Liver cirrhosis (LC) in habitual drinkers is divided into three categories: (1) alcoholic LC, (2) LC due to hepatitis C virus and alcohol, and (3) LC due to hepatitis C virus. In Japan, the frequency of LC related to hepatitis B virus in habitual drinkers is comparatively low. Although making a distinct differentiation is very difficult, it is possible to point out some characteristics which ars due to either alcohol or hepatitis C virus: (1) multiple spider angioma, acne rosacea, and palmar erythema are more frequently found in categories 1 and 2 than in 3, (2) levels of AST/ALT, gamma-GTP, T

The alcoholic liver cirrhosis usually causes overall immunological changes which might be attributed to either the hepatic disease itself, to ethanol action and/or to malnourishment of the patient. These immune abnormalities comprise both cellular and humoral immunity, consisting of increased immunoglobulin levels, depressed late-skin response to antigens, lowered proliferative response of lymphocytes to mitogens, lower plasma levels of complement proteins (C3 and C4) and by either lower (IL2 and gamma IF) or increased (IL1, TNF, IL6 and IL8) cytokine levels. Parallel to the systemic immune su

BACKGROUND: Alcohol and hepatotropic viruses cause the majority of liver cirrhosis cases in the Western World. Colchicine is an anti-inflammatory and anti-fibrotic medication. Several randomised clinical trials have addressed the question whether colchicine has any efficacy in patients with alcoholic or non-alcoholic fibrosis and cirrhosis.
OBJECTIVES: To assess the beneficial and harmful effects of colchicine in patients with alcoholic or non-alcoholic fibrosis or cirrhosis, excluding primary biliary cirrhosis.

Patients with liver disease require skilled, complex management of the inevitable complications associated with hepatic dysfunction. Portal hypertension, a major consequence of cirrhosis of the liver, causes ascites and esophageal variceal hemorrhages. Many clinical and surgical treatment options are available to manage these complications; however, they often produce only temporary symptom control and may beget further morbidity. This article focuses on the etiology, complications, and treatment of alcoholic cirrhosis. Perioperative nursing care of patients with alcoholic cirrhosis is based o

Approaches for the diagnosis of alcoholic cirrhosis of the liver at the dissecting table are given on the basis of the analysis of autopsy materials and liver biopsies. The etiological verification of cirrhosis is based on the peculiar morphological lesions of the liver and other organs. The liver is enlarged, its surface micronodal; histologically, an attack of acute alcoholic hepatitis is found not infrequently: necroses of hepatocytes, predominantly neutrophilic infiltration of the stroma and necrotic zones, alcoholic hyalin. Due to autolysis, alcoholic hyalin in the autopsy material change

The earliest and most reproduceable lesion associated with chronic alcohol abuse is fatty liver. In some alcoholics this may be superseded by alcoholic hepatitis, which may represent the link between the early lesion and cirrhosis. Alcoholic cirrhosis usually begins as a regular, monolobular variety, but is eventually transformed into an irregular, multilobular type. All stages of alcoholic liver injury have now been produced in the baboon, despite high protein and vitamin supplemented diets. Alcohol may therefore now be regarded as a direct hepatotoxin. Epidemiological studies have indicated

Hypergammaglobulinaemic purpura (HP) is commonly found in Sjögren's syndrome. We report a rare association of HP arising in a patient with alcoholic liver cirrhosis. A 65-year-old man presented with palpable purpura on the legs. Histological examination of a biopsy taken from the purpura found leucocytoclastic vasculitis in the superficial and mid-dermis. The activity of the vasculitic skin lesions correlated with liver dysfunction. Increased IgA and IgG levels, and hypocomplementaemia, may account for the pathogenesis of the hypergammaglobulinaemic immune complex-mediated vasculitis in this c

The intercellular adhesion molecule 1 (ICAM-1) level in the serum of patients with alcoholic liver cirrhosis was measured using a sandwich enzyme-linked immunosorbent assay. Circulating ICAM-1 was significantly elevated in the sera of all 19 patients as compared with normal healthy subjects. There was no correlation with the Child grade of the disease, but the highest levels of serum ICAM-1 (2,416 +/- 560 ng/ml) were measured in those patients who suffered from superimposed acute alcoholic hepatitis. No elevated serum ICAM-1 level was found in the ascitic fluid of cirrhotic patients. The eleva

Patients diagnosed with acute alcoholic hepatitis (AAH) are routinely managed medically and not considered suitable for orthotopic liver transplantation (OLT). The eligibility for OLT in these patients has been questioned due to the social stigma associated with alcohol abuse, based on the fact that AAH is "self-induced" with an unacceptably high recidivism rate. Many centers in Europe and the United States require abstinence periods between 6 and 12 months before OLT listing. AAH outcomes in the literature are poor, in particular due to patient noncompliance during the immediate 3 months prec

BACKGROUND: Alcoholic liver disease is a major cause of liver cirrhosis and the hepatorenal syndrome is a serious complication. Risk factors for hepatorenal syndrome (HRS) in alcoholic liver cirrhosis are not entirely explored.
AIM: To assess the risk factors for hepatorenal syndrome in alcoholic liver cirrhosis.
PATIENTS AND METHODS: Consecutive patients with alcoholic liver disease were followed for two months, development of renal failure, classified either as HRS or renal failure not fulfilling criteria of HRS, was the main outcome.
RESULTS: Of 171 patients, 14 (8.2%) developed HRS and

An elevated serum concentration of soluble intercellular adhesion molecule-1 (sICAM-1) was observed in patients with alcoholic liver cirrhosis in strong correlation with the current activity of the liver disease and the history of alcohol consumption. Circulating ICAM-1 levels were significantly elevated in the sera of all of 25 patients (1,400 +/- 850 ng/ml) compared with normal healthy subjects. The highest levels of sICAM-1 (2,650 +/- 560 ng/ml) were measured in those patients who suffered from superimposed acute alcoholic hepatitis (SAH). Regularly drinking cirrhotic patients without SAH a

Thirty patients with alcoholic cirrhosis of the liver were studied for in vivo and in vitro correlates of cellular immunity. Seventy-seven percent failed to be sensitized to dinitrochlorobenzene, indicating impairment of the in vivo cellular immune response. A significant decrease in the number of T-rosette-forming cells was observed in this group of patients (.01 smaller than P smaller than .025). This finding suggests that the active T-rosette test is a valuable tool in detecting partial alterations in cell-mediated immunity in alcoholic cirrhosis of the liver. Our results also suggest that

BACKGROUND: Alcohol-associated liver disease is the leading cause of liver transplantation in the western world. For these patients we calculated life expectancies both at time of transplant and several years later, stratified by key risk factors, and determined if survival has improved in recent years.
METHODS: Data on 14 962 patients with alcohol-associated liver disease who underwent liver transplantation in the MELD era (2002-2018) from the United States Organ Procurement and Transplantation Network database were analyzed using the Cox proportional hazards regression model and life table

Iliopsoas muscle hematoma in a patient with alcoholic liver cirrhosis is rarely seen, however it has a high mortality. Thus we should cautiously make a diagnosis and treatment. This is the case of a 60-year-old male. He had a 15-year history of alcoholic liver disease and emphysema. He presented with low back pain after a fall that had happened 2 months before. Due to persistent back pain, he went to see a local physician who, after detailed examination, suspected rupture of bilateral common iliac artery aneurysms and transferred the patient to our hospital. The same presumptive diagnosis was

The authors examined in detail the clinical and laboratory data and pathologic findings for 12 patients with alcoholic cardiomyopathy who were autopsied in the preceding ten years to determine the types of liver disease prevalent in this population. Neither alcoholic hepatitis nor cirrhosis was present in any patient, and most of the hepatic changes could be related to the effects of acute and chronic congestive heart failure. The major hepatic lesions included centrilobular congestion and/or ischemic necrosis, cardiac sclerosis (fibrosis about central veins and in perisinusoidal spaces), mild
