The Effects of Exercise and Manual Manipulation on Sacroiliac Joint Dysfunction: A Critically Appraised Topic.

CLINICAL SCENARIO: Sacroiliac joint dysfunction (SIJD) is pain in the sacroiliac region that can be debilitating and difficult to treat. Many interventions can be used to treat SIJD, including exercise and manual manipulation. The effectiveness of these treatments by themselves, or in conjunction, is not well understood. CLINICAL QUESTION: In active individuals with SIJD, is an exercise intervention, manual manipulation, or a combination of the 2 effective in reducing pain and improving function? SUMMARY OF KEY FINDINGS: Three studies were included in this critically appraised topic. One stu

Small-incision manual extracapsular cataract extraction using deep-topical, nerve-block anesthesia.

BACKGROUND AND OBJECTIVE: To determine whether deep-topical anesthesia is suitable for small-incision manual extracapsular cataract extraction (ECCE). PATIENTS AND METHODS: Three hundred twenty-six eyes of 253 patients had small-incision manual ECCE under topical anesthesia with a 4% lidocaine-soaked sponge. The severity of the pain, eye movements, blepharospasm, and intraoperative complications were recorded. Patient and surgeon satisfaction levels were assessed. RESULTS: Operations on 323 eyes (99%) were completed with topical anesthesia. Intraoperatively, topical anesthesia was converted

Manual small-incision cataract surgery under topical anesthesia for post-uveitis complicated cataract.

PURPOSE: Cataract development is a common sequelae associated with uveitis. Despite phacoemulsification being the popular method of cataract surgery today, manual small-incision cataract surgery (MSICS) may still be a safe and effective alternative because of several inherent benefits. There is not much literature and studies on the efficacy and safety of MSICS under topical anesthesia in complicated cataract in patients with uveitis. We aimed to study the safety and visual outcome of MSICS under topical anesthesia for post uveitis complicated cataract. METHODS: This was a retrospective obser

Comparing Themes Extracted via Topic Modeling and Manual Content Analysis: Korean-Language Discussions of Dementia on Twitter.

We randomly examined Korean-language Tweets mentioning dementia/Alzheimer's disease (n= 12,413) posted from November 28 to December 9, 2020, without limiting geographical locations. We independently applied Latent Dirichlet Allocation (LDA) topic modeling and qualitative content analysis to the texts of the Tweets. We compared the themes extracted by LDA topic modeling to those identified via manual coding methods. A total of 16 themes were detected from manual coding, with inter-rater reliability (Cohen's kappa) of 0.842. The proportions of the most prominent themes were: burdens of family ca

The TAMERE Randomized Controlled Trial: Comparative Effectiveness of Topical Manual Pain Therapy Versus Exercise Therapy in Chronic Nonspecific Neck Pain.

OBJECTIVES: We compared topical manual pain therapy (TMPT), a myofascial manual therapy using patient-guided pressure techniques, with exercise training, in adults with chronic nonspecific neck pain. METHODS: In this parallel (1:1) randomized controlled trial, 91 participants (68 women, 23 men) with neck pain ≥3 months were allocated to 3 weeks of TMPT (n = 51) or exercise training (n = 40). TMPT included six therapist-guided myofascial massage sessions; exercise training involved resistance-based neck and shoulder exercises. The primary outcome was pain intensity (100-point visual analogue sc

One step beyond tomorrow; Ranjan manual small-incision cataract surgery (MSICS) marker - Welcome to the topical, flapless and astigmatism-free MSICS era.

The prevalence of blindness in India is 14.9 per 1000. Cataract causes 80% of this blindness. Most of these blinds are in the rural areas while the surgical service delivery channels are concentrated in the urban areas. This situation has many social impacts like loss of productivity, breakdown of interpersonal relationships, depressive manifestations, loss of self-esteem, and isolated humiliating life. Manual small-incision cataract surgery (MSICS; also SICS) is a low-cost, small-incision, high-valued cataract surgery that is principally employed in the developing world. In poor settings, MSI

Comparison of Subconjunctival Triamcinolone with Topical Prednisolone for Routine Anti-Inflammatory Prophylaxis in Manual Small Incision Cataract Surgery: A Single-Center, Randomized Controlled Trial Pilot Protocol.

BACKGROUND: Cataract surgery is the most commonly performed operation worldwide. Prophylaxis against postoperative endophthalmitis and inflammation currently includes perioperative, patient-administered instillation of topical antibiotic and anti-inflammatory drops in the US, Europe and Latin America. As patients can face challenges with adherence to topical drops for self-instillation, injected drugs may offer a solution. Intracameral antibiotic injection is superior to drops for endophthalmitis prevention in manual small incision surgery (MSICS) and phacoemulsification eyes. Subconjunctival

Use of Topical Lidocaine Gel Plus Paracervical Blockade vs. Paracervical Blockade Alone for Pain Management During Manual Vacuum Aspiration: ADouble-Blind, Randomized, Placebo-Controlled Trial.

OBJECTIVE: To evaluate if the use of lidocaine gel applied to the cervix prior to manual vacuum aspiration (MVA) in addition to paracervical blockade is useful in reducing the level of pain associated with the procedure. METHODS: A total of 88 patients were randomized to receive either 5 mL of lidocaine gel or a placebo applied topically to the cervix 5 minutes prior to paracervical blockade. Both groups received the same drugs for pain control (tramadol hydrochloride, diazepam, and sodium diclofenac). A visual analogue scale (VAS) was used for evaluation of pain intensity at two times: 2 min

The 1918 Influenza Pandemic Versus COVID-19: A Historical Perspective From an Italian Point of View.

The ongoing COVID-19 pandemic has a major precedent almost exactly a century ago: the world-famous H1N1 influenza virus pandemic, sometimes known to the general public as the Spanish flu. From a history of medicine perspective, it is possible to underline many potential common traits between the two. In this article, hygiene and prophylaxis strategies are analyzed in a review of the most popular Italian general medical journals at the time of Spanish flu, Il Policlinico being the most representative of them. The analysis included 40 original journal articles as well as important references to

Sleep disturbances in bereavement: A systematic review.

Severe, persistent and disabling grief occurs among a sizable minority experiencing bereavement, with diagnostic manuals newly including complicated grief (CG) disorders. Sleep disturbances/disorders have been established as worsening affective and stress-related conditions. However, the role of sleep difficulties in bereavement and CG has not received similar scientific attention. We therefore conducted a systematic review with narrative syntheses on this topic to clarify the role of sleep in bereavement (PROSPERO: CRD42018093145). We searched PubMed, Web of Science and PsychInfo for peer-rev
