Lightning injuries.

Lightning causes more deaths than any other weather phenomenon. It is an electrical current that will choose the shortest paths between the contact points of the human body and may involve vital structures in its pathway. Almost every organ system can be injured by the electrical current of lightning. A broad spectrum of complications resulting from damage to the various organ systems has been reported. The sequelae peculiar to this specific type injury will dictate the choice of therapy.

Lightning and thermal injuries.

Electrical burns are classified as either high voltage (1000 volts and higher) or low voltage (<1000 volts). The typical injury with a high-voltage electrical contact is one where subcutaneous fat, muscles, and even bones are injured. Lower voltages may have lesser injuries. The electrical current has the potential to injure via three mechanisms: injury caused by current flow, an arc injury as the current passes from source to an object, and a flame injury caused by ignition of material in the local environment. Different tissues also have different resistance to the conduction of electricity.

Electrical and lightning injuries.

The aim of this study was to explore the mechanisms, complications, morbidity, and mortality associated with electrical injuries. Of 5053 acute burn admissions during a 5-year period, 202 patients (4%) had electrical burn injuries. Their mean age was 27.5 years (range, 3-71 years). Ninety-eight percent were male, and the extent of burn ranged from 1% to 70% TBSA (mean, 10.5 +/- 10.7% TBSA). High-voltage electricity caused 54% of the electrical injuries. Forty-two percent were caused by low-voltage currents and 4% by lightning. A total of 217 surgical procedures were performed on 202 patients F

Lightning injuries during snowy conditions.

Skiers and other snow sports enthusiasts can become lightning casualties. Two such accidents are reported, one being fatal. There are fewer warning signals of impending lightning strikes in winter-like conditions. However, outdoor activists should be aware of at least two suspicious clues: the appearance of convective clouds, and the presence of graupel (snow pellets) during precipitation.

Lightning injuries in sports: situations to avoid.

There is no absolute protection against lightning because of its random and capricious nature. However, the risk of being struck by lightning can be substantially reduced. There are general safety rules that apply to all athletic and recreational events. The athlete should have a proactive plan that can be instituted when storms approach. He/she should go to a safe shelter before the storm arrives and stay there until the danger is gone. Because the location, climate, terrain and playing site vary with different sporting activities, safety plans may have distinct elements for different recreat

Lightning injuries and acute renal failure: a review.

Lightning strikes cause multimodal injuries in victims, and although the number of deaths due to lightning is reportedly in the area of 1,000 deaths per year, they cause significant morbidity in many others. A major complication of a lightning strike is acute renal failure (ARF). The true incidence of ARF due to lightning injuries worldwide is difficult to ascertain because of significant underreporting, due in large measure to cultural and sociodemographic factors. Its incidence is reportedly rare in some literature and significant in others. However, lightning's potential to cause ARF due to

Lightning injury of the tympanic membrane.

Lightning injury to the ear is known, but specific reports are lacking. Four patients with tympanic membrane perforations who were managed surgically are reported. Their presentations, evaluations, intraoperative findings, and outcomes are discussed as they relate to the proposed pathogenic mechanisms. The authors' standard wide exposure tympanoplasty approach with two layer tympanic membrane repair is described. The added steps in performing this procedure may be necessary to ensure a good result in this unique group of patients.

Lightning injury of the tympanic membrane.

A number of cases of otologic injuries by lighting strikes have been described in the otolaryngological literature. The mechanism of these injuries remains uncertain. We report 3 cases of lightning injury that presented to us. Analysis of these cases suggests that the mechanism of injury is direct conduction of electricity from the scalp to the soft tissues of the external auditory canal to the tympanic membrane. The conduits of the electrical surge are the subcutaneous blood vessels, smaller vessels being damaged more than larger vessels. Since the tympanic membrane central vessels are smalle

The physical effects of lightning injury.

Several thousand lightning-related injuries occur each year in the United States resulting in nearly 600 deaths. Most incidents involve individual victims; group lightning strikes are rare. Ten soldiers were simultaneously injured in a group lightning strike while on training maneuvers at Fort Benning. Georgia. No deaths or loss of consciousness occurred, although two of the soldiers had amnesia for the event. All of the soldiers were hospitalized and observed for potential lightning-related complications. Ninety percent of the soldiers had first-degree skin burns, and all had focal muscular t

Myocardial injury due to lightning.

The report deals with a 27-year-old male who was standing in a tent and was injured by lightning as it struck a tree about 1.5 m away. He immediately lost consciousness and exhibited ventricular fibrillation when the emergency physician arrived. A clinical picture of hypoxaemic brain damage emerged after initially successful resuscitation. Brain death was diagnosed on the fifth day after injury. The discrete external findings (remaining arborescent skin marks) contrasted markedly with the severe thermal damage to the pectoral muscle and cardiac musculature found during the autopsy. The histolo

Modern concepts of treatment and prevention of lightning injuries.

Lightning is the second most common cause of weather-related death in the United States. Lightning is a natural atmospheric discharge that occurs between regions of net positive and net negative electric charges. There are several types of lightning, including streak lightning, sheet lightning, ribbon lightning, bead lightning, and ball lightning. Lightning causes injury through five basic mechanisms: direct strike, flash discharge (splash), contact, ground current (step voltage), and blunt trauma. While persons struck by lightning show evidence of multisystem derangement, the most dramatic ef

[Emergency treatment of injuries following lightning and electrical accidents].

The pattern of injuries following electrical accidents and the treatment by emergency teams differ depending on whether exposure was to a low-voltage or high-voltage power source or to a lightning bolt. Tissue damage results from the direct effect of current on cell membranes and from conversion of electrical energy into heat. Depending on the magnitude of electrical energy and the duration of exposure, cardiac dysrhythmia, damage to nerve tissue, extensive burns and shock may occur. Multi-system injury is frequently observed, either directly related to electrical shock or secondary to concurr

Lightning injuries of the posterior segment of the eye.

Lightning causes serious injuries and deaths worldwide every year. Ophthalmic injuries due to lightning are due to direct or indirect transmission of electric current, resistance-induced heat, and heat-induced shock wave. PubMed search of articles related to posterior segment injuries caused by lightning using keywords (lightning injury, ophthalmic manifestations, ocular injuries, and posterior segment) was conducted, and 19 case reports in 17 articles including 29 eyes with lightning injury to the posterior segment of the eye from 1984 to 2019 were reviewed. The majority of case reports (n =

Autonomic nervous system dysfunction in lightning and electrical injuries.

Autonomic nervous system (ANS) dysfunction is a serious complication of lightning and electrical trauma (L/ET). The ANS regulates the normal vegetative functioning of many organ systems. When ANS is compromised after lightning and electric trauma, patients are vulnerable to serious medical problems. Three conditions of ANS dysfunction of particular concern for L/ET patients are complex regional pain syndrome (CRPS), cardiovascular abnormalities, and keraunoparalysis (KP). The patient with CRPS presents with pain, hyperpathia, sweating, and edema hours to days after trauma. Neurorehabilitation

Lightning injury: report of two cases.

Two cases of lightning injury which occurred while mountain climbing and took different clinical courses are reported. One case with lightning marks on the abdomen was treated as a crush injury because of myoglobinuria and elevation of serum glutamic oxaloacetic transaminase, lactic dehydrogenase and creatine phosphokinase. The other case had lightning burns and complained of abdominal pain. He was treated with fluid transfusion resulting from superficial and deep dermal burns estimated at 55 per cent of the body surface and received a skin graft.
