Nutritional risk amongst community-living Maori and non-Maori older people in Hawke's Bay.

INTRODUCTION: Maintaining good nutrition is vital for healthy ageing. Poor nutrition increases the risk of hospitalisation, disability and mortality. Research shows clinical malnutrition is preceded by a state of nutritional risk and screening can identify older people at risk of poor nutrition or who currently have impaired nutritional status. AIM: To assess the population prevalence of nutritional risk amongst community-living Maori and non-Maori older people in Hawke's Bay. METHODS: A postal survey of 1268 people aged 65 years or older on the electoral roll for Hawke's Bay was conducted.

Genetics of club foot in Maori and Pacific people.

The role of major gene and multifactorial inheritance in the aetiology of club foot in the New Zealand Polynesian population was studied using 287 New Zealand Maori and Pacific club foot families. The club foot family data were analysed by complex segregation analysis under the mixed model using the computer program POINTER. This analysis shows that the best genetic model for club foot in this population is a single dominant gene with a penetrance of 33% and a predicted gene frequency of 0.9%. These data provide a scientific foundation for molecular studies in the Maori and Polynesian populati

The incidence of treated end-stage renal disease in New Zealand Maori and Pacific Island people and in Indigenous Australians.

BACKGROUND: Although Indigenous Australians, New Zealand Maori and Pacific Island people comprise an unduly high proportion of patients treated for end-stage renal disease (ESRD) in the two countries, no population-based age- and disease-specific rates have been published. METHODS: From data provided to the Australia and New Zealand Dialysis and Transplant Registry (ANZDATA), truncated age- and sex-standardized incidence rates were calculated for treated ESRD due to all causes and by primary renal disease, in four broad age groups of Maori, Pacific Island people and all 'other' New Zealanders

An assessment of the Hua Oranga outcome instrument and comparison to other outcome measures in an intervention study with Maori and Pacific people following stroke.

AIM: Health outcomes research for Maori has been hampered by the lack of adequately validated instruments that directly address outcomes of importance to Maori, framed by a Maori perspective of health. Hua Oranga is an outcome instrument developed for Maori with mental illness that uses a holistic view of Maori health to determine improvements in physical, mental, spiritual and family domains of health. Basic psychometric work for Hua Oranga is lacking. We sought to explore the psychometric properties of the instrument and compare its responsiveness alongside other, more established tools in a

Cultural enhancement of a clinical service to meet the needs of indigenous people; genetic service development in response to issues for New Zealand Maori.

The delivery of good health care services within clinical settings is predicated by an understanding of the needs of the stakeholders. Most of the information generated to date on the transfer of mutational analysis to clinical service has been within a Eurocentric model favouring individual autonomy. It is predictable that this model does not easily translate for other cultures. Current genetic technology has elucidated the molecular basis of many diseases. In familial cancer and other late-onset disorders, there is now the possibility of 'prediction' where a high risk conferred by family his

A Maori perspective of health.

Health is not a universal concept nor are health professionals necessarily best suited to formulate the health aspirations of a people. Like other fundamental objectives, health is defined for Maori people by their elders, at traditional tribal gatherings. Four cornerstones of health have been recognised: te taha wairua (a spiritual dimension), te taha hinengaro (a psychic dimension), te taha tinana (a bodily dimension), te taha whanau (a family dimension). Between Maori elders and Western health professionals, priorities for health are likely to differ, the Western approach emphasising person

Trends in Maori mental health in Otago.

OBJECTIVE: This paper outlines the methods used, and preliminary descriptive data collected, in a study on a cohort of Maori and non-Maori patients admitted to the inpatient psychiatric services in Otago between 1990 and 1992. METHOD: The notes of 42 Maori and 217 non-Maori first admissions to psychiatric inpatients were reviewed. Information concerning this admission was entered onto a database and analysed. RESULTS: The Maori admission rate was 4 per 1000 compared with 1 per 1000 for non-Maori people. This was higher than expected based on Otago population figures. Rates of family psychiat

Characteristics of sarcoidosis in Maori and Pacific Islanders.

BACKGROUND AND OBJECTIVE: Ethnicity is strongly associated with variable clinical presentation in sarcoidosis but the association between ethnicity and clinical characteristics has not previously been described in patients of Polynesian ancestry, Maori and Pacific Islander (PI). The objective of this study was to describe the clinical characteristics of sarcoidosis in Maori and PI patients and determine if those were different to European patients. METHODS: A retrospective review of the medical records of 406 patients (69 Maori/PI) attending a specialist interstitial lung disease (ILD) clinic

Amylin gene promoter mutations predispose to Type 2 diabetes in New Zealand Maori.

AIMS/HYPOTHESIS: Amylin gene mutations are known to predispose Chinese and Japanese subjects, but not Caucasian subjects, to Type 2 diabetes. New Zealand Maori, who have a high prevalence of Type 2 diabetes, have genetic origins in South East Asia. Amylin gene mutations could therefore predispose New Zealand Maori to Type 2 diabetes. METHODS: The amylin gene was screened for mutations in the proximal promoter region, exons 1 and 2, intron 1, and coding region of exon 3 by polymerase chain reaction amplification and direct sequencing of 131 Type 2 diabetic Maori patients and 258 non-diabetic M

Mortality and social class in Maori and nonMaori New Zealand men: changes between 1975-7 and 1985-7.

AIMS: Social class mortality differences in Maori and nonMaori New Zealand men aged 15-64 years have previously been examined for the period 1975-7. The analysis has now been repeated for the period 1985-7 in order to examine changes over time. METHODS: Numerator data were obtained from national death registrations and denominator data were obtained from the 1976 and 1986 censuses. These were used to calculate age-standardized death rates in Maori and nonMaori. RESULTS: Mortality declined by 28% in Maori and 14% in nonMaori between 1975-7 and 1985-7. The death rate for diseases amenable to m

Problem gambling risk factors and associated behaviours and health status: results from the 2002/03 New Zealand Health Survey.

AIMS: To investigate the extent of current problem gambling in New Zealand, and the risk factors, addictive behaviours, and self-rated health status associated with problem gambling. METHODS: Analysis of the gambling questions from the 2002/03 New Zealand Health Survey, which interviewed 12,529 people aged 15 years and over, and included increased sampling of Maori, Pacific, and Asian people. RESULTS: Approximately 1.2% (95% confidence interval: 1.0-1.5) of the New Zealand adult population were found to be current problem gamblers, representing an estimated 32,800 (26,200-39,400) people. The incidence of hepatocellular carcinoma in New Zealand.

AIM: To measure the incidence rate of hepatocellular carcinoma (HCC) in New Zealand, by ethnicity, sex, region and age. METHODS: Cancer Registry data for 1983-94 were used to calculate rates of primary liver cancer (ICD code 155.0) and HCC (both ICD code 155.0 and ICD-O morphology code 8170) directly standardised to Segi's world population and standardised for region. RESULTS: Rates of HCC per 100,000 person years 6.6 and 1.3 for Maori males and females, 14.7 and 4.6 for Pacific people, and 0.8 and 0.3 for Other (inclusive of Chinese). The rates for Chinese for 1989-94 were 19.9 and 5.8. The

Racial and environmental factors in acute haematogenous osteomyelitis in New Zealand.

Data on the hospital incidence of acute haematogenous osteomyelitis in New Zealand children between 1965 and 1973 are presented. The incidence in Maori children is four times that in European children. The condition is more common in the North Island than the South Island, but this difference is mainly due to the distribution of the Maori people. Admissions to hospital are common in late summer and autumn. The relationship of osteomyelitis to skin and ear sepsis and some possible reasons for ethnic difference, are discussed.

Mā te Whakarongo-a qualitative study exploring the impact of middle ear disease on New Zealand Māori.

AIM: The study aimed to explore the impact of middle ear disease on the lives of New Zealand Māori. Ear disease is common, yet there is a paucity of research into the effect it has on people's lives, particularly indigenous populations. METHOD: The study used Kaupapa Māori-based qualitative methodology and involved a series of seven semi-structured interviews with Māori adults living with middle ear disease. RESULTS: All participants felt there were delays in recognition and treatment of their ear condition and that there were barriers to accessing healthcare. The ear condition prevented par

Ethnicity, socioeconomic deprivation and consultation rates in New Zealand general practice.

OBJECTIVES: To explore the relationship between ethnicity, socioeconomic deprivation and utilization of general practice. METHODS: Data routinely collected by New Zealand general practices in 2001 were analysed using generalized linear regression. RESULTS: The mean number of doctor consultations for registered individuals was 3.7 and the median 2. After adjusting for age, gender and area socioeconomic deprivation, people from Maori, Pacific, Asian and other-unspecified ethnic groups consulted general practitioners at similar rates or less frequently than European people. There was a signific

Mood and anxiety disorders in Australia and New Zealand's indigenous populations: A systematic review and meta-analysis.

The Indigenous populations of Australia and New Zealand are considered at higher risk of mood and anxiety disorders but many studies do not include direct comparisons with similar non-Indigenous controls. We conducted a systematic search of relevant electronic databases, as well as snowballing and targeted searches of the grey literature. Studies were included for meta-analysis if they compared rates of mood and anxiety disorders between Indigenous and non-Indigenous Australians or Maori. Seven Australian and 10 NZ studies were included. Overall, Indigenous people in both countries did not hav

Myasthenia gravis with thymoma is more common in the Maori and Pacific Island populations in New Zealand.

BACKGROUND: The association of myasthenia gravis (MG) with thymoma is well recognized. Our clinical impression has been that MG associated with thymoma may be more common in patients of Polynesian descent than in other races. AIM: To determine the influence of ethnicity on the association of MG with thymoma in our population. METHOD: Review of all cases of thymectomy performed at Greenlane Hospital in Auckland for the 20-year period from June 1978 to June 1998. RESULTS: There were 103 thymectomies performed in the study period. Fifty-five thymomas were identified, 15 in subjects of Maori or
