Mediastinal Lymphoproliferative Disorders.

Lymphoproliferative disorders comprise 50% to 60% of all mediastinal malignancies in both children and adults. Primary mediastinal involvement is rare (∼5%), whereas secondary mediastinal involvement by systemic disease is more common (10% to 25%). Primary mediastinal disease is defined as involvement by a lymphoproliferative disorder of mediastinal lymph nodes, the thymus, and/or extranodal mediastinal organs without evidence of systemic disease at presentation. In this review, the clinical, radiologic, histopathologic, immunohistochemical, and genetic features of some of the most characteris

Gastrointestinal lymphoproliferative disorders.

Malignant lymphomas can be first detected in some patients in endoscopic biopsies of the gastrointestinal (GI) tract. However, their recognition and accurate classification often pose problems for the pathologist for several reasons. First, the small sampling size limits pattern recognition and the number of ancillary studies which can be performed. Second, the immune system of the GI tract is capable of intense hyperplastic responses which may mimic lymphoma. Third, in a fashion similar to cutaneous lesions, those in the alimentary tract may be visualized and biopsied at a very early phase in

Benign lymphoproliferative disorders.

OBJECTIVE: To provide a review of three benign lymphoproliferative disorders commonly encountered in nursing practice: (1) infectious mononucleosis, (2) cat scratch disease, and (3) sarcoidosis. DATA SOURCES: Research studies, review articles, and book chapters pertaining to benign lymphoproliferative disorders. CONCLUSIONS: Benign lymphoproliferative disorders may be associated with infections, autoimmune disorders, hypersensitivity reactions, and unknown causes. Most of these disorders are self-limiting; however, some are associated with significant morbidity and mortality. IMPLICATIONS F

[Chronic lymphoproliferative disorders].

Chronic lymphoproliferative disorders include several entities that share an initial indolent presentation but are considered at present not to be curable. Follicular lymphoma are more frequent and well characterized; therapeutic recommendations are adapted to prognostic factors such as stage and tumour mass. Marginal zone lymphoma were more recently described and include extranodal MALT lymphomas as well as splenic, leukemic or nodal lymphomas, each with its own clinical features. Lymphocytic lymphomas resemble to chronic lymphocytic leukemia and can be treated in the same way, while mantle c

Chronic lymphoproliferative disorders.

Chronic lymphoproliferative disorders are a heterogeneous group of diseases with a highly variable clinical course. In the past few years, important progress has been made in the classification, biology, and therapy of some of these disorders of which B-cell chronic lymphocytic leukemia is not only the most frequent but also the model for their study. Regarding classification, entities that may be confounded with chronic lymphocytic leukemia have been identified (eg, splenic lymphoma with villous lymphocytes, mantle-cell lymphoma in leukemic phase); these diseases should be known and clearly s

Chronic lymphoproliferative disorders.

There have been important advances in the diagnosis and treatment of chronic lymphoproliferative disorders. Three diseases stand out as examples of progress: chronic lymphocytic leukemia (CLL), hairy-cell leukemia (HCL), and adult T-cell leukemia-lymphoma. Improvements in diagnosis are the result of greater attention to cell morphology and the routine use of monoclonal antibodies, which in turn help define composite phenotypes that are specific for CLL and HCL. The combination of morphology and immunophenotype allowed the characterization of a new disorder, splenic lymphoma with villous lympho

CD30-Positive Lymphoproliferative Disorders.

Primary cutaneous CD30-positive lymphoproliferative disorders (CD30+ LPD) encompass lymphomatoid papulosis (LyP), primary cutaneous anaplastic large cell lymphoma (pcALCL), and borderline lesions [1]. CD30+ LPD are the second most common cutaneous T-cell lymphomas (CTCL) after mycosis fungoides (MF) and represent approximately 25% of all CTCL cases [2]. Their common phenotypic hallmark is an expression of the CD30 antigen, a cytokine receptor belonging to the tumor necrosis factor (TNF) receptor superfamily. Both LyP and pcALCL show numerous clinical, histological and immunophenotypic variants

[Inheritance in lymphoproliferative disorders].

Lymphoproliferative disorders, especially chronic lymphocytic leukaemia (CLL), non-Hodgkin's lymphomas, Hodgkin's lymphoma and multiple myeloma are regarded as a hereditary entity with pleiotropic clustering in families, although the genuine alleles have not been found so far. The world-wide highest incidence of CLL and the existence of a systematic cancer registration since 1943 make Denmark a perfect place for epidemiological and genealogical investigations in the search for the genetics of the lymphoproliferative disorders. In Scandinavia, we see no signs of anticipation but marked linkage

Immunodeficiency-associated lymphoproliferative disorders.

The incidence of lymphoproliferative disease is significantly higher in individuals who have congenital, acquired, or iatrogenically induced immunodeficiency. The immunodeficiency-associated lymphoproliferative disorders are clinically and pathologically heterogeneous, are of variable clonal composition, and vary according to the immunodeficiency syndrome. Nonetheless, they share several features, including frequent origination in or involvement of extranodal sites, diffuse aggressive histology, B-cell lineage derivation, association with the Epstein-Barr virus (EBV), and, often, rapid clinica

Post-transplant lymphoproliferative disorders.

Post-transplant lymphoproliferative disorder (PTLD) is one of the most dreaded complications of orthotopic transplantation. It consists of a heterogeneous group of lymphoproliferative disorders of varying clonal composition, occurring in immuno-suppressed organ allograft recipients and is frequently due to EBV infection. It is most common in heart/lung transplants followed by heart, liver, and kidney and rarely in bone marrow transplants. Clinically, PTLD can present in a number of ways ranging from features resembling infectious mononucleosis, lymphoproliferative masses involving both nodal a

Post-Transplant Lymphoproliferative Disorders.

Post-transplant lymphoproliferative disorders (PTLDs) are a heterogenous set of unregulated lymphoid cell proliferations after organ or tissue transplant. A majority of cases are associated with the Epstein-Barr virus and higher intensity of pharmacologic immunosuppression. The clinical presentations are numerous. The diagnosis is ideally by histology, except in cases where the tumor is inaccessible to biopsy. While some pre-emptive therapies and treatment strategies are available have reasonable success are available, they do not eliminate the high morbidity and significant mortality after PT

[EBV-associated lymphoproliferative disorders].

Epstein-Barr-virus-associated lymphoproliferations (EBV-LPD) constitute a wide spectrum from benign, self-limiting lymphoproliferations to malignant lymphoma. Because of the clinical and morphological heterogeneity of EBV-LPD and the high prevalence of EBV infections, knowledge of the diagnostic criteria is of great importance for the practice of diagnostic pathology. In this review, clinical and pathological characteristics of the non-malignant EBV-related LPD in patients with and without immunosuppression are presented and their current classification is discussed.EBV-LPD can be of B‑cell or

Lymphoproliferative and myeloproliferative disorders.

Nomenclature regarding neoplasia of the hematopoietic and lymphoid tissues in the horse is confusing. This article will clarify terminology, and discuss the individual lymphoproliferative and myeloproliferative disorders recognized in the horse. Diagnostic techniques that are useful in cases in which hematopoietic or lymphoid tissue neoplasia are suspected include histochemical staining profiles, bone marrow aspiration, and bone marrow biopsy.

This article will focus on the cutaneous lymphoproliferative disorders associated with EBV, with an emphasis on the upcoming changes in the revised 4th Edition of the WHO classification of tumors of the hematopoietic system, many of which deal with cutaneous disorders derived from NK-cells or T-cells. Extranodal NK/T-cell lymphoma usually presents in the upper aerodigestive tract, but can involve the skin secondarily. EBV-associated T- and NK-cell lymphoproliferative disorders (LPD) in the pediatric age group include the systemic diseases, chronic active EBV infection (CAEBV) and systemic EBV+

Primary cutaneous CD30(+) lymphoproliferative disorders are the second most common group of cutaneous T-cell lymphomas (CTCL) and include lymphomatoid papulosis (LyP) and primary cutaneous anaplastic large T-cell lymphoma (cALCL). Both disease entities share overlapping clinical, histopathological, and molecular features, thus representing a spectrum of cutaneous CD30(+) lymphoproliferative disorders. LyP may be distinguished from cALCL by clinicopathological correlation. In some patients, both diseases may coexist at initial diagnosis or develop over the course of the disease. Mycosis fungoid

Lymphoproliferative disorders associated with hypereosinophilia.

Hypereosinophilia, defined as peripheral blood eosinophil counts > 1,500/μL, may complicate the course of various lymphoproliferative disorders. Among these, Hodgkin lymphoma (HL) and certain peripheral T-cell lymphomas (PTCLs) derived from CD4 cells, including Sezary syndrome (SS), adult T-cell leukemia/lymphoma (ATLL), and angioimmunoblastic T-cell lymphoma (AITL), are most commonly associated with increased reactive eosinophilopoiesis. Rarely, marked hypereosinophilia (HE) may occur in the setting of acute B-cell lymphoblastic leukemia, with a substantial impact on disease course. The mecha

[Primary cutaneous CD30+ lymphoproliferative disorders].

CD30+ lymphoproliferative disorders are the most common group of cutaneous T-cell lymphomas after mycosis fungoides and its subtypes. This group includes lymphomatoid papulosis and CD30+ anaplastic large-cell lymphoma; these 2 entities are the extremes of a spectrum with numerous intermediate varieties in which it is not possible to establish a clear diagnosis based on clinical and histopathologic criteria. CD30+ lymphoproliferative disorders must be differentiated from other lymphoproliferative diseases with CD30+ cells in the tumor infiltrates, such as mycosis fungoides or Hodgkin disease, a
