Within-subject comparisons of implant-supported mandibular prostheses: choice of prosthesis.

Although previous studies have demonstrated that implant-supported prostheses are more satisfactory and efficient for edentulous patients than are conventional prostheses, until now no investigation has directly compared different types of implant-supported prostheses. We carried out a within-subject cross-over clinical trial with fixed and long-bar removable implant-supported mandibular prostheses. Fifteen subjects were randomly divided into two groups. One group received the fixed prosthesis first, while the other first received the removable. After a two-month adaptation period, psychometri

Implant-supported prosthesis in a child with hereditary mandibular anodontia: the use of ball attachments.

This report describes the provision of an implant-supported hybrid overdenture in an 8-year-old child with mandibular anodontia. An interdisciplinary approach to treatment planning was employed. Two implant fixtures were placed in the canine regions and ball attachments connected to provide support and retention for the prosthesis. There was only one implant-related complication during the first follow-up year. Close follow-up during the child's growing years is essential for the maintenance of the peri-implant tissues and to replace the prosthesis as mandibular growth occurs.

A Novel 3D-Printed Titanium Prosthesis with Osseointegrated Dental Implants for Reconstruction of Segmental Mandibular Defects.

INTRODUCTION: Segmental mandibular defects present complex reconstructive challenges and are associated with significant functional and aesthetic morbidity. While vascularized bone flaps remain the standard for reconstruction, they are associated with donor-site morbidity, prolonged operative times, and limitations in predictable dental rehabilitation. This proof-of-concept study evaluated the mechanical feasibility, in a synthetic bench-top model, of a patient-specific, 3D-printed titanium mandibular prosthesis incorporating conventional dental implants for both dental rehabilitation and biol

Pediatric Mandibular Defect Rehabilitation by Human Transforming Growth Factor-β3 with an Implant-Supported Prosthesis.

Large mandibular defects in children are an uncommon but challenging problem for surgeons to solve. The time-honored options of autologous bone grafts are seldom a viable option, as suitable donor sites are unavailable. Osteoinductive morphogens may yet provide a solution in these cases. A large mandibular tumor in a child 10 years of age necessitated the resection of the entire dentate portion of the mandible. The defect was reconstructed at a second stage with a composite graft of human transforming growth factor-β3 (hTGF-β3), human demineralized bone matrix, and 12 g of autologous bone harv

Digital technique for anatomic design and bone-implant interface analysis of a custom bar for a mandibular implant-supported fixed dental prosthesis.

With the evolution of computer-aided design and computer-aided manufacturing (CAD-CAM), the digitally guided fabrication of implant-supported fixed dental prostheses has become increasingly precise and customized. Conventional substructure designs may fail to ensure a passive fit or adequate biomechanical performance in edentulous patients with severe mandibular bone resorption. Anatomic limitations and load concentration at the bone-implant interface present significant challenges for long-term prosthetic success. This article describes a step-by-step digital technique for designing a custom

Mandibular Flexure and Crestal Bone Stress Distribution on an Implant-Supported Fixed Full Arch Mandibular Prosthesis: Finite Element Analysis in Three Dimensions.

Aim This study's objective was to assess and analyze, using 3D Finite Element Analysis, the impact of four mandibular complete arch superstructures on the distribution of stress in the crestal bone during mandibular flexure. Materials and methods Four Finite element models of the mandible with different implant-retained framework designs have been developed. Three of these models had six axial implants placed at intervals of 11.8 mm, 18.8 mm and 25.8 mm from the midline, respectively. One model had two tilted implants and four axial implants splinted with a single piece of framework at interva

Prosthetic framework improvement using lattice structure: A comparative finite element study of a mandibular implant-supported prosthesis.

OBJECTIVES: An alternative option was proposed regarding the prosthetic rehabilitation of a fully edentulous mandible using only four implants. The aim was to reduce the stiffness of the prosthetic framework. To that end, the alternative option consists of a prosthetic framework optimized with a porous structure. Mechanical differences were analyzed between non-prosthetic mandible and restored mandible either with a conventional bulk titanium framework or with this alternative option. The non-prosthetic mandible corresponds to the mandible in its natural state, without prosthesis. This will be

Rehabilitation of an irradiated mandible after mandibular resection using implant/tooth-supported fixed prosthesis: a clinical report.

Patients undergoing mandibular resection often have facial asymmetry and cosmetic disfigurement, which include a retruded, deviated mandible, motor and sensory deficiencies, and abnormal intermaxillary relationship and occlusion. An implant-supported fixed prosthesis can be an optimal treatment modality. However, there is a problem in creating a repeatable, stable plane of occlusion and maxillomandibular relationship. This report describes the treatment sequence and considerations to rehabilitate a patient undergoing mandibular resection and radiotherapy with an implant-supported fixed prosthe

Accuracy of individual mandibular motion records using intraoral scanner for fixed implant- supported prosthesis designs: a comparative study.

BACKGROUND: Accurate occlusal registration is critical for successful dental implant treatments. Traditional mechanical articulation provides insight into occlusal relationships but cannot replicate complex mandibular movements accurately. CAD/CAM advancements, including virtual articulators, enable dynamic occlusion analysis using either average-value settings or individualized mandibular movement data, depending on the system used. Patient Specific Motion (PSM) technology, using intraoral scanners to record actual mandibular movements, offers enhanced functional occlusal design. Despite thes

An early loaded implant-supported mandibular complete arch fixed prosthesis in a young completely edentulous patient: a case report.

The restoration of the mandibular arch up to the first molars with 5 one-piece implants presents a viable and cost-effective treatment plan in patients with adequate bone volume and favorable ridge relationships. The early loading of the implants with a provisional restoration reduces the period of edentulousness and restores the patient's ability to eat, talk, and smile effectively. Implants in younger patients prevent resorption of the residual alveolar ridge. This article presents a case report of the restoration of an edentulous mandibular arch with an early loaded implant-supported fixed

Immediate functional loading of an implant-supported fixed prosthesis at the time of ablative surgery and mandibular reconstruction for squamous cell carcinoma.

The authors describe a case of squamous cell carcinoma of the oral cavity managed by ablative surgery, mandibular reconstruction with a fibula free flap, and implant placement during the same session. Immediate functional implant loading, respecting the principles of basal implantology, was performed 48 hours later using a highly rigid, screw-secured fixed prosthesis that served as an external fixator for the implants and grafted bone. Implant loading before external beam radiotherapy improves flap stability, bone consolidation, and quality of life. Functional and esthetic outcomes were evalua
