Evaluation of the luteal phase.

The values of various methods used to evaluate the luteal phase, including basal body temperature, measurement of progesterone (P), endometrial biopsy, ultrasonographic measurement of endometrial thickness, and measurement of endometrial proteins, are reviewed. Luteal phase defect (LPD) is a controversial entity. The diagnosis of this condition is best based on a histological study of the endometrium. Methods to improve the accuracy of the diagnosis are discussed. LPD is more likely to be a result of an abnormal response of the endometrium to P, than to a subnormal production of P by the corpu

The defective luteal phase.

A defective luteal phase (DLP) results from a relative deficiency in secretion of progesterone by the corpus luteum. Approximately 30% of normal women show on biopsy an occasional DLP, while only 7-14% of infertile patients have a recurrent DLP. Endometrial biopsy remains the classical way to diagnose an inadequate luteal phase. However because of the discomfort and expense associated with endometrial biopsy, attention has turned to direct measurements of plasma progesterone levels as a means to rule out an inadequate luteal phase. A single plasma progesterone level of 10 ng/ml in the luteal p

Luteal phase after ovarian hyperstimulation.

The luteal phase was investigated in 17 women with normal menstrual cycles and tubal infertility who were superovulated with clomiphene (9 cycles), clomiphene plus pulsatile human menopausal gonadotrophin (hMG) (12 cycles) and clomiphene plus pulsatile follicle stimulating hormone (FSH) (11 cycles) during an in-vitro fertilization programme. Follicles were aspirated 34-36 h after the onset of the endogenous LH surge. Urinary total oestrogen levels during the first 6 days of the luteal phase were significantly higher, the duration of the luteal phase was significantly shorter and the luteal lev

Luteal phase stimulation, the future of fertility preservation? Retrospective cohort study of luteal phase versus follicular phase stimulation.

RESEARCH QUESTION: Is luteal phase stimulation capable of improving fertility preservation? MATERIALS AND METHODS: We performed a retrospective cohort study of consecutive ovarian stimulations, during July 2012 and September 2018 at Strasbourg University Teaching Hospital in France. Enrollment criteria were patients aged below 40 who had been referred to our center following a diagnosis of cancer or requiring gonadotoxic treatment. All patients enrolled had regular menstrual cycles and normal ovulation. Non-enrollment criteria were an expected low ovarian response (defined by an anti-Mülleria

Endocrinology in luteal phase and implantation.

The normality of the luteal phase after superovulation depends on the method and adequacy of the stimulation regimen. In comparison with natural cycles, the luteinisation of multiple follicles produces higher concentrations of steroids in blood, and soon after ovulation a correct ratio of serum oestradiol (E2) to progesterone is critical to establish viable pregnancies. Out of phase and asynchronous endometria can arise after stimulation with clomiphene (CC) alone or in association with human menopausal gonadotrophin (HMG), and pregnancies mostly arose in patients with advanced endometrial cha

Luteal Phase Defects and Progesterone Supplementation.

IMPORTANCE: Luteal phase defects (LPDs), or an insufficiency of progesterone production during the luteal phase of the menstrual cycle, have been identified as a potential cause of recurrent pregnancy loss (RPL), but its exact contribution to RPL is not well-defined. In addition, the role of exogenous progesterone supplementation during pregnancy remains controversial. OBJECTIVE: The goal of this review is to provide an updated, evidence-based summary of LPD, including prevalence and potential pathophysiologic mechanisms, and to explore the current controversies regarding progesterone supplem

Luteal Phase in Assisted Reproductive Technology.

Luteal phase (LP) is the period of time beginning shortly after ovulation and ending either with luteolysis, shortly before menstrual bleeding, or with the establishment of pregnancy. During the LP, the corpus luteum (CL) secretes progesterone and some other hormones that are essential to prepare the uterus for implantation and further development of the embryo, the function known as uterine receptivity. LP deficiency (LPD) can occur when the secretory activity of the CL is deficient, but also in cases of normal CL function, where it is caused by a defective endometrial response to normal leve

Luteal phase defects and ectopic pregnancy.

OBJECTIVE: To determine whether luteal phase defect (LPD) may be an etiologic factor in ectopic pregnancy (EP). DESIGN: All patients who were seen over a 6-year period with the chief complaint of infertility underwent an extensive infertility workup and were followed prospectively. The diagnoses of the causes of infertility were assigned retrospectively. SETTING: Two hospital-based tertiary care reproductive endocrine-infertility units. PATIENTS: A total of 1,077 infertility patients were evaluated. Of the 633 who became pregnant, the infertility had been due to LPD in 51 and to anovulation

Ovulation induction disrupts luteal phase function.

Abnormalities in the luteal phase have been detected in virtually all the stimulation protocols used in in vitro fertilization, on both the hormonal and endometrial levels. Supraphysiological follicular or luteal sex steroid serum concentrations, altered estradiol: progesterone (E2/P) ratio, and disturbed luteinizing hormone pituitary secretion leading to corpus luteum insufficiency or a direct drug effect have been postulated as the main etiologic factors. Luteinizing hormone supports corpus luteum function, and low LH levels have been described after human menopausal gonadotropin treatment,

Unusual cleaning behavior in the luteal phase.

18 undergraduate women recorded episodes of unusual cleaning, defined as excessive cleaning or cleaning of things not usually cleaned, and the dates of their menstruation over a 2-mo. period. Unusual cleaning was significantly more likely to occur during the luteal phase than at any other time in women not using oral contraceptives. Possible links between changes in progesterone levels and obsessive compulsive disorders are suggested.

The endometrium in natural and artificial luteal phases.

The normal human endometrium reacts precisely and sensitively to any hormonal stimulation with predictable changes. If the corpus luteum develops normally after ovulation, the progesterone secreted induces specific changes in endometrial glandular and stromal cells that can be dated by daily fine-structural alterations. With corpus luteum deficiency the endometrial differentiation is delayed and remains incomplete. The defect in the differentiation of glands and stroma varies in their distribution and intensity and may be dissociated or coordinated depending upon the cause of the corpus luteum

Impact of ovarian hyperstimulation on the luteal phase.

The contemporary approach to ovarian stimulation in IVF treatment results in supraphysiological concentrations of progesterone and oestrogen in the luteal phase. These sex steroids act directly and indirectly to mature the endometrium, thus influencing its receptivity to implantation. The development of endometrial receptivity is a complex process that may be altered by inappropriate exposure to sex steroids. Alterations in the oestrogen to progesterone ratio, growth factor concentrations and cell adhesion molecule profiles may occur after ovarian stimulation, potentially affecting the recepti

Prolactin in the evaluation of luteal phase in infertility.

Hyperprolactinemia was detected in 15 of 130 infertile patients (11.5%) with regular menstrual cycles and no galactorrhea who underwent luteal phase evaluation by basal body temperature (BBT), plasma estradiol (E2), and progesterone (P) determination, and endometrial biopsy (repeated in a later cycle when the first was defective). Luteal phase length and plasma levels of P and E2 were similar in the hyperprolactinemic and normoprolactinemic patients. Moreover, a significantly higher incidence of inadequate luteal phase, histologically documented, was found in the normoprolactinemic group. It i

Regulation of luteal function by luteinizing hormone and prolactin at different times of the luteal phase.

In 54 healthy women luteal function was assessed by sequential withdrawals of blood samples at 10-min intervals for 8-10 h. Subgroups of the women were studied during the early and late ovulatory period and during the early, mid- and late luteal phase. Bio- and immunoreactive luteinizing hormone (LH), prolactin, testosterone, estradiol and progesterone levels were determined in each sample. While the bio- and immunoreactivity of LH pulses correlated fairly well, a number of bio- or immunoreactive LH pulses were observed that were not detected by the respective other method. Responsivity of the

Luteal phase support in assisted reproductive technology.

Infertility affects one in six couples, with in vitro fertilization (IVF) offering many the chance of conception. Compared to the solitary oocyte produced during the natural menstrual cycle, the supraphysiological ovarian stimulation needed to produce multiple oocytes during IVF results in a dysfunctional luteal phase that can be insufficient to support implantation and maintain pregnancy. Consequently, hormonal supplementation with luteal phase support, principally exogenous progesterone, is used to optimize pregnancy rates; however, luteal phase support remains largely 'black-box' with insuf

Luteal phase support for assisted reproduction cycles.

BACKGROUND: Progesterone prepares the endometrium for pregnancy by stimulating proliferation in response to human chorionic gonadotropin (hCG), which is produced by the corpus luteum. This occurs in the luteal phase of the menstrual cycle. In assisted reproduction techniques (ART) the progesterone or hCG levels, or both, are low and the natural process is insufficient, so the luteal phase is supported with either progesterone, hCG or gonadotropin releasing hormone (GnRH) agonists. Luteal phase support improves implantation rate and thus pregnancy rates but the ideal method is still unclear. Th

Luteal phase support in assisted reproduction cycles.

BACKGROUND: The aspiration of the granulosa cells that surround the oocyte and the use of gonadotropin releasing hormone agonists (GnRHa) during assisted reproduction technology (ART) treatment can interfere with the production, during the luteal phase, of progesterone, which is necessary for successful implantation of the embryo. Providing hormonal supplementation during the luteal phase with either progesterone itself, or human chorionic gonadotropin (hCG), which stimulates progesterone production, may improve implantation and, thus, pregnancy rates. OBJECTIVES: To determine (1) if luteal p

Luteal phase defect. Etiology, diagnosis, and management.

Luteal phase defect is an ovulatory disorder of considerable clinical importance that is implicated in infertility and recurrent spontaneous abortion. As a subtle disruption of ovulatory or luteal function, it may be the most common ovulatory disorder in women. Pathophysiologic alterations of the complex reproductive process that lead to delayed endometrial maturation characteristic of LPD include disordered folliculogenesis, defective corpus luteum function, and abnormal luteal rescue by the early pregnancy. A variety of clinical conditions, such as hyperprolactinemia, hyperandrogenic states,

Luteal phase pregnancies in female sterilization patients.

Little is known of the incidence, predictors or outcomes of luteal phase pregnancies (LPPs) in women presenting for voluntary interval female sterilization. A series of 37 LPPs was identified from a large multicenter female sterilization data set. Each LPP case was matched with two nonpregnant (NP) controls for clinic, operator, surgical approach, tubal occlusion technique and date of surgery. The LPP cases did not have a distinct demographic or clinical profile, but were generally more fecund than NP controls. LPPs were further compared with a second unmatched group consisting of 123 true ste
