Laryngeal electromyography in the diagnosis of laryngeal nerve injuries.

A practical technique of electromyographic (EMG) sampling of the cricothyroid and thyroarytenoid (vocalis) muscles is described. Quantitative motor unit action potential (MUAP) analysis on a group of seven healthy subjects revealed a mean amplitude of 426 microV for the vocalis and 500 microV for the cricothyroid muscle. Mean MUAP duration for the vocalis muscle was 3.5 msec and 4.4 msec for the cricothyroid. Normal activation patterns are reviewed for each muscle. The results of EMG studies in 18 consecutive patients referred to the EMG laboratory with chronic hoarseness was reviewed. There w

Recurrent laryngeal nerve injuries after esophagectomy.

Unilateral vocal-cord paralysis after esophagectomy involving a cervical anastomosis is uncommon in experienced hands but can cause marked morbidity because of pulmonary complications. It is important for the surgeon to understand the anatomy of the recurrent laryngeal nerve to avoid this complication. The diagnosis is usually easily made at the bedside and can be confirmed by visualization of the glottis. Early treatment may include supportive care with a speech pathologist or injection of the vocal-cord with temporary material to improve glottic closure. Definitive treatment can best be perf

Injection injuries of the recurrent laryngeal nerve.

We describe a new etiology for vocal cord paralysis. Two patients, chronic drug abusers, had in time lost access to the usual peripheral veins, and proceeded then to have the drugs injected repeatedly into the jugular veins in the neck. Both presented with persistent hoarseness and were found to have a unilateral vocal cord paralysis. Neck exploration was undertaken to evaluate this pathology, to rule out other obscure disease, and to decompress the recurrent laryngeal nerve within the carotid sheaths. Severe fibrosis was encountered in the neck of one patient and mild fibrosis in the other. A

Differences in laryngeal neurotrophic factor gene expression after recurrent laryngeal nerve and vagus nerve injuries.

OBJECTIVES: Recurrent laryngeal nerve (RLN) and vagus nerve (VN) injuries characteristically are followed by differing degrees of spontaneous reinnervation, yet laryngeal muscle neurotrophic factor (NF) expression profiles after RLN and VN injuries have not been well elucidated. This study's objective was to determine the relative changes in gene expression of 5 well-characterized NFs from laryngeal muscle after RLN or VN injuries in a time-dependent fashion, and demonstrate how these changes correspond with electromyography-assessed innervation status. METHODS: Thirty-six male rats underwent

Recurrent laryngeal nerve injury with the laryngeal mask.

We report a case of recurrent laryngeal nerve injury after anesthesia with the LMA and review the published cases of major pharyngolaryngeal morbidity. Prevention of this and other pharyngolaryngeal injuries probably depends on the skilled application of the correct insertion technique and the use of low intracuff pressures and volumes.

[Prophylaxis of laryngeal nerve injuries in thyroid surgery].

The problem of injury of recurrent laryngeal nerve (RLN) is topical despite large usage and detailed development of surgical treatment of thyroid gland (TG). The results of 3603 operations on TG were analyzed. The 1st group consisted of 560 patients operated in 1981-1988 by O.V. Nikolaev's method without dissection of RLN, the 2nd group--3043 patients operated in 1989-1998 with intraoperative imaging of RLN on the one or two sides. In the 1st group the injury of RLN was revealed in 39 (7.0%) patients, in 4 of them--bilateral. In 2nd group it was revealed in 10 (0.3%) patients. Present data lea

Severity of Recurrent Laryngeal Nerve Injuries in Thyroid Surgery.

BACKGROUND: Few studies in the literature have reported recovery data for different types of recurrent laryngeal nerve injuries (RLNIs). This study is the first attempt to classify RLNIs and rank them by severity. METHODS: This prospective clinical study analyzed 281 RLNIs in which a true loss of signal was identified by intraoperative neuromonitoring (IONM), and vocal cord palsy (VCP) was confirmed by a postoperative laryngoscope. For each injury type, the prevalence of VCP, the time of VCP recovery, and physical changes on nerves were analyzed. Additionally, different RLNI types were experi

Collateral reinnervation by the superior laryngeal nerve after recurrent laryngeal nerve injury.

This study investigates the role of the intact superior laryngeal nerve (SLN) in the reinnervation process of one of the laryngeal muscles, the posterior cricoarytenoid muscle (PCA), following recurrent laryngeal nerve (RLN) injury. Using a chronic RLN injury model in the adult rat, PCA reinnervation was assessed by retrograde double-tracing techniques in combination with electrophysiology and immunohistochemistry of muscle sections. The results demonstrate that the PCA receives dual innervation from both laryngeal nerves even in the uninjured system. Functionally significant collateral reinne

Nerve injury and the laryngeal mask airway.

The laryngeal mask airway is a widely used, non-invasive, general purpose airway. We report the case of a temporary vocal cord palsy following the use of such an airway. The development of inappropriately high cuff pressures secondary to nitrous oxide diffusion into the cuff is proposed as the most likely cause. Knowledge of the existence of nerve injuries complicating laryngeal mask use is particularly important when counselling certain patients. Mandatory intraoperative cuff pressure monitoring should lower the risk of subsequent voice problems.

[Intraoperative injuries of the superior laryngeal nerve in thyroid surgery].

The intraoperative injury of the external branch of the Superior Laryngeal Nerve (SLN) is a disregarded complication of thyroid surgery that becomes a serious one for particular professional groups. The Authors retrospectively evaluate 124 cases of total thyroidectomies performed by the same surgeon from 1999 to 2002. Clinically evaluated SLN lesions (hypomobility of the vocal cord) was registered in six patients (4.8%). Routine identification of SLN and meticulous dissection of the superior thyroid vessels must be the technical benchmark to avoid nerve injury, even if bulky disease, anatomic
