[Extracorporeal shockwave lithotripsy of stones in lower calices of kidney].

The article presents the results of the study aimed to evaluation of possible relationship between anatomical structure of the renal pelvis of the kidney, the size of the stone and the effectiveness of extracorporeal shockwave lithotripsy (ESWL) of stones in lower calices of kidney, defined as "stone-free state". ESWL was performed in 285 patients. Sizes of stones varied from 5 to 25 mm. With interval distribution of stone sizes, the greatest number of cases was detected with size of 5 to 12 mm. The destruction of stone required one ESWL session in 196 cases, and three sessions only in 12 case

Late caliceal fistula after kidney transplantation.

Caliceal fistula is a rare urological complication that can occur usually shortly after kidney transplantation (KTx). The occlusion of the renal accessory artery with subsequent necrosis of the kidney pole is the most common cause of the fistula development. We report a case of a 57-year-old man with reconstruction of two accessory renal arteries by anastomosis to the side of the main artery during graft placement complicated by late caliceal fistula, managed surgically. Directly after KTx good kidney graft function (serum creatinine concentration 151 micromol/L) was observed. The patient noti

[Intra- and postoperative complications in interventions of the kidney, kidney calices and upper ureter].

Open routine operations on the kidney and the renal calyces have diminished in favour of endourological methods. Renal and renal pelvis calculi as well as ureteral calculi are rarely approached through open operation. Selective indications for operation depend on special situations and anatomy, since complications such as injury to large abdominal wall nerves, the peritoneum or the pleura present no problem for the skilled surgeon. The control of a sudden and serious hemorrhage coming from the renal hilus or the vena cava surely is an important complication, which should not be underestimated,

[Extravasation of contrast medium from kidney calices in overfilling of the urinary bladder].

The most frequent cause of contrast media extravasation from the renal pelvis is a nephrourolithiasis. There are frequent causes enumerated in the literature, that are also characterized by a mechanical obstruction of the contrast media excretion. The presented case reports are characterized by a technical dependency, namely a long angiographic investigation and an overfilling of the urinary bladder without any indication of an organical excretory impediment. The clinical consequence of such observations should be discussed. 150 ml contrast media should not be exceeded during one examination.

[Changes in the system of kidney pelvis and calices in bladder neck adenoma].

The development of a renal lesion following an adenoma of the vesical neck is one of the most important complications of this disease. For the determination of the frequency of changes of the renal calyces in the adenoma of the vesical neck the findings of 253 patients of the years 1976 and 1977, in whom for the first time such an excrescence was established were evaluated. 135 excretion urogrammes showed changes of the renal pelvis and the renal calyces. The main findings were a narrowing of the renal calyces with spindle-shaped necks of the calyces. The calyces themselves were deformed only

[Transformation of the kidney pelvis and the major and minor calices during their development].

The investigation has been performed in 28 series of histological sections of human embryos and prefetuses (4-20 weeks of the intrauterine development) and in 17 fetal corpses by means of certain histological methods, preparation of graphic reconstructive models, microscopy and fine preparation. The structural form of the definitive organ renal pelvis is demonstrated to depend on the ramification type of the metanephric duct derivatives. This is stipulated by interinductive processes of the ureteral sprout and metanephrogenic tissue. Quantitative and qualitative characteristics of the divertic

[Ultrasonography of pregnancy-induced dilatation of maternal kidney calices in a normal patient sample].

To examine the physiological changes of the calyceal system during pregnancy, we selected a normal group of 187 pregnant women by anamnestic, clinical, ultrasonic and laboratory findings. The sonographic investigation of renal pelvis, renal calyces and ureters revealed a more frequent and more pronounced dilatation of the right side. The dilatation was more marked as expected by literature. The complete involution post partum occurred mostly during the first ten days. Even after marked dilatations, a parenchymal reduction could not be found.

Case report: transmesocolic laparoscopic reconstruction of ureteropelvic junction obstruction in pelvic kidney associated with extrarenal calices.

Extrarenal calices are an uncommon congenital anomaly that may occur in an ectopic kidney. Rarely, they are associated with ureteropelvic junction obstruction. We report the first such case managed laparoscopically using a transperitoneal, transmesocolic approach.

Caliceal diverticulum with ureteropelvic junction obstruction in a dysplastic kidney: a pediatric case report.

A caliceal diverticulum is a rare entity in children. Its etiology is closely associated with that of ureteropelvic junction malformations and renal dysplasia. We herein present a case of these complex disorders in an infant. A renal cyst and hydronephrosis were found in the left kidney during the fetal period. The postnatal diagnosis was hydronephrosis due to ureteropelvic junction obstruction and a caliceal diverticulum in the left dysplastic kidney. Although left renal function was severely decreased, the patient had no symptoms. Therefore, we did not perform surgical treatment. At the time

Sepsis complicated by brain abscess following ESWL of a caliceal kidney stone: a case report.

A 47-year old, Caucasian man underwent extracorporeal shock wave lithotripsy (ESWL) of a 14mm calcium stone in the right renal pelvis, without urinary tract obstruction or sepsis. 24 hours after ESWL septic shock occurred and the patient was admitted to the Intensive Care Unit (ICU). Escherichia coli emerged from the blood and urine culture. The patient developed acute renal failure and it was necessary to start a continuous renal replacement therapy (CRRT). Infection was successfully treated, patient recovered renal function and an improvement of general condition occurred. The patient was th

A veno-caliceal fistula related to ureteric stricture in a kidney allograft masquerading as renal failure.

We report an unusual case of veno-caliceal fistula that developed because of high ureteric pressure caused by graft ureteric stricture after kidney transplantation in a 60-year-old patient. We further confirmed its presence with radiological images. Recirculation of creatinine and other uremic toxins resulted in a biochemical picture of renal failure in the presence of normal kidney function, confirmed by normal scintigraphy findings. Drainage of the pelvi-caliceal system could not be assessed accurately by means of diuretic renogram using technetium-99m diethylenetriaminepentaacetic acid with

Laparoscopic surgery for renal urolithiasis: pyelolithotomy, caliceal diverticulectomy, and treatment of stones in a pelvic kidney.

Laparoscopic techniques may be considered for the uncommon patient in whom open stone surgery is considered. Stones in caliceal diverticula not accessible percutaneously may be approached transperitoneally. Pelvic stones in ectopic kidneys can be retrieved laparoscopically through a pyelotomy created with scissors or a knife. Percutaneous nephrolithotomy under laparoscopic guidance can be used if the stone extends beyond the renal pelvis. In the patient with difficult stone disease, laparoscopy does not replace other minimally invasive techniques but rather complements them. It should be the r

Retrograde intrarenal surgery for stones associated with renal anomalies: caliceal diverticulum, horseshoe kidney, medullary sponge kidney, megacalycosis, pelvic kidney, uretero-pelvic junction obstruction.

PURPOSE OF REVIEW: Nowadays, due to the increase of imaging diagnosis, we identify easily renal anomalies, and we can choose between a wide range of armamentarium to treat symptomatic stones in those challenging cases. However, there is a lack of evidence and consensus on its use. The aim of this narrative review is to collect all the available data about safety and efficacity of retrograde intrarenal surgery (RIRS) in the treatment of kidney stones associated to a renal anomaly. RECENT FINDINGS: Renal anomalies are uncommon findings and even more if it has to be associated with renal stones.

Is lower pole caliceal anatomy predictive of extracorporeal shock wave lithotripsy success for primary lower pole kidney stones?

PURPOSE: The management of lower pole kidney stones is controversial. We examined whether lower pole caliceal anatomy could predict the success of extracorporeal shock wave lithotripsy of primary lower pole kidney stones 20 mm. or less. MATERIALS AND METHODS: From December 1997 to June 2001, 246 adults with a single, 20 mm. or less radiopaque lower pole renal stone were treated with the Doli 50 lithotriptor (Dornier Medical Systems, Marietta, Georgia) while under general anesthesia. Of the 246 patients 190 (77%) had excretory urography available for review. Lower pole infundibular length and

The impact of pelvicaliceal anatomical variation between the stone-bearing and normal contralateral kidney on stone formation in adult patients with lower caliceal stones.

OBJECTIVE: We aimed to investigate the effect of pelvicaliceal anatomical differences on the etiology of lower caliceal stones. MATERIALS AND METHODS: Records of adult patients between January 1996 and December 2005 with solitary lower caliceal stone were reviewed. After exclusion of patients with hydronephrosis, major renal anatomic anomalies, non-calcium stones, history of recurrent stone disease and previous renal surgery, 78 patients were enrolled into the study. A proposed new classification for the renal collecting system of cattle.

OBJECTIVE: To evaluate the intrarenal anatomy of kidneys obtained from cattle and to propose a new classification for the renal collecting system of cattle. SAMPLE POPULATION: 37 kidneys from 20 adult male mixed-breed cattle. PROCEDURES: Intrarenal anatomy was evaluated by the use of 3-D endocasts made of the kidneys. The number of renal lobes and minor renal calyces in each kidney and each renal region (cranial pole, caudal pole, and hilus) was quantified. RESULTS: The renal pelvis was evident in all casts and was classified into 2 types (nondilated [28/37 {75.7%}] or dilated [9/37 {24.3%}
