Macular edema. Miscellaneous.

This chapter provides the reader with practical information to be applied to the various remaining causes of macular edema. Some clinical cases of macular edema linked to ocular diseases like postradiotherapy for ocular melanomas remained of poor functional prognosis due to the primary disease. On the contrary, macular edema occurring after retinal detachment or after diverse systemic or local treatment use is often temporary. Macular edema associated with epiretinal membranes or vitreomacular traction is the main cause of poor functional recovery. In other cases, as in tractional myopic vitre

Pseudophakic cystoid macular edema.

PURPOSE OF REVIEW: Pseudophakic cystoid macular edema (PCME) is the most common postoperative complication of cataract surgery, resulting in visual decline. In this review, we discuss its pathophysiology, epidemiology, clinical presentation, and the current available evidence on therapeutic management. RECENT FINDINGS: Patients with diabetes mellitus have twice the risk of developing PCME as compared to nondiabetic individuals. Recent large database studies have revealed an increased risk among young, male, and black patients. A previous history of PCME is perhaps the strongest risk factor fo

Drug-induced macular edema.

Macular edema constitutes a serious pathologic entity of ophthalmology resulting in vision loss with a remarkable impact on the quality of life of patients. It is the final common pathway of various systemic diseases and underlying intraocular conditions, with diabetes mellitus being the most frequent cause. Other causes include venous occlusive disease, intraocular surgery, and inflammatory conditions of the posterior segment of the eye. Macular edema is a recognized side effect of various systemic and local medications and requires special consideration among ophthalmologists and other clini

Pseudophakic cystoid macular edema.

PURPOSE OF REVIEW: Pseudophakic cystoid macular edema (PCME) is a common cause of visual impairment after cataract surgery. This article systematically reviews and discusses the epidemiology, risk factors, diagnosis, and treatment of PCME, with a focus on advances in the past 1-2 years. RECENT FINDINGS: The incidence of PCME has declined with the advent of modern surgical techniques. Optical coherence tomography (OCT) has become an important adjunct to biomicroscopy and fluorescein angiography. PCME prophylaxis with topical nonsteroidal anti-inflammatory drugs remains unproven because long-te

Postsurgical cystoid macular edema.

Cystoid macular edema (CME) is a primary cause of reduced vision following both cataract and successful vitreoretinal surgery. The incidence of clinical CME following modern cataract surgery is 0.1-2.35%. Preexisting conditions such as diabetes mellitus and uveitis as well as intraoperative complications can raise the risk of developing CME postoperatively. The etiology of CME is not completely understood. Prolapsed or incarcerated vitreous and postoperative inflammatory processes have been proposed as causative agents. Pseudophakic CME is characterized by poor postoperative visual acuity. Flu

Mechanisms of macular edema.

Macular edema is the pathological accumulation of fluid in the central retina. It is a complication of many retinal diseases, including diabetic retinopathy, retinal vascular occlusions and uveitis, among others. Macular edema causes decreased visual acuity and, when chronic or refractory, can cause severe and permanent visual impairment and blindness. In most instances, it develops due to dysregulation of the blood-retinal barrier which permits infiltration of the retinal tissue by proteins and other solutes that are normally retained in the blood. The increase in osmotic pressure in the tiss

[Pseudophakic cystoid macular edema].

Pseudophakic cystoid macular edema (PCME) is still one of the most frequent causes of loss of vision after cataract surgery. Clinical PCME occurs after initial improvement of vision 2-10 weeks after surgery in 1-2% with secondary vision loss and biomicroscopically visible macular cysts. Alterations, which are often not associated with functional limitations, can frequently be seen with angiography and optical coherence tomography (OCT). Inflammation mediators are released due to the surgical trauma in the anterior segment of the eye and diffuse to the capillaries of the macula where they lead

Macular Edema - Rationale for Therapy.

Macular edema represents the end-stage of multiple pathophysiological pathways in a multitude of ocular vascular, inflammatory, and other diseases. The rationale for clinical treatment of macular edema is based on the understanding and the inhibition of these pathophysiological mechanisms. When macular edema is caused by a generalized health problem such as diabetes, high blood pressure, or generalized inflammatory conditions, treatment of these generalized diseases can in many cases cure macular edema directly. In ocular diseases, the local exudation of fluid from blood vessels is governed by

[Cystoid macular edema in pseudophakia].

One hundred and eighty six patients (217 eyes) after cataract extraction with intraocular lens implantation were analyzed in search of cystoid macular oedema. In general--9 cases of cystoid macular oedema were found, among them 5 patients after intracapsular cataract extraction with implantation of an anterior chamber lens (9.8 p.c.) and 4 after extracapsular cataract extraction with implantation of a posterior chamber lens (2.7 p.c.). One could observe a beneficial influence of a prophylactic application of indomethacin compounds in order to lessen the incidence of the macular oedema.

Pathology of cystoid macular edema.

In order to define the pathology of cystoid macular edema (CME), 35 human cases with histopathologic features of CME were reviewed. The macular cysts appeared in different layers of the retina in eyes after cataract extraction, retinal vein occlusion, trauma, diabetes mellitus, and accelerated hypertension. The pathologic process varied from transudation, exudation to liquefaction necrosis. The experimental models of lens extraction and talc retinopathy in rhesus monkeys were examined. Disruption of the blood-retinal barrier at the retinal vasculature and retinal pigment epithelium were noted

Pathogenesis of aphakic macular edema.

Six months after cataract extraction, a 71-year-old man had cystoid macular edema confirmed by fluorescein angiography. Microscopic examination revealed cystoid spaces in the outer plexiform and outer nuclear layer of the neurosensory retina. Chronic inflammatory cells were scattered throughout the ciliary body. Inflammatory cells were also present in a section of iris that had an adherent strand of vitreous leading to the pars plana. Comparison of these findings with other related clinical entities supported the hypothesis that ciliary body inflammation may play a significant role in postcata

Diabetic macular edema: Upcoming therapies.

Diabetic macular edema (DME) is a serious vision-threatening complication that can arise at any stage of diabetic retinopathy. Primary treatment involves anti-vascular endothelial growth factor (VEGF) agents, which are highly effective but associated with challenges, such as the need for frequent injections, relapses, and resistance to therapy. Therefore, there has been a growing interest in developing new treatments that offer similar or superior outcomes in DME. This review article explores emerging treatments, including WNT agonists, gene therapy, protein inhibitors, and, most importantly,

Macular Edema in Childhood Uveitis.

BACKGROUND: Pediatric uveitis is associated with a high incidence of severe and frequently permanent visual loss. This article summarizes the current understanding of the disease and the therapeutic options that are available to improve treatment outcomes. METHODS: A Medline search spanning the last 10 years was undertaken using the key terms "pediatric uveitis" or "childhood uveitis" and "macular edema". Articles which appertained to case reports or small case series were excluded from consideration, whereas those in which the opinions of experts were expressed, as well as reviews, were not.

Macular edema--rationale for therapy.

Blood-retinal barrier breakdown with macular edema is caused by many diseases, which modulate--via different growth factors--the integrity of the tight junctions. Starling's law predicts furthermore that macular edema will develop if the hydrostatic pressure gradient between capillary and retinal tissue is increased, for example in the presence of elevated blood pressure, or if the osmotic pressure gradient is decreased, for example when protein accumulates excessively in the extracellular space within the retina. The rationale for clinical treatment of macular edema is based on the understand

[Cystoid macular edema in uveitis].

Cystoid macular edema (CME) is the most frequent cause of visual deterioration in uveitis patients. Intraocular inflammation disturbs the blood-retina barrier and leads to retinal edema. The basis of successful treatment is the anti-inflammatory and immunosuppressive therapy of uveitis. Restoration of the blood-retina barrier is mediated by corticosteroids and nonsteroidal anti-inflammatory agents. Resorption of extracellular fluid is improved by systemic carboanhydrase inhibitors. Despite aggressive therapy loss of visual acuity is frequent. Therefore, early diagnosis of CME and initiation of
