similar to Rocky Mountain spotted fever but the rash is less frequently present in ehrlichiosis. The diagnosis is confirmed by elevated antibody titers to Ehrlichia chaffeensis or visualization of inclusions in white blood cells and the disease is easily cured with tetracyclines or chloramphenicol. Cases of ehrlichiosis are reported from southeastern, southcentral and midatlantic states. We report three cases of Ehrli

Histoplasmosis--Kentucky, 1995.

Histoplasmosis is an infection resulting from inhalation of spores from the dimorphic fungus Histoplasma capsulatum; the condition primarily affects the lungs. During August 1995, the Department for Health Services, Kentucky Cabinet for Human Resources (KDHS), and local public health officials investigated two unrelated outbreaks of acute histoplasmosis in eastern Kentucky. This report summarizes preliminary findings of the investigations of these outbreaks.

Pulmonary coccidioidomycosis in Kentucky.

Coccidioidomycosis is a highly infectious disease caused by the dimorphic fungus, Coccidioides immitis that is endemic to the arid and semiarid regions of the southwestern United States, Mexico, Central America, and South America. The majority of infections from C immitis are asymptomatic; however, approximately 40% of infected individuals present with symptoms ranging from a mild flu-like respiratory infection to acute pneumonia that may lead to chronic progressive pulmonary infection or occasionally disseminated disease. Due to the mobility of the population, increasing numbers of cases are

Sarcocystosis in cattle in Kentucky.

Sarcocystosis was diagnosed in 41 eighteen-month-old heifers and steers. Clinical signs included anorexia, severe weight loss, nervousness, hypersalivation, lameness, and hair loss on the extremities. Hair loss was noticed especially at the end of the tail, where there was complete loss of the switch, giving the animals a "rat-tail" appearance. Consistent gross changes observed at necropsy of four affected animals included generalized lymphadenopathy, erosions and ulcerations in the oral cavity and esophagus, and severe laminitis. Microscopically, young cysts of Sarcocystis sp were disseminate

Human rabies - Kentucky/Indiana, 2009.

On October 19, 2009, clinicians from Kentucky contacted CDC regarding a suspected case of rabies in a man from Indiana aged 43 years. This report summarizes the patient's clinical presentation and course, the subsequent epidemiologic investigation, and, for the first time, provides infection control recommendations for personnel performing autopsies on decedents with confirmed or suspected rabies infection. Before the patient's death on October 20, a diagnosis of rabies was suspected based on the history of acute, progressive encephalitis with unknown etiology. Preliminary serology results on

Parental vaccine concerns in Kentucky.

BACKGROUND: An increasing number of parents are questioning the safety and necessity of routine childhood immunizations. Locally produced vaccine risk communication materials may be effective in reassuring these parents. However, little is known about specific vaccine safety concerns in the state of Kentucky. METHODS: An Internet-based survey focusing on parental vaccine safety concerns and potential vaccine risk communication strategies was sent to all members of the Kentucky Chapter of the Amerian Academy of Pediatrics. RESULTS: There were 121 respondents who routinely administered childho

Maduromycosis (Madura foot) in Kentucky.

The first case of proven maduromycosis originating in the state of Kentucky, and the 21st case reported in the United States, is described. The disease, which involved the foot, was caused by Monosporium apiospermum. The importance of suspecting and culturing for fungal organisms in cases of chronic or recurrent infections of the extremities is stressed. The method of treatment was experimental; whether true cure was accomplished may not be known for years. Although many fungi, including Monosporium apiospermum, are relatively resistant to all forms of chemotherapy, some types of fungi readily

Oral health status of older adults in Kentucky: results from the Kentucky Elder Oral Health Survey.

The purpose of the Kentucky Elder Oral Health Survey (KEOHS) was to assess the oral health status of Kentuckians 65 and older. The KEOHS consisted of a self-administered questionnaire and a clinical examination. Recruitment occurred from May 2002 through March 2005 of persons aged 65 and older (n = 1,386) whose functional ability was classified by residential setting. Independent elders living in their own homes were designated as "well-elders," those who lived in skilled nursing facilities and who were functionally dependent were designated as "nursing home elders," and those older adults who

Injuries to tobacco farmers in Kentucky.

Like all types of farming, raising tobacco has many hazards that may lead to injury, disability, or death. Burley tobacco farming is a multistep process that is not highly mechanized. An 8-year emergency department surveillance system in Kentucky identified 674 injury cases related to the production of burley tobacco. Most of the injuries were a result of a fall, cutting and piercing instruments, or sprains and strains. More than 90% of the injured patients were treated as outpatients, and the average charge for emergency room services was $402 (range, $0 to $14,729). One quarter of the patien

Rabies postexposure prophylaxis survey--Kentucky, 1994.

A survey of rabies postexposure prophylaxis administered by local health departments for a 1-year period showed that very few patients received treatment as a result of exposure to a confirmed rabid animal. Most prophylaxis was administered for contact with domestic animals in situations where existing recommendations for quarantine or laboratory testing of the animal were not followed. Because rabies in domestic animals in Kentucky is uncommon, these findings suggest that had the existing recommendations been followed, the prophylaxis would have been unnecessary in most cases.

Shigellosis in Kentucky, 1986 through 1989.

A marked increase in the number of cases of shigellosis was reported in Kentucky in 1988. To examine reasons for this increase, we reviewed the 389 cases of shigellosis reported from 1986 through 1989. Ninety-three percent (285/305) of cases due to known species were due to Shigella sonnei. Sixty-two percent (243/389) of cases occurred in children less than 10 years of age. The annual incidence for blacks (6.8 cases per 100,000) was nearly four times that for whites (1.8 per 100,000). The increased number of cases in 1988 was due to outbreaks in five areas of the state, which accounted for 85%

Myodegeneration in Kentucky white-tailed deer.

Skeletal or cardiac myodegeneration, or both, was observed in 4 free-ranging female white-tailed deer that were found dead or recumbent and unable to rise. Gross pathologic findings included white or chalky streaks in heart or skeletal musculature. Degeneration, necrosis, and mineralization were observed microscopically in affected striated muscles. Although the precise cause of myodegeneration was not determined, changes were considered compatible with nutritional or exertional myopathy.

Trismus pseudocamptodactyly syndrome: Dutch-Kentucky syndrome.

A hand, foot, and mouth combination of anomalies, which is apparent on motion, has been traced through eight generations of a southern Appalachian family. The clinical features from birth to old age are presented, and attention is called to the handicapping aspects. There is a great variability in severity of the trait. Inheritance occurs as an autosomal dominant trait, but there are fewer affected persons than expected, especially females.

Nonketotic Hyperglycinemia in Central and Appalachian Kentucky.

Nonketotic hyperglycinemia (NKH) is a rare genetic disorder with a global incidence of 0.4 to 1.3 per 100 000 live births with regional variation. We sought to estimate the regional birth incidence and describe the clinical and genetic features of NKH in central and Appalachian Kentucky. Fifteen patients met standard diagnostic criteria for NKH; 13 were born in Kentucky, yielding an estimated birth incidence of 2.53 per 100 000 live births (13/513 419; 95% CI 1.35-4.33). Of those with available genetic data (n = 9), 8 carried GLDC variants, with 6 having the c.1166C>T (p.A389V) variant. We fou

Blastomycosis: a reminder of Kentucky's other fungus.

Although not as common as Histoplasmosis, the disease caused by Blastomyces dermatitidis is also endemic in the Commonwealth of Kentucky. Greater clinical awareness of this systemic mycosis and the newer effective forms of treatment may lessen the significant morbidity and mortality of this potentially serious infection. To illustrate the varied manifestations of this illness, we contrast the presentation of an urban female with blastomycosis and atypical chest radiographic changes to the more classic features of blastomycosis described in the literature.

Clinical study of keratoconus in central Kentucky.

Keratoconus is an ectatic corneal disorder that results in a central or inferior conelike anterior protrusion. This results in irregular myopic astigmatism, causing considerable visual impairment. We have completed a retrospective clinical study of keratoconus in the central Kentucky area. When compared to other national studies of keratoconus, our study patients generally had more severe disease at the time of diagnosis and had a higher rate of keratoplasty. Keratoconus treatment is highly successful, and early diagnosis is warranted to prevent unnecessary visual loss.

Trichinosis in sows slaughtered at a Kentucky abattoir.

From July 26 to 30, 1976, diaphragmatic pillar samples were collected from 513 sows slaughtered at a federally inspected abattoir in Kentucky. The sows represented 5 marketing areas: Rushville, In (186); Cincinnati, Oh )158); Indianapolis, In (70); Georgetown, Ky (54); and Columbus, Oh (45). Using the pooled-sample digestion technique, 3 dead trichina larvae were identified in 1 sample pooled from 15 diaphragms (10 g each). An attempt to identify the individually infected sow(s) was unsuccessful, but the pooled sample originated from animals marketed at Rushville, In. The prevalence (1 of 513)
