Study of multidrug therapy in paucibacillary leprosy.

A study was undertaken to evaluate the efficacy of multidrug therapy (MDT) in paucibacillary leprosy. Out of 155 fresh cases studied 48 had indeterminate, 38 tuberculoid and 69 had borderline tuberculoid leprosy. Out of 155 cases, 64 patients in the first group were treated with dapsone 100 mg daily for 12 months. In the second group, 91 patients were given MDT, consisting of rifampicin 600 mg once a month and dapsone 100 mg daily for consecutive 12 months. The cases receiving dapsone and the cases having MDT remained clinically active at the end of 6 months in 56.1% and 37.2% respectively; 13

Persistent reaction in paucibacillary leprosy: case reports.

Three patients of histopathologically confirmed borderline-tuberculoid leprosy showing no acid-fast bacilli and with lesions confined to the face, 2 on the cheek and 1 on the forehead, were given multidrug therapy as recommended by the WHO for paucibacillary cases. Within 3 months the lesions showed signs of upgrading (or reversal) reaction which was substantiated by histopathology. In 1 patient the facial nerve was affected leading to facial palsy. The lymphocyte transformation test did not show a significant rise. All 3 patients were given oral prednisolone for periods varying between 5 and

Solitary plaque of borderline lepromatous leprosy clinically masquerading as paucibacillary leprosy.

Borderline lepromatous (BL) leprosy typically manifests as numerous asymmetric ill-defined macules or infiltrated plaques. Localized cutaneous involvement in BL leprosy is infrequently reported. Type 2 reaction (T2R), an immune complex syndrome, occurs in patients with BL and lepromatous leprosy, as crops of tender evanescent papules or nodules with constitutional symptoms. T2R can also present with various atypical morphologies and rarely as type 1 reaction (T1R), thereby creating a diagnostic dilemma.

Erythematous macules as an atypical presentation of paucibacillary leprosy.

Leprosy, caused by Mycobacterium leprae, remains a significant health concern in certain endemic regions despite global elimination efforts. This case describes a 53-year-old Moroccan woman presenting with a 1-year history of asymptomatic erythematous macules associated with sensory deficits. Initial diagnoses included common dermatological conditions, leading to delays in definitive treatment. A detailed clinical examination revealed hallmark sensory loss in the lesions, with dermoscopy and histopathology supporting a diagnosis of paucibacillary leprosy. While bacilloscopic findings were nega

[Location of single skin lesions of paucibacillary leprosy].

In this paper the authors study the sites of single lesions in 317 paucibacillary patients registered at the outpatient units of the CMS Jorge Saldanha and the Curupaiti State Hospital in Rio de Janeiro, Brazil. The preferential sites of lesions in the population studied, their relation with age and sex and factors likely to influence their distribution are discussed. The findings are compared with other similar studies performed in Asia and Africa.

Diagnostic value of cardinal signs/symptoms in paucibacillary leprosy.

With the help of sensitivity and specificity criteria, an attempt is made to quantify the gain in certainty in diagnosis with the use of various cardinal signs/symptoms (S/s) of leprosy in order to study their predictive value in correct diagnosis of paucibacillary leprosy (PB) by the Paramedical Workers. The study was based on the findings in 326 new cases of paucibacillary leprosy detected by 10 paramedical workers during a recent field survey. Observations in the present study confirm the scientific basis of presently used combinations of cardinal S/s for correct diagnosis of leprosy especi

Histopathological activity in paucibacillary leprosy patients after ROM therapy.

Histopathological activity was assessed in the skin tissue of 13 skin-smear negative, borderline tuberculoid leprosy patients after administration of a single dose of ROM (rifampin 600 mg, ofloxacin 400 mg and minocycline 100 mg) therapy. Biopsies taken just before therapy showed Mycobacterium leprae to be present in eight cases. After 6 months, only three showed granulomatous lesions and others showed only resolving or inactive lesions. Acid-fast bacilli (AFB) persisted in the nerves of three cases. At the end of 12 months, granulomas persisted in 2 out of 13 (15%) patients. No bacilli, howev

Altered cytokine profiles in relapsed paucibacillary leprosy: a case report.

BACKGROUND: Individuals with relapses of leprosy should be monitored carefully, however, with respect to paucibacillary (PB) leprosy, it is sometimes difficult to make a definitive diagnosis of relapse, because the bacillary index is often negative. To evaluate the usefulness of cytokine profiling in a patient with relapsed PB leprosy who tested negative for anti-phenolic glycolipid-I antibodies, we analyzed the Mycobacterium leprae protein-induced cytokine expression in peripheral blood mononuclear cells of the patient. CASE PRESENTATION: An 89-year-old-male relapsed PB patient, first treate

Nerve conduction studies in paucibacillary and multibacillary leprosy: a comparative evaluation.

Involvement of peripheral nerves in patients with leprosy results in sensory, motor and autonomic dysfunctions along with deformities and disability. Pattern of nerve involvement is different for different forms of leprosy. In this study, we evaluated and compared the nerve conduction parameters of paucibacillary leprosy with that of multibacillary leprosy. In this study, 40 consecutive patients of leprosy (19 cases of paucibacillary and 21 cases of multibacillary leprosy) were included. Nerve conduction studies were performed according to the standard procedure described in the manual of the

Indeterminate leprosy: histopathologic and histochemical predictive parameters involved in its possible change to paucibacillary or multibacillary leprosy.

In an attempt to find clinical, bacteriological, histopathological, and immunohistochemical parameters to predict the progress of indeterminate leprosy patients to either paucibacillary (PB) or multibacillary (MB) leprosy, skin biopsies from 51 patients with indeterminate leprosy were retrieved from the files of the São Paulo Health Institute (Brazil). All of these patients had progressed to either PB or MB leprosy over a period of time which varied from 2 months to 24 years. Clinical records were examined, and new sections were cut from the paraffin blocks and stained by hematoxylin-eosin and

Single lesion paucibacillary leprosy: baseline profile of the Brazilian Multicenter Cohort Study.

In Brazil, there is little information about the clinical and epidemiological characteristics of paucibacillary, single skin lesion leprosy patients (SSL-PB). Only recently has the official notification system distinguished leprosy patients with a single lesion as a clinical entity, for whom the single-dose ROM (rifampin, ofloxacin and minocycline) regimen has been recommended. In this paper, we describe the baseline clinical features and the immunological background of a multicenter cohort of SSL-PB leprosy cases enrolled between December 1997-1998. Patients were recruited at health centers l

Distinct Roles of Th17 and Th1 Cells in Inflammatory Responses Associated with the Presentation of Paucibacillary Leprosy and Leprosy Reactions.

It is well established that helper T cell responses influence resistance or susceptibility to Mycobacterium leprae infection, but the role of more recently described helper T cell subsets in determining severity is less clear. To investigate the involvement of Th17 cells in the pathogenesis of leprosy, we determined the immune profile with variant presentations of leprosy. Firstly, IL-17A, IFN-γ and IL-10 were evaluated in conjunction with CD4+ T cell staining by confocal microscopy of lesion biopsies from tuberculoid (TT) and lepromatous leprosy (LL) patients. Secondly, inflammatory cytokines

Diagnostic evaluation of a novel recombinant multi-epitope protein for paucibacillary and multibacillary leprosy.

The diagnosis of leprosy remains challenging due to its clinical similarity with other infectious diseases and the variable sensitivity and specificity of current laboratory tests. Therefore, the identification and/or development of novel antigens is crucial for improving diagnostic accuracy. In this study, we designed a novel recombinant multi-polypeptide construct, termed M3, which was composed of specific B-cell epitopes from nine Mycobacterium leprae proteins, previously shown to be antigenic in leprosy. The recombinant M3 protein was expressed, purified, and evaluated as an antigen for th

Mycobacterium leprae is identified in the oral mucosa from paucibacillary and multibacillary leprosy patients.

The epidemiology of acute paediatric osteoarticular infections (OAI) has recently evolved, mainly due to the improvement of microbiological diagnosis. We conducted a prospective study to analyse the recent epidemiology and the clinical evolution of paediatric OAI in order to validate the adequacy of our probabilistic first-line antibiotic treatment (intraveinous cefamandole + gentamicin). All children suspected of community acquired OAI were included and followed-up for 3 years. The etiologic diagnosis was based on blood cultures, joint aspirations and bone punctures. All osteoarticular (OA) s

Improved detection of paucibacillary and multibacillary leprosy using a novel recombinant chimeric protein.

The diagnosis of leprosy remains challenging because available diagnostic tests show variable sensitivity and typically require blood sampling. In this study, we assessed the diagnostic performance of the chimeric M2 protein using ELISA assay based on both serum and urine samples from paired specimens of 315 participants. The cohort included paucibacillary (PB; n = 50) and multibacillary (MB; n = 60) leprosy patients, individuals diagnosed with leishmaniasis, Chagas disease, tuberculosis, or HIV infection, as well as household contacts and healthy controls. In serum-based ELISA, mean optical d
