BACKGROUND: Traditional letrozole or clomiphene treatment is started on cycle days 3 to 5, following a spontaneous menses or progestin-induced bleeding. If ovulation does not occur, the dose is increased following a progestin-induced bleeding. An alternative stair step protocol has been described, and if ovulation does not occur in this protocol, a higher dose is used without inducing withdrawal bleeding. This open-label prospective study evaluated the efficacy of letrozole (2.5 mg/day) combined with norethisterone acetate (2.5 mg/day) in the treatment of pain symptoms related to the presence of rectovaginal endometriosis. The treatment significantly and quickly decreased the intensity of symptoms, but pain recurred at 3-month follow-up; five women underwent surgery during the follow-up, and histologic examination of rectovaginal nodules revealed the presence of active endometriotic lesions.

INTRODUCTION: Infertility is defined by failure to achieve a pregnancy after 12 months or more of regular unprotected sexual intercourse. Letrozole is a type 2 aromatase inhibitor, which reduces availability of estrogen in postmenopausal women, thereby decreasing its ability to stimulate breast cancer cells. Phase III trials in both the advanced and early breast cancer setting have shown an improvement in disease-free survival compared with other compounds, including tamoxifen. Letrozole is well-tolerated, with the main adverse effects reported as hot flushes, arthritis, arthralgia and myalgia, and a trend towards increased risk of fracture.
