New method to differentiate between lupus anticoagulants, progressive coagulation inhibitors and coagulation factor deficiencies in the mixing tests.

INTRODUCTION: Mixing tests in activated partial thromboplastin time (APTT) are used for the differentiation between lupus anticoagulants (LA), coagulation inhibitors, and factor deficient samples with APTT prolongation. However, the indexes for the differentiation have not been established. The present study aimed to develop new mixing test indexes for the differentiation. METHODS: Twenty-six LA-positive, 8 progressive coagulation factor VIII inhibitor, and 35 coagulation deficient samples were employed. APTT were measured for normal plasma, patient plasma, and mixing plasma prepared at a rat

[Lupus anticoagulant-hypoprothrombinemia syndrome with a false-positive test for coagulation factor inhibitors].

We report a case of a 1-year-old boy diagnosed with lupus anticoagulant-hypoprothrombinemia syndrome (LA-HPS), which is a rare disorder. His initial presentation of sinusitis was accompanied by hemorrhagic episodes including ecchymoses and epistaxis 6 months after antibiotic therapy. Laboratory results revealed prolonged prothrombin time (PT) and activated partial thromboplastin time (APTT) that did not correct with mixing studies. Factors II, VIII, IX, X, XI, and XII activities were 20%, 44%, 42.5%, 59%, 4%, and 10%, respectively. The Bethesda inhibitor assay showed inhibitors against multipl

[Acquired coagulation disorders].

Acquired coagulation inhibitors have become a popular area of research because they cause severe bleeding tendency in many patients. The use of acquired coagulation inhibitors requires rapid and precise diagnosis. Some acquired coagulation inhibitors show prolongation in the activated partial thromboplastin time (APTT) and/or prothrombin time (PT). To diagnose these disorders, mixing test is very useful. However, lupus anticoagulant related disorders, such as lupus anticoagulant hypoprothrombinemia syndrome (LAHPS), are difficult to diagnose because they are sometimes associated with a reducti

Lupus anticoagulant and protein S deficiency in otherwise healthy children with acute varicella infection.

UNLABELLED: Acquired protein S deficiency and lupus anticoagulant have been described in children with varicella who had purpura fulminans, disseminated intravascular coagulation or thrombosis. The aim of this study was to investigate the natural anticoagulants, hypercoagulability markers, other parameters of coagulation and fibrinolytic systems, and the presence of the lupus anticoagulant in otherwise healthy children with acute varicella infection. Blood samples were obtained from 17 children with varicella without thrombosis during acute varicella infection and 1 mo after onset. Coagulation

["Lupus anticoagulant" in immune hyperthyroidism].

A 56-year-old woman with autoimmune hyperthyroidism (Basedow) whose blood coagulation had at first been normal developed prolonged partial thromboplastin time (PTT) of 48 s and a fall in prothrombin time (Quick value) to 52%. At the same time, total activity of factor VIII was reduced to 18% and factor IX to 16%. These values not having changed after the addition of normal plasma, it is assumed that an acquired inhibitor of plasmatic coagulation was responsible. Such inhibitors were first described in lupus erythematodes and therefore called lupus anticoagulant, but later also demonstrated in

Unmasking the phospholipid-induced "hook effect" and its implications - increasing phospholipid can also lead to a longer clotting time in coagulation assays.

It is generally accepted that higher phospholipid concentrations generate shorter clotting times. Phospholipid-dependent lupus anticoagulants (LA) assays such as dilute Russell's viper venom time (dRVVT) and silica clotting time (SCT) have therefore been developed. However, cases have been observed where LA confirming tests (concentrated phospholipid) generates longer clotting times than screening tests (diluted phospholipid). This study investigates the underlying cause of this paradox and assess its implications. With different phospholipid concentrations, Russell's viper venom and/or silica

The risk of thrombosis in patients with lupus anticoagulants is predicted by their specific coagulation profile.

Lupus anticoagulants belong to the family of antiphospholipid antibodies. They include two phospholipid-dependent inhibitors of coagulation that may be distinguished on the basis of specific coagulation profiles generated from the comparison of the ratios of the Kaolin Clotting Time (KCT) and the dilute Russell's Viper Venom Time (dRVVT): when the ratio of the KCT exceeds that of the dRVVT, the plasma is allocated to the "KCT" coagulation profile, when the opposite occurs, the plasma is defined to belong to the "dRVVT" coagulation profile group. We prospectively followed-up a historical cohort

[An assay of lupus anticoagulants by use of plasma-agarose gel].

We developed a sensitive and quantitative method for assaying lupus anticoagulants. The method was based on the inhibition of fibrin formation in a plasma-agarose gel plate, which was described as a method for assaying hemophilic factor VIII-inhibitor (Bird, 1975). The final concentration of plasma in agarose gel was set up 30% instead of 50%. Fibrin formation was stopped, when the clear zone of normal plasma as negative control disappeared and that of 0.039 units heparin as positive appeared. Then this improved the precision of measurement. The method was not only more sensitive than the dilu

Antiphospholipid antibodies.

Antiphospholipid antibodies are prevalent in systemic lupus erythematosus. These antibodies are not specific for systemic lupus erythematosus but may be found in other diseases such as rheumatoid arthritis, mixed connective tissue disease. Sjögren's syndrome, syphilis and chronic idiopathic thrombocytopenic purpura. High serum levels of anticardiolipin antibodies in systemic lupus erythematosus are associated with circulating lupus anticoagulant, false positive VDRL and the occurrence of thrombosis, thrombocytopenia and recurrent abortions. The immunological reactions of the antiphospholipid a

[Acquired hemophilia A after an early abortion].

HISTORY: A 30-year-old woman was referred to our clinic because she had developed recurrent spontaneous hematomas of both calves within the last 2 months. 6 months earlier the patient had developed an ovarian hyperstimulation syndrome after ovarian stimulation treatment and intrauterine insemination. Shortly afterwards a missed abortion (8 (th) week) had been diagnosed. A curettage was carried out. INVESTIGATIONS: Routine coagulation tests confirmed a prolongation of aPTT to 90 s and a lupus anticoagulant. A high-titre factor VIII inhibitor (56 Bethesda units) was identified. TREATMENT AND C

A Case of Coagulation Dysfunction Caused by Antithrombin Pittsburgh Mutation.

BACKGROUND: In clinical practice, coagulation dysfunction resulting from the antithrombin Pittsburgh mutation is rare and prone to misdiagnosis. METHODS: Coagulation time was measured using coagulation methods, and coagulation factor activity was measured using chromogenic substrate methods. Platelet aggregation function was assessed by light transmission aggregometry (LTA), serum protein was determined through capillary electrophoresis, and mutations were detected by whole-exome high-throughput sequencing. RESULTS: A 16-year-old female patient had significantly prolonged activated partial t

[Cross-mixing test to detect lupus anticoagulant for diagnosis of antiphospholipid syndrome].

The activated partial thromboplastin time (APTT) is generally used to screen for intrinsic coagulation factor deficiency. APTT prolongation suggests a bleeding tendency, although it can also reveal the presence of lupus anticoagulant (LA), which is one of the antiphospholipid antibodies associated with a high risk of thrombosis. Therefore, differential diagnosis in patients with a prolonged APTT is important. Recently, a cross mixing test has been proposed as a useful laboratory examination for differential diagnosis in these patients. When a cross-mixing test utilizes a mixture of patient and

Lupus anticoagulant associated syndrome in benign and malignant systemic disease.

A strong correlation exists between the presence of an in vitro plasma coagulation inhibitor, named "lupus anticoagulant", and a clinical syndrome of recurrent arterial and venous thromboses, neurological abnormalities, repeated obstetrical complications, thrombocytopenia, a biologic false-positive serological test for syphilis, and a variety of rarer manifestations. This syndrome has predominantly been observed in patients with autoimmune diseases, but it may be of similar importance in association with monoclonal gammopathies. As an introduction to a detailed analysis of ten observations whi

Pitfalls in Coagulation Testing: A Pediatric Case of Lupus Anticoagulant Hypoprothrombinemia Syndrome Following Enterocolitis.

In children, the lupus anticoagulant (LA) is transiently produced after infection and rarely causes bleeding tendency, known as LA-hypoprothrombinemia syndrome (LAHPS). In this case report, we describe a child presenting with difficulty standing and walking after enterocolitis and complicated false coagulation abnormalities. Prolonged prothrombin time and activated partial thromboplastin time (APTT); decreased factors II, VIII, IX, XI, and XII; and inhibitors for factors VIII and IX were detected. On the basis of such findings as inhibitor pattern in the APTT cross-mixing test and positivity f

Viewpoint: Lupus anticoagulant detection and interpretation in antiphospholipid syndrome.

Lupus anticoagulant (LA) is a well-established risk factor for the clinical manifestations of antiphospholipid syndrome (APS). Accurate LA detection is an essential prerequisite for optimal diagnosis and management of patients with APS or aPL carriers. Variability remains a challenge in LA testing, with reliable detection influenced by multiple factors, including pre-analytical conditions, anticoagulation treatment, choice of tests and procedures performed, as well as interpretation of results, that can lead to false-positives or negatives. A standardised approach to LA testing, following curr

Coagulation mixing studies: Utility, algorithmic strategies and limitations for lupus anticoagulant testing or follow up of abnormal coagulation tests.

Coagulation testing underpins the investigation of hemostasis and/or monitoring of anticoagulation therapy for prevention and/or treatment of thrombosis related pathology. Assessment of coagulation results requires comparison against a normal reference range or interval (NRR/NRI). Results flagged as "abnormal" (ie, above the NRR/NRI for patients not on anticoagulant therapy), typically require further evaluation. eg, follow up or reflexive testing is used to identify the reason for prolongation, especially when supported by clinical context (eg, bleeding). Mixing tests may have utility to help
