Neoplasms of the liver.

Primary neoplasms of the liver are composed of cells that resemble the normal constituent cells of the liver. Hepatocellular carcinoma, in which the tumor cells resemble hepatocytes, is the most frequent primary liver tumor, and is highly associated with chronic viral hepatitis and cirrhosis of any cause. Benign tumors, such as hepatocellular adenoma in a noncirrhotic liver or a large, dysplastic nodule in a cirrhotic liver, must be distinguished from well-differentiated hepatocellular carcinoma. Cholangiocarcinoma, a primary adenocarcinoma that arises from a bile duct, is second in frequency.

[Liver neoplasms in children].

Presentation is made of 27 cases of liver neoplasms in infancy and childhood that were admitted to the Pediatric Hospital of the National Medical Center of the IMSS, through a period of 15 years. Sixteen cases were diagnosed as hepatoblastoma, 2 as hepatocellular carcinoma, 4 as mesenchymal hamartoma, 2 as hemangioendothelioma of infantile type, 2 were embryonal rhabdomyosarcoma of the intrahepatic bile-ducts and one case of multiple bile-ducts hamartoma. The hepatoblastoma and the mesenchymal hamartoma are neoplasms that appear more frequently before the age of 2 years; the hepatocellular car

[Cryosurgery of liver neoplasms].

Cryosurgery is the in situ destruction of tissue using subzero temperatures. Its use for the treatment of some unresectable liver tumors has been clearly established as a therapeutic option. Experimental studies have demonstrated the feasibility of freezing of large liver volumes without any major metabolic and hemorrhagic complications. Modern cryosurgery has received substantial impetus from the development of automated cryosurgical apparatuses using liquid nitrogen. Intraoperative ultrasound has enhanced the process by enabling visualization of tissue freezing and ensuring precise and optim

Liver Transplantation for Liver and Perihilar Neoplasms.

Transplantation offers curative potential for patients with select primary hepatobiliary malignancies when disease biology or hepatic reserve limit surgical resection. Contemporary practice for transplant for hepatocellular carcinoma and cholangiocarcinoma emphasizes selection through radiographic criteria, biomarker thresholds, and downstaging and neoadjuvant protocols. In carefully selected patients, transplantation is associated with increased overall survival and reduced recurrence compared to resection. Evolving allocation policies, living donor expansion, and integration of systemic ther

Mesenchymal Neoplasms of the Liver.

Mesenchymal neoplasms of the liver can be diagnostically challenging, particularly on core needle biopsies. Here, I discuss recent updates in neoplasms that are specific to the liver (mesenchymal hamartoma, undifferentiated embryonal sarcoma, calcifying nested stromal-epithelial tumor), vascular tumors of the liver (anastomosing hemangioma, hepatic small vessel neoplasm, epithelioid hemangioendothelioma, angiosarcoma), and other tumor types that can occur primarily in the liver (PEComa/angiomyolipoma, inflammatory pseudotumor-like follicular dendritic cell sarcoma, EBV-associated smooth muscle

Surgical Considerations for Primary Liver Neoplasms.

Primary liver neoplasms, primarily hepatocellular carcinoma and intrahepatic cholangiocarcinoma, remain a major global health burden with poor survival and limited systemic therapy options. Determining resectability requires comprehensive preoperative assessment, including high-quality imaging, liver function evaluation, and future liver remnant volume calculations. Surgical resection with negative margins offers the best chance for long-term survival, and minimally invasive approaches are increasingly used in selected patients. For those with advanced but potentially resectable disease, strat

Pathology of Liver and Biliary Neoplasms.

This review begins with a brief overview of liver anatomy and histology, followed by a summary of the classification of primary hepatic tumors. It then focuses on hepatocellular carcinoma and cholangiocarcinoma, the 2 most common primary hepatic malignancies. In this section, emphasis is placed on key molecular features and histologic variants with important clinical implications. The review next addresses diagnostic challenges from the pathologist's perspective, providing practical insights into how these scenarios can be approached. Finally, the focus shifts to therapy-related changes in liv

Liver neoplasms and the oral contraceptives.

For sixteen years (1954 to 1970) oral steroids had been used for contraception by millions of women. We were unable in this time span to find any cases of liver neoplasms in women using oral contraceptives. However, since 1970, there have been 138 cases reported in the English literature. We will add nine cases to this number, including an unusual case report. Evidence for the oral contraceptive as an etiologic agent in causing liver neoplasms is circumstantial but but nevertheless substantial. The literature is reviewed, and our case is presented. At present, the etiology of liver neoplasms i

Biliary papillary neoplasm of the liver.

Biliary papillary neoplasia of the liver characterized by intraductal papillary growth of neoplastic biliary epithelia with a fine fibrovascular stalk has been sporadically reported, and includes intraductal growing cholangiocarcinoma and biliary papillomatosis. In addition, biliary papillary dysplasia and in situ and microinvasive carcinoma with papillary configuration reported in hepatolithiasis and in other chronic biliary diseases, could be included in this category. Usually, they arise in the intrahepatic large bile ducts, and the neoplastic and non-neoplastic parts of the intrahepatic bi

[Mucinous cystic neoplasm of the liver].

Mucinous cystic neoplasms are rare benign tumours of the liver, accounting for less than 5% of all liver cysts. The exact etiology of mucinous cystic neoplasms is unclear, but female dominance and estrogen receptor positivity of ovarian type stromal cells suggest a role of hormonal factors in their development. The diagnosis is challenging as there are no specific laboratory or imaging characteristics, and the final diagnosis is often based on histological examination. A 27-year-old female patient was admitted to hospital with jaundice and abdominal pain. Laboratory results showed elevated lev

Pediatric liver neoplasms: a radiologic-pathologic correlation.

Only 1-2 % of all pediatric tumors occur in the liver. Two thirds of these tumors are malignant and almost all of the tumors cause clinical symptoms due to their mass effects. Besides the poor prognosis in most of the malignant tumors, for further treatment the origin and nature of the neoplasm has to be known. Due to the mostly unimpeded growth into the peritoneal cavity, the origin of the tumors is primarily often unclear and can non-invasively only be determined by advanced imaging techniques. The display of the macro- and microhistological key features of primary pediatric liver neoplasms,

Radiological Assessment of Liver and Biliary Neoplasms.

Primary liver cancer, encompassing hepatocellular carcinoma and cholangiocarcinoma, represents a growing global health burden with significant morbidity and mortality. Imaging is central to the clinical management of these malignancies, informing surveillance, diagnosis, staging, and treatment response assessment. A comprehensive overview of current imaging modalities-including ultrasound, computed tomography, MRI, and PET/CT-and their respective roles across the spectrum of hepatic and biliary neoplasms is presented. Established diagnostic frameworks such as LI-RADS and staging systems includ

Palliative Therapy for Liver and Biliary Neoplasms.

Liver and biliary tumors often impose a high symptom burden from pain, obstructive jaundice, pruritus, hepatic encephalopathy, bleeding, and ascites. Pain management requires a multimodal approach including procedural and pharmacologic interventions. Jaundice and pruritus are managed through biliary drainage and medications. Encephalopathy is addressed by avoiding triggers, reducing ammonia via lactulose or interventional shunt modification. Endoscopic and embolization procedures are the mainstay to palliate bleeding. Ascites requires distinguishing cirrhosis-related from malignant causes to d

Other Primary Epithelial Neoplasms of the Liver.

Primary liver carcinoma (PLC) is the sixth most common malignancy worldwide and the third leading cause of cancer-related mortalities. Hepatocellular carcinoma (HCC) is the most prevalent form of PLC, followed by intrahepatic cholangiocarcinoma (iCCA). In addition, there is a group of rarer PLCs that do not fit neatly into the HCC or iCCA categories. This review explores this heterogeneous group, including combined hepatocellular-cholangiocarcinoma (cHCC-CCA), intermediate cell carcinoma (ICC), mixed hepatocellular-neuroendocrine carcinoma, and undifferentiated primary liver carcinoma. cHCC-CC

Liver mesenchymal neoplasms: something old, something new.

Histological examination of liver biopsies and resection specimens remains the gold standard to establish a definitive diagnosis of liver lesions. While hepatocellular carcinoma remains the most commonly encountered liver lesion on mass-directed biopsies, surgical pathologists must be aware of other entities that may pose diagnostic challenges, as an accurate diagnosis is key for patient management. Mesenchymal tumours of the liver are relatively uncommon, therefore many pathologists are unfamiliar with these tumours. While the clinical presentation and radiological features of these lesions o

Liver transplantation in malignant primary hepatic neoplasms.

Between April 1986 and August 1990, 151 liver transplantations were performed at our institution, 16 (11%) of them in 14 patients with primary hepatic tumors. There were 12 hepatocellular carcinomas, 1 angiosarcoma, and 1 Klatskin tumor. None of the tumors was resectable, and there was no preoperative evidence of extrahepatic tumoral extension. Exploratory laparotomy was performed prior to transplantation in three patients and selective embolization of the tumor in six patients. There was no difference in the intraoperative requirements for blood or plasma in the patients with hepatic tumors w
