Loteprednol Etabonate for the Treatment of Dry Eye Disease.

Dry eye disease (DED) is a common ocular condition that can impair vision and may adversely impact quality of life. Due to the inflammatory nature of this disorder, topical corticosteroids are an effective treatment option, particularly for moderate-to-severe DED when first-line treatments, such as ocular lubricants, are insufficient. Loteprednol etabonate (LE) is a retrometabolically designed corticosteroid with a low propensity to cause corticosteroid-related adverse effects, such as elevated intraocular pressure (IOP). This review was conducted to provide an assessment of published studies

Loteprednol etabonate: a review of ophthalmic clinical studies.

Loteprednol etabonate (LE) is a corticosteroid designed using the "soft drug" concept of Bodor. LE has been extensively evaluated as a treatment for ophthalmic inflammatory conditions. LE is administered as a sterile eye drop suspension and is commercially available as either a 0.5% or a 0.2% suspension. Lotemax (0.5% LE) has been demonstrated as effective in reducing the signs and symptoms of giant papillary conjunctivitis (GPC), acute anterior uveitis and inflammation following cataract extraction with intraocular lens (IOL) implantation. It is also effective for the prophylaxis of seasonal

Loteprednol etabonate nanostructured lipid carriers for enhanced ocular bioavailability.

Loteprednol etabonate (LTP), a corticosteroid approved for management of post-operative ocular inflammation and dry eye symptoms, suffers from limited ocular bioavailability and high dosing frequency in its current marketed form, Lotemax® suspension. The current study reports the development and optimization of a novel nanostructured lipid carrier (NLC) system for LTP, designed to overcome these limitations through enhanced corneal penetration, sustained release, and improved therapeutic efficacy. Excipient screening assessment identified Gelucire 44/14, Miglyol 829, and Tween 80 as optimal so

Advances in corticosteroid therapy for ocular inflammation: loteprednol etabonate.

Topical corticosteroids are effective in reducing anterior segment inflammation but are associated with adverse drug reactions (ADRs) including elevation of intraocular pressure (IOP) and cataract formation. Retrometabolic drug design has advanced the development of new corticosteroids with improved therapeutic indices. Engineered from prednisolone, loteprednol etabonate (LE) has a 17α-chloromethyl ester, in lieu of a ketone group, and a 17β-etabonate group. LE is highly lipophilic and binds with high affinity to the glucocorticoid receptor; any unbound LE is metabolized to inactive metabolite

Loteprednol etabonate: clinical potential in the management of ocular inflammation.

Loteprednol etabonate is a corticosteroid which is the product of 'soft drug' design (synthesis of a compound that undergoes predictable metabolism to inactive metabolites after its therapeutic effects have been expressed at or near the site of application). It has been developed as a topical treatment for ocular inflammation. Loteprednol etabonate is designed to be rapidly converted to inactive and nontoxic metabolites, thus minimising systemic adverse effects. The concentration of the drug in plasma from 10 healthy volunteers who received ocular loteprednol etabonate at therapeutic or suprat

Review of Loteprednol Etabonate 0.5%/Tobramycin 0.3% in the Treatment of Blepharokeratoconjunctivitis.

Use of a combination corticosteroid and antibiotic in a single formulation is common in the treatment of ocular inflammatory conditions for which corticosteroid therapy is indicated and there exists a risk of superficial bacterial infection. Loteprednol etabonate (LE) is a corticosteroid engineered to maintain potent anti-inflammatory activity while minimizing the risk of undesirable class effects of corticosteroids, such as elevated intraocular pressure and cataract. Impact of the Topical Ophthalmic Corticosteroid Loteprednol Etabonate on Intraocular Pressure.

Corticosteroids are a mainstay therapeutic option for the treatment of ocular inflammation. However, safety remains a concern for clinicians, particularly with long-term use. Though highly effective at suppressing inflammatory and allergic responses, topical ophthalmic corticosteroids carry an inherent risk of side effects, including elevated intraocular pressure (IOP), a risk factor for the development of glaucoma. The corticosteroid loteprednol etabonate (LE) contains an ester rather than a ketone at the C-20 position, minimizing the potential for side effects, including IOP elevation. In ea

An update on the surgical management of pterygium and the role of loteprednol etabonate ointment.

Pterygium, a sun-related eye disease, presents as wing-shaped ocular surface lesions that extend from the bulbar conjunctiva onto the cornea, most commonly on the nasal side. Pterygia show characteristic histological features that suggest that inflammation plays a prominent role in their initial pathogenesis and recurrence. Appropriate surgery is the key to successful treatment of pterygia, but there is also a rationale for the use of anti-inflammatory agents to reduce the rate of recurrence following surgery. Multiple surgical techniques have been developed over the last two millennia, but th

Loteprednol etabonate: comparison with other steroids in two models of intraocular inflammation.

Loteprednol etabonate (LE) is a novel steroid with a low tendency to raise IOP. It is metabolized in the eye to an inactive metabolite. The current study was undertaken to assess the intra-ocular anti-inflammatory activity of LE in two models of experimental uveitis in rabbits. In the endotoxin induced rabbit model, LE was effective at reducing measures of inflammation, but less so that either fluorometholone (FML) or dexamethasone. In the Freunds adjuvant chronic uveitis model, FML was also very effective with LE and dexamethasone showing similar activity. The data demonstrate that LE is effe

A comparison of intraocular pressure elevating activity of loteprednol etabonate and dexamethasone in rabbits.

A "Soft" corticosteroid, loteprednol etabonate I, designed based on the "inactive metabolite" concept can be used as a safe ophthalmic anti-inflammatory drug. By design, the metabolism of I follows a predicted biotransformation pathway, thus unwanted systemic side effects are avoided. Local side effects are also reduced. Accordingly, in a cross-over study I did not elevate the intraocular pressure (IOP) in rabbits, as opposed to dexamethasone.

Ocular absorption and distribution of loteprednol etabonate, a soft steroid, in rabbit eyes.

Loteprednol etabonate (LE) is a "soft" steroid belonging to a unique class of glucocorticoids. LE possesses a metabolically labile 17 beta-chloromethyl ester function which was designed in order to be hydrolyzed to an inactive carboxylic acid moiety. The ocular absorption and metabolism of a 14C-labelled LE was evaluated in New-Zealand White rabbits after administration of a 0.5% suspension in both eyes. At various time points following ocular administration, the cornea, aqueous humor, and iris-ciliary body were collected. LE and the putative inactive metabolites, PJ-90 and PJ-91, were identif
