Progress in the treatment of articular fractures and ligament injuries.

The treatment of joint injuries has seen a marked development in the last years. The surgical trauma of articular reconstruction was reduced and our deeper understanding of the mechanism of connective tissue healing influenced the rehabilitation program after joint injuries significantly. Indirect soft-tissue sparing reduction techniques can be applied to most joint fractures and improved fixation techniques using intraoperative fluoroscopy or arthroscopy allow to avoid vast exposures of the joint. The process of ligament healing requires joint movement and a certain stress to the healing tiss

Articular and ligamentous contributions to the stability of the elbow joint.

This preliminary study of four elbow specimens investigates the relationship of articular geometry and ligamentous structures in providing stability to the elbow joint. A technique is presented that describes the constraining features of varus-valgus and distraction in extension and at 90 degree of elbow flexion. Valgus stability is equally divided among the medial collateral ligament, anterior capsule, and bony articulation in full extension; whereas, at 90 degrees of flexion the contribution of the anterior capsule is assumed by the medial collateral ligament which provides approximately 55%

Anatomical Description of the Spring Ligament Articular Facet.

BACKGROUND: The spring ligament fibrocartilaginous complex (SLFC), which is essential for stabilizing the medial longitudinal arch, features a little-explored fibrocartilaginous facet within its superomedial aspect, articulating with the talar head. This research aimed to provide a detailed anatomical description of this facet, designated as the spring ligament articular facet (SLAF). METHODS: Nine normally aligned cadaveric lower limbs were dissected, approaching the SLFC from a superior direction. Following talus disarticulation, high-resolution images of the ligament complex were captured

[Neurohistological studies in allogeneic cruciate ligament transplants as intra-articular ligament replacement].

A multiplicity of surgical operations have been developed in an attempt to achieve satisfactory function after ACL repair. None of these procedures have been able to duplicate the fiber organization, attachment site anatomy, vascularity, or function of the ACL. 18 foxhounds received a deep frozen bone-ACL-bone allograft and a ligament augmentation device (LAD). Neurohistological changes were evaluated 3, 6, and 12 months following implantation. Modified silver impregnation method and gold chloride technique were used to examine the presence of nerve endings and axons. Two morphological distinc

Ligament contribution to patterns of articular fractures of the distal radius.

PURPOSE: Intra-articular fractures of the distal radius are common injuries, but the correlation between ligament attachments and fracture location is poorly understood. The purpose of this study was to assess the location of intra-articular fractures compared to the known ligament attachments of the distal radius. METHODS: The authors retrospectively reviewed computed tomography scans of acute intra-articular distal radius fractures performed at 1 institution between 2001 and 2008. Of 145 scans, 45 were deemed unsuitable due to poor quality or presence of internal fixation in the distal radi

Multiaxial 3D MRI of the Ankle: Advanced High-Resolution Visualization of Ligaments, Tendons, and Articular Cartilage.

MRI is a valuable tool for diagnosing a broad spectrum of acute and chronic ankle disorders, including ligament tears, tendinopathy, and osteochondral lesions. Traditional two-dimensional (2D) MRI provides a high image signal and contrast of anatomic structures for accurately characterizing articular cartilage, bone marrow, synovium, ligaments, tendons, and nerves. However, 2D MRI limitations are thick slices and fixed slice orientations. In clinical practice, 2D MRI is limited to 2 to 3 mm slice thickness, which can cause blurred contours of oblique structures due to volume averaging effects

[Intra-articular fracture of the calcaneus associated with injury to lateral collateral ligaments of the ankle].

Calcaneal fractures are the most frequent of the tarsus (3.5% of all fractures). Young adults are mainly affected, being more frequent in men (5.9:1). On the other hand, ligament injuries associated with fracture of the calcaneus are very infrequent. We describe a case of a 39 year old patient who suffered ankle trauma. He presented instability in plantar flexion and inversion. A fracture of the calcaneus was diagnosed. During surgery, a complete lesion of the lateral ligament complex was found. The reduction and osteosynthesis associated with ligament reconstruction was performed. We consider

Developmental failure of the intra-articular ligaments in mice with absence of growth differentiation factor 5.

OBJECTIVE: To show the phenotypic characteristics of the knee joints in brachypodism mice (bp mice), which carry a functional null mutation of the growth differentiation factor 5 (GDF5) gene, we investigated the adult and embryonic bp mice. METHOD: Radiographic and macroscopic examinations of the knee joint of adult bp mice were performed. A histological examination of the knee joint of bp mice from E12.5 to E18.5 was also performed. RESULTS: Radiographic and macroscopic examinations of the adult bp mice showed anterior dislocation, hypoplastic condyles, and absence of the intra-articular li

Anterior cruciate ligament functional instability. A static intra-articular and dynamic extra-articular procedure.

The static and functional results in a series of 50 patients evaluated at least two years after surgery were gratifying. None had any episodes of functional instability. Forty-six of the 49 who participated in sports prior to operation returned to all desired sports, including six patients who returned to intercollegiate and national team sports. The overall subjective and objective results were 30, excellent; 17, good; one, fair, and two, failure. Of the two failures, one had excellent static stability but significant pain due to pre-existing chondromalacia patella, and the other had a postsu

The effect of exercise on the protein profile of rat knee joint intra- and extra-articular ligaments.

Injuries to the intra-articular anterior cruciate ligament (ACL) and the extra-articular medial collateral ligament (MCL) result in significant knee joint instability, pain, and immobility. Moderate endurance-type exercise can increase ligament strength but little is known on the effect of short-term regular bouts of high-intensity exercise on the extracellular matrix (ECM) structure of knee ligaments. Therefore, this study aimed to identify the effect of short-term regular bouts high exercise on the proteome of the rat ACL and MCL using mass spectrometry. Sprague-Dawley male rats (n = 6) were

Variations in internal structure, composition and protein distribution between intra- and extra-articular knee ligaments and tendons.

Tendons and ligaments play key roles in the musculoskeletal system in both man and animals. Both tissues can undergo traumatic injury, age-related degeneration and chronic disease, causing discomfort, pain and increased susceptibility to wider degenerative joint disease. To date, tendon and ligament ultrastructural biology is relatively under-studied in healthy, non-diseased tissues. This information is essential to understand the pathology of these tissues with regard to function-related injury and to assist with the future development of tissue-engineered tendon and ligament structures. A comparison of isokinetics and muscle strength ratios following intra-articular and extra-articular reconstructions of the anterior cruciate ligament.

A prospective, randomized clinical trial was conducted to compare an intra-articular with an extra-articular reconstruction of the anterior cruciate ligament. Isokinetic muscle testing was carried out 6 months to 2 years following operation using the Biodex isokinetic dynamometer. The extra-articular procedure resulted in a lower deficit for the peak quadriceps and hamstring torques and the range of motion.

Risk of intra-articular injection with longitudinal ultrasound-guided injection of collateral ligaments of the equine distal interphalangeal joint.

Desmopathy of the collateral ligaments of the distal interphalangeal joint is a common cause of equine foot lameness and carries a poor prognosis with conservative management. Intralesional injections may improve healing, although accuracy of radiographically guided injections is significantly less than when guided by MRI, which requires special needles. Longitudinal ultrasound-guided injection of the distal collateral ligament has not been evaluated objectively. In this prospective, anatomic study, seven equine cadaver limbs (14 collateral ligaments) were injected with methylene blue dye and

The Role of an Extra-Articular Tenodesis in Revision of Anterior Cruciate Ligament Reconstruction.

Patients who present for anterior cruciate ligament (ACL) revision with a high-grade pivot shift at the time of an index ACL revision procedure and subsequent reconstruction failure or a high-grade pivot shift at revision surgery, patients with generalized joint laxity, and those requiring softs tissue grafts should be considered candidates for lateral tenodesis to supplement intraarticular graft revision. Although there is no consensus regarding the optimal lateral tenodesis technique, due to the tibial positioning associated with tensioning and fixation of extra-articular procedures, a later

Loose intra-articular body following anterior cruciate ligament reconstruction.

We report a case of intra-articular fracture of a bioabsorbable fixation device from the femoral tunnel in an anterior cruciate ligament reconstruction using a bone-tendon-bone graft. Thirteen months after successful reconstruction surgery, the patient experienced episodes of locking and medial joint pain. There was no history of trauma and no symptoms of instability or swelling. On revision arthroscopy, a fractured tip of a bioabsorbable RIGIDfix cross pin (Mitek, Westwood, MA) was identified in the medial compartment of the knee. There was a broad area of chondral erosion affecting the media

[Intra- and extra-articular hamstring reconstruction of anterior cruciate ligament tears].

We examined a reconstruction technique for tears of the anterior cruciate ligament using the hamstring tendons. The tendons were harvested en bloc, leaving the tibial insertion intact. Three intra-articular strands (two semitendinous and one gracilis) and lateral tenodesis were used with continuity via the gracilis. The intra-articular procedure was performed arthroscopically with tunneling laterally to medially to achieve the best position. Ligamentoplasty was performed in 262 cases. Outcome was satisfactory. This method preserves the lateral iliotibial sheath and spares the extensors apparat

Anterior cruciate ligament reconstruction using one-third of the patellar ligament, augmented by extra-articular tendon transfers.

Eighty patients with persistent clinical and functional instability of the knee due to anterior cruciate ligament insufficiency underwent the operation reported, in which one-third of the patellar ligament substitutes for the cruciate ligament and extraarticular tendon transfers, medial and lateral, augment the substitution. The patients were followed for a minimum of two years, and fifty returned for personal evaluation. The average follow-up was thirty-three months, with a range of two to five years. There were thirty-five male and fifteen female patients. The average age was twenty-three ye

A fluid-structure interaction investigation of intra-articular pressure and ligament in wrist joint.

Understanding the stresses on the scapholunate interosseous ligament (SLIL) and its interaction with synovial fluid pressure could be vital to improve wrist treatment for various wrist conditions such as arthritis, sprains and tendonitis. This study investigated the interaction between the intra-articular pressure, specifically the synovial fluid pressure change and the SLIL stresses in a computational model during wrist radioulnar deviation (RUD). Magnetic resonance imaging (MRI) scans were used to acquire the anatomical model of the carpal bones and ligament, while the kinematics of scaphoid
