Rheumatic disease aspects of leg length inequality.

Inequality of leg lengths is associated with low back pain, greater trochanteric bursitis, and degenerative hip disease. The pelvic obliquity that results from a discrepancy of leg lengths may contribute to degenerative changes in the lumbar spine. The diagnosis of leg length inequality may be overlooked. If this occurs, extensive and costly investigations are then often ordered to evaluate these secondary manifestations of leg length inequality. This is unfortunate because a simple shoe lift is all that was needed to provide symptomatic relief for selected patients.

Leg length inequality. Implications for running injury prevention.

Leg length inequality is a relatively common musculoskeletal malalignment related to structural, postural and environmental factors. The inequality is a plausible aetiological factor in the development of a variety of overuse injuries because it alters the magnitude and distribution of mechanical stress within the body. Leg length inequality has been linked with lower extremity stress fractures, low back pain, hip pain and vertebral disk problems of runners. The appropriateness of using inshoe lifts to reduce or eliminate the inequality has not been rigorously evaluated. Further research is ne

[Braces for congenital leg length inequality in children].

Despite recent advances in techniques of limb equalisation, some congenital anomalies cannot be corrected and shortening at the end of growth is severe. These children then need to be braced permanently. The role of the surgeon is then limited to satisfactory adaptation of the prosthesis which must be light, cosmetic and functional. The authors described different types of bracing according to the type of anomaly, its level, its permanent state and the existence of unilateral or bilateral involvement. Indications are given for surgery to obtain satisfactory fitting of the patient to the brace.

Leg length inequality in humans: a new neurophysiological approach.

In conditions where there is moderate or severe pain, such as in low back pain, sciatica and osteoarthritis affecting the hip, the patient often reports the sensation that the two legs are of unequal length. This sensation cannot be explained as a result of fracture or other trauma that might cause a real change in length of the legs. There is confusion as to whether this 'leg length inequality' causes the pain or vice versa. It is argued here that the sensation occurs when there is pressure on the sensory nerves, especially the dorsal spinal roots. This causes pain but, more relevantly, cause

Phemister epiphysiodesis in leg length inequality treatment--evaluative outcome analysis.

BACKGROUND: The aim of this study was to evaluate the efficacy of Phemister epiphysiodesis for leg length inequality (LLI) treatment in comparison with epiphysiodesis techniques currently in use and to determine factors that may influence outcomes. MATERIALS AND METHODS: Nineteen girls and sixteen boys with a mean skeletal age of 12.2 years and a mean functional LLI of 3.8 cm at the time of surgery were enrolled. Data were retrieved from hospital records and bone ages were determined using standard atlases. Mean follow-up duration was 7 years. RESULTS: Good results (final LLI < 1.5 cm) were

Management of congenital leg length inequality: value of early axis correction.

The principles of treatment of congenital leg length inequality include three strategies: joint stabilization, axis correction, and limb length equalization. We believe that early axis correction is important for normal musculoskeletal development and that residual deformity may be self-perpetuating. Of 97 lower limbs assessed, 14 had PFFD alone, 6 fibula hemimelia alone, and 49 a combination of the two. Seventy-four limb segments were operated on, including 29 joint stabilizations, 65 axis corrections, and 62 lengthenings. Axis correction was the commonest procedure in the under-five age grou

Anatomical leg length inequality, scoliosis and lordotic curve in unselected clinic patients.

Anatomic leg length inequality (LLI) and various mechanical measures of the lumbar spine, particularly scoliosis, lordosis and Ferguson's angle, were analyzed retrospectively from the X-rays of 106 consecutive patients in a private chiropractic practice. Results showed that 40% of the subjects had LLI greater than 6 mm while 70% had LLI greater than 3 mm. These data compared closely with studies done by other investigators, indicating that our routine diagnostic X-ray procedure is sufficient for accurately determining LLI. Our most noteworthy finding was that patients with LLI greater than 6 m

Unilateral tibial hemimelia with leg length inequality and varus foot: external fixator treatment.

A 15-year-old girl with type II unilateral hemimelia presented with a 13.5-cm shortening of her right leg, absence of the distal half of the tibia, tibiofibular synostosis, and medial dislocation of a cavus and varus foot. She was treated by means of an external fixator. The shortening was significantly corrected, and realignment of the foot with the limb was achieved. An arthrodesis of the talus and lower end of the fibula was carried out operatively and stabilized with an external fixator. In the same surgical procedure, we performed an osteotomy of the tibiofibular synostosis, and progressi

Clinical symptoms and biomechanics of lumbar spine and hip joint in leg length inequality.

A simple and reliable low dose radiologic method developed by the author was used to measure leg length inequality of 798 patients with chronic and therapy resistant low-back and/or unilateral hip symptoms and 359 symptom free subjects. Statistically highly significant correlations of the symptoms and leg length inequality were observed. In the majority (79 and 89%), the chronic or recurrent sciatic pain and unilateral hip symptoms occurred on the side of the longer lower extremity. When correcting the leg length inequality simply with an adequate shoe lift, a permanent and mostly complete all

[Percutaneous epiphyseodesis. Correction of leg length inequalities and frontal plane deformities].

The percutaneous epiphysiodesis is recommended as a save and reliable technique. We have performed 21 distal femoral, 17 proximal tibial and 3 proximal fibular percutaneous epiphyseodeses on 25 patients. On 11 patients the epiphysiodesis was caused by an average leg length discrepancy of 2,7 cm (1-4 cm). 13 patients had angular deformity near the knee joint in frontal plane (6 valgus and 7 varus deformities). One patient suffered from a combined deformity with 3, 5 cm shortened leg and varus deviation of 10 degrees. The average deformity angle of the knee was 7,4 degrees (4 degrees -16 degrees

Pelvic reference selection in patients with unilateral Crowe type IV DDH for measuring leg length inequality.

We identified the presence of deformities in the affected pelvis of unilateral Crowe type IV DDH patients, and if present, whether the teardrop and ischial lines were parallel with the sacral base line. We also verified whether the sacral base line provided a better pelvic landmark than the teardrop line for determining leg length inequality (LLI). After leveling the pelvis by using a block to lift the short leg, standard anterior-posterior full-length radiography was performed on 10 patients and 10 healthy volunteers as controls. The ratio of pelvic heights on each side of the pelvis, the ang

Biomechanical compensation mechanisms during stair climbing - The effect of leg length inequalities.

BACKGROUND: Stair climbing is a complex and demanding daily activity with increased physical loads. Therefore, analyzing stair climbing abilities is a frequently used diagnostic tool. Leg length inequalities (LLIs) are a common condition in the population, with individual consequences like lower back pain, scoliosis, and osteoarthritis. Despite its high prevalence, the necessary treatment, for mild LLIs, is still controversial. Previously, the focus was to analyze the effects of LLIs during static standing and walking. To create a holistic view on the dynamic effects of LLIs, and since climbin

Ruptures of the Achilles tendon: relationship to inequality in length of legs and to patterns in the foot and ankle.

We studied 101 patients who were operated on at Oulu University Hospital for ruptured Achilles tendon from 1987 to 1992 (inclusive), and 87 healthy control patients from among Army conscripts. The mean inequality in length of legs (ILL) was 5 +/- 4 mm. Among the 48 patients with ILL > or = 5 mm, the side affected with ruptured tendon was longer in 48% of cases and shorter in 52%. An underpronating alignment of the ankle and foot (based on beta45 measurement of the angle < or = 4 degrees) was found in 21% of patients and 5% of controls (P < 0.001). A combination of high longitudinal arch and un

Effectiveness of gluteal control training in chronic low back pain patients with functional leg length inequality.

Chronic low back pain (LBP) is a common musculoskeletal disorder and is often accompanied by functional leg length inequality (FLLI). However, little was known about the effects of gluteal muscle control training in patients with LBP and FLLI. This study was designed to investigate the effects of gluteal control training in patients with LBP and FLLI. This is a double-blinded, randomized controlled study design. Forty-eight LBP patients with FLLI were randomized to the gluteal control training (GT) (47.58 ± 9.42 years) or the regular training (RT) (47.38 ± 11.31 years) group and received alloc

Leg-length inequality and low-back pain after Perthes' disease: a 28-47-year follow-up of 96 patients.

A follow-up of 96 patients with Perthes' disease was made 28-47 years after onset of the disease. We paid special attention to leg-length inequality and its consequences and low-back disability. Leg-length inequality was a common finding, but low-back pain was not a significant problem. Leg-length inequality and lumbar scoliosis correlated poorly with low-back disorders. Degeneration of the lumbar spine was the only factor that correlated well with low-back pain.
