Smooth muscle tumors are uncommon lesions of the vulva and represent a variety of histologic types. When encountered, surgical treatment is guided by the malignant potential of the tumors. This article presents the case of a 45-year-old woman who underwent conservative excision of a 10-cm vulvar lesion consistent with benign epithelioid leiomyoma. This unusual case provides an opportunity to review the clinical and pathologic features of this uncommon variant of leiomyoma and to describe the recently suggested pathologic criteria for determining the malignant potential of smooth muscle tumors

An ultrastructural and light-microscopic study of the second published case of epithelioid leiomyoma of the vulva is reported. The tumor occurred in the right labium minus of a 26-year-old nonpregnant female. Optic microscopy revealed a clear cell neoplasm with a marked relationship to arterial vessels from which it may originate. The ultrastructure was analogous to that of similar cases reported in the uterus and stomach.

Gastric epithelioid leiomyomatous (leiomyoblastomas) tumours primarily affect middle-aged men and usually present with upper gastro-intestinal bleeding, manifesting as haematemesia and/or melaena. This report documents a case of gastric epitheliod leiomyoma presenting with spontaneous haemoperitoneum. Its possible relationship with haemangiopericytoma and glomus tumour is discussed.

BACKGROUND: Smooth muscle tumors of the vulva are rare and sometimes create diagnostic difficulty because of unusual morphological features.
CASE: A 29-year-old African-American female presented with a vulval mass during her 5th week of pregnancy. The lesion was surgically removed under the clinical impression of a Bartholin cyst. RATIONALE: Smooth muscle tumors of the vulva are infrequent neoplasms with diverse histologic features and unclear biologic behavior. Herein, we report a very rare case of vulvar epithelioid leiomyoma and review of previous reported cases of these tumors. We present 4 cases of uterine mesenchymal tumors that were located mostly in the myometrium in middle-aged women. Grossly the tumors vaguely resembled conventional leiomyomas. All tumors were extremely hyalinized. The tumor cells were remarkable because of their large size and epithelioid shape.
