One of the theories as to the etiology and pathogenesis of lunatomalacia (Kienböck's disease) is based on the presentation of an osseous compartment syndrome caused by a venous block in the pedicle. This gave us cause to examine the lunate bone more closely and to investigate possible anatomical causes for the disruption. For this purpose, ten hands were removed from cadavers proximal to the radiocarpal articular space. Through an artificial intraosseous canal, which did not touch the vascular structures of the lunate bone, epoxy could be injected under controlled conditions. The venous draina

BACKGROUND: There still is no standard therapy that predictably results in healing of avascular necrosis of the lunate bone. Nevertheless, there exists a wide spectrum of operative treatment options for different stages.
OBJECTIVE: This article reviews the treatment options for necrosis of the lunate bone and proposes algorithms based on the age of the patient and condition of the lunate bone and the wrist.
METHODS: Surgical treatment options for necrosis of the lunate bone can be divided into relieving or revascularization procedures and salvage procedures.
RECOMMENDATIONS: For patients un

Intraosseous lunate bone ganglia (ILBG) are known to be a cause of chronic wrist pain and disability. Standard treatment consists of curettage and autologous bone grafting. Open procedures have shown good results with few recurrences, but with frequent stiffness or persistent pain. Arthroscopic techniques are more recent and seem very reliable. Several arthroscopic techniques have been reported for ILBG approach and treatment. The present study describes an approach that preserves all the lunate cartilage of both radiocarpal and midcarpal surfaces. The surgical technique allows easy and direct

Aneurysmal bone cysts are rare lesions which even more rarely involve the bones of the hand. We report a case of a cyst involving the lunate which was initially mistaken for interosseous ganglion. Due to delay in diagnosis, marked destruction of the lunate occurred, necessitating excision and scapho-capitate arthrodesis. Although rare, aneurysmal bone cysts should be included in the differential diagnosis and ruled out prior to embarking on a course of treatment.

In this study, a novel rapid prototyping technology was used to fabricate scaphoid and lunate bone prostheses, two carpal bones that are prone to avascular necrosis. Carpal prostheses were fabricated with an Envisiontec Perfactory SXGA stereolithography system using Envisiontec eShell 200 photocurable polymer. Fabrication was guided using 3-D models, which were generated using Mimics software (Materialise NV, Leuven, Belgium) from patient computer tomography data. The prostheses were fabricated in a layer-by-layer manner; approximately 50-microm thick layers were observed in the prostheses. Ha

Respiratory pathogens such as Chlamydia pneumoniae may activate osteoclast cells, thereby inducing bone resorption and joint inflammation. Herein is a case report of a young man with misdiagnosed persistent wrist pain without any major trauma. Investigation of the patient's medical history confirmed a recent systemic C. pneumoniae infection. Preoperative X-ray and magnetic resonance imaging (MRI) showed a substantial decrease in cancellous lunate bone compactness. A stepwise approach was undertaken considering the rapid onset of bone devascularisation and the clinical presentation. Wrist arthr

Kienböck's disease, also known as avascular necrosis of the lunate bone describes the slowly progressive osteonecrosis of the lunate bone with secondary development of osteoarthritis and carpal collapse. In order to emphasize the atraumatic origin of the disease, this is normally referred to as aseptic, idiopathic or avascular lunate necrosis. There are thought to be a number of factors predisposing to Kienböck's disease, such as ulnar negative variance, the shape of the lunate itself and various types of vascular anatomy. The ongoing development of radiographic techniques in recent years has

Aneurysmal bone cysts rarely affect the carpus. We present a case of aneurysmal bone cyst affecting the lunate. Curettage and bone grafting of the lesion was successful, with no recurrence after 2 years of follow-up.

This work was performed on 50 wrist dissections. The scaphoid is situated in the territory of the radial artery. Two extracapsular vascular pedicles are constant: --the lateral pedicle arises in the anatomical snuff-box, --the anterior pedicle arises in the "wrist pulse" area and passes behind the flexor carpi radialis. It lies on the radio-carpal ligaments. THe intra-capsular pedicles approach the scaphoid from its proximal pole. There are anastomoses and they lie on the deep dorsal and palmar ligaments. The multiple anastomotic branches may form an inconstant pedicle. The lunate is supplied

Intraosseous ganglia is one of the most frequent lytic defect at the wrist. Its location in the lunate may be discovered by chance on an X-ray performed for another reason, or because of wrist pain and very rarely for a lunate fracture. A.P., lateral and oblique X-rays are mandatory. Bone scan, CT scan and MRI may be of help. Differential diagnosis may exist with an ulnar abutment syndrome with a lunate defect and with all the lytic bone tumours, a systemic disease or multiple defects as in overuse syndromes. In some cases, there is a condensation around the defect and a Kienböck's disease may

A patient sustained multiple injuries during a road traffic accident, including closed palmar luxation of the lunate together with dislocation of the triquetrum and hamulus of the hamate bone. Initially the injury was treated closed, but redislocation necessitated closed reduction and stabilisation with K-wires. Three years later movement was painless and slightly restricted despite persisting palmar dislocation of the lunate and triquetral bones as well as ulnar translocation.

We report a case of a symptomatic unicameral (simple) bone cyst of the lunate in a 42-year- old woman. The lesion was treated with curettage and cancellous autogenous iliac bone grafting. At five years of follow-up the wrist was pain free, there were no limitations of motion, and the radiographs showed complete obliteration of the cavity. To the best of our knowledge, no other unicameral bone cyst of the lunate has been reported in an adult. Cysts with significant cavities at the carpal bones in an adult should be approached cautiously, as they may require early curettage and bone grafting for

This report describes three cases of osteonecrosis of the lunate bone of the wrist in patients with systemic sclerosis presenting with wrist pain. All three patients had limited skin scleroderma but severe Raynaud's phenomenon. Two patients never received corticosteroids and one patient received only low doses for a brief period. None of the patients had other definable risk factors for osteonecrosis. Two patients underwent vascular bone grafting with improvement in symptoms. Osteonecrosis may represent an under-recognized cause of wrist pain in scleroderma patients.

Since April 1986, implantation of a vascular bundle containing the ramus carpeus dorsalis of the interosseal anterior artery and vein into the lunate has been undertaken for aseptic necrosis of the lunate bone. A series of 11 cases were performed in East-South Hospital with satisfactory results. The average follow-up period was 36 months. A series of follow-up roentgenograms reveal that necrosis of the lunate bone is being continuously repaired. Fourteen months postoperative, the following results are obtained: (1) the sclerosis of the lunate bone has completely disappeared on the roentgenogra

Injuries to the carpals and their ligaments are difficult to diagnose unless one utilizes special X-ray techniques based upon a careful history of the injury and examination for tender points and painful joints. Use of only standard X-rays and cursory examinations caused many injuries to be missed in earlier years. Many fractures resolve with simple splinting or casting, but many do not, and late sequelae are now being diagnosed more commonly than in the past. Localization of pathologic conditions by diagnostic injections of local anesthesia or arthrography (or both) is very helpful. Salvage m

Inspite of the excellent subjective judgement and functional results of Graner's operation in case of Kienbock's disease stage III, this technique is rarely used nowadays because of its high rate (20 to 30%) of disturbed fracture healing due to impaired blood supply. To avoid the risk of complete devascularization in the capitate region, we are using the callotaxis lengthening technique of Ilizaron, carrying out a segmental shifting. After percutaneous temporary SC- or STT-arthrodesis with the scaphoid in the horizontal or high position, the lunate is completely resected using a dorsal approac

Hydroxyapatite deposition disease (HADD) is a mostly uniarticular, self-limiting condition caused by deposition of hydroxyapatite (HA) crystals in tendons or in the peritendinous soft tissues. Commonly, the glenohumeral joint is affected. More rarely, the HA depot can be cause of a carpal tunnel syndrome due to an acute inflammatory reaction and space-occupying soft tissue oedema. We report a case of acute HA depot located at the volar site of the right wrist with affection of the deep flexor tendons and intraosseous migration into the lunate bone in a 50-year-old female. There are two main go
