The provision of analgesia in mass casualty incidents has traditionally been viewed as low-priority and reserved for later stages of care. Poor pain management is commonplace in trauma victims, and inadequate acute pain management can hinder evacuation efforts and may lead to the development of chronic pain and posttraumatic stress disorder. New, safe, and simple methods for administering quality analgesia have proven to be safe and effective in the prehospital setting and, as such, could easily be implemented into mass casualty incident protocols and allow for analgesia at earlier stages in s

OBJECTIVE: We hypothesized that resident education is inadequate with respect to management of mass casualty incidents that may involve chemical, biological, and nuclear exposures.
METHODS: Chief level residents in surgery (n = 10), emergency medicine (n = 10), and anesthesia (n = 8) were asked to complete a survey questionnaire. Responses were tabulated and statistically analyzed with Mann-Whitney Rank Sum, Student's t test, and Kruskal-Wallis one-way analysis of variance.
RESULTS: All of the residents were similar with respect to age, sex, and intended setting of clinical practice. Only a

The aims of this article are to describe the events of a recent mass casualty incident (MCI) at our level 1 trauma center and to describe the radiology response to the event. We also describe the findings and recommendations of our radiology department after-action review. An MCI activation was triggered after an amphibious military vehicle, repurposed for tourist activities, carrying 37 passengers, collided with a charter bus carrying 45 passengers on a busy highway bridge in Seattle, WA, USA. There were 4 deaths at the scene, and 51 patients were transferred to local hospitals following preh

Terrorist violence has emerged as an increasingly common cause of mass casualty incidents (MCI) due to the sequelae of explosive devices and shooting massacres. A proper emergency medical system disaster plan for dealing with an MCI is of paramount importance to salvage lives. Because the number of casualties following a MCI is likely to exceed the medical resources of the receiving health care facilities, patients must be appropriately sorted to establish treatment priorities. By necessity, clinical signs are likely to prove cornerstones of triage during MCI. An appropriate and effective appl

Bomb blast injuries are no longer confined to battlefields. With the ever present threat of terrorism, we should always be prepared for bomb blasts. Bomb blast injuries tend to affect air-containing organs more, as the blast wave tends to exert a shearing force on air-tissue interfaces. Commonly-injured organs include the tympanic membranes, the sinuses, the lungs and the bowel. Of these, blast lung injury is the most challenging to treat. The clinical picture is a mix of acute respiratory distress syndrome and air embolism, and the institution of positive pressure ventilation in the presence

Background: Mass casualty incidents lead to significant psychological distress and mental health complaints. In recent years, psychosocial and psychotherapeutic health care offers have increasingly been integrated into mass casualty protocols, yet it has become clear that emergency systems lack structured evaluation mechanisms, and research on exemplary healthcare models remains limited. This commentary discusses gaps in health service utilisation evaluation and assessment after mass casualty incidents. Methods: This commentary provides a narrative overview of conceptual and methodological eval

BACKGROUND: Mass casualty incidents with a focus on visceral surgery pose major medical and organizational challenges for hospitals. They are characterized by a large number of simultaneously admitted patients, limited personnel and material resources as well as the necessity to make time-critical surgical decisions. From a visceral surgical perspective, abdominal injuries involving severe bleeding, contamination and multiorgan involvement are particularly demanding as they require rapid surgical intervention and close interdisciplinary cooperation. This injury pattern is typically encountered

INTRODUCTION: To prepare military doctors to face mass casualty incidents (MCIs), the French Army Health Service contributed to the development of TRAUMASIMS, a serious game (SG) for training medical responders to MCIs.
METHODS: French military doctors participated in a three-phase training study. The initial war trauma training was a combination of didactic lectures (Phase 1), laboratory exercises (Phase 2), and situational training exercises (STX) (Phase 3). Phase 1 lectures reviewed French Forward Combat Casualty Care (FFCCC) practices based on the acronym MARCHE (Massive bleeding, Airway,

OBJECTIVES: Inappropriate distribution of casualties in mass-casualty incidents (MCIs) may overwhelm hospitals. This study aimed to review the consequences of evacuating casualties from a bus accident to a single peripheral hospital and lessons learned regarding policy of casualty evacuation.
METHODS: Medical records of all casualties relating to evacuation times, injury severity, diagnoses, treatments, resources utilized and outcomes were independently reviewed by two senior trauma surgeons. In addition, four senior trauma surgeons reviewed impact of treatment provided on patient outcomes. T

INTRODUCTION: Maritime mass casualty incidents (MCIs) present significant operational challenges due to environmental instability, geographic isolation, and delayed sea-to-shore evacuation. Despite increasing maritime disaster risks, limited evidence exists on how emergency personnel experience and enact coordinated responses and operational resilience in island settings.
METHODS: A qualitative descriptive study was conducted using individual semi-structured interviews with 12 emergency physicians and emergency nurses involved in maritime MCIs coordination in southern Thailand. Data were anal

The effective medical management of a suspected acute radiation overexposure incident necessitates recording dynamic medical data, measuring appropriate radiation bioassays, and estimating dose from dosimeters and radioactivity assessments in order to provide diagnostic information to the treating physician and a dose assessment for personnel radiation protection records. The accepted generic multiparameter and early-response approach includes measuring radioactivity and monitoring the exposed individual; observing and recording prodromal signs/symptoms and erythema; obtaining complete blood c

BACKGROUND: Effective management of burn mass casualty incidents (BMCIs) requires coordinated systems that include early stabilization, definitive burn care, surge capacity, and rehabilitation, supported by adequate infrastructure and workforce capacity. Although different countries have national guidelines, the full synthesis of the components of the health system needed by BMCIs response remains limited. This study aimed to systematically identify and classify the key components of the response to BMCIs.
METHOD: A systematic literature review was conducted to identify and synthesize the key

BACKGROUND: Hospitalized patients are inherently one of the most vulnerable populations. When the hospital is damaged, this population becomes even more at risk. Violence and natural disasters always have the potential to damage hospitals and other healthcare facilities, creating additional strain on an already catastrophic event. This study presents the preparedness of hospitalized patients during a mass casualty incident in order to understand their perspectives during a time of war in order to prepare, optimize, and to guide policy in order to maintain the highest level of care, efficiency,

INTRODUCTION: Mildly injured and "worried well" patients can have profound effects on the management of a mass-casualty incident. The objective of this study is to describe the characteristics and lessons learned from an event that occurred on 28 August 2005 near the central bus station in Beer-Sheva, Israel. The unique profile of injuries allows for the examination of the medical and operational aspects of the management of mild casualties.
METHODS: Data were collected during and after the event, using patient records and formal debriefings. They were processed focusing on the characteristic

The management of traumatic brain injury (TBI) in mass casualty incidents (MCI) is characterized by strong time dependence, complex injury patterns, and acute strain of medical resources, rendering it substantially more challenging than routine clinical practice. Currently, most existing domestic and international TBI guidelines and consensus statements are developed for daily medical scenarios, and lack systematic full-process standard tailored to the resource-constrained MCI environment. To fill this gap, the Chinese Neurosurgical Society convened a multidisciplinary expert panel covering ne

INTRODUCTION: Mass casualty incidents (MCIs) involving extensive ballistic and explosive injuries place considerable pressure on healthcare resources. This study aimed to evaluate the rehabilitation resources required for individuals who sustained blast and ballistic injuries during an MCI.
METHODS: A retrospective review was conducted using Electronic Medical Records (EMRs) of patients admitted to Sheba Medical Center (SMC), Israel, following an MCI on 7 October 2023. Patients diagnosed with gunshot wounds (GSW), or blast injuries (BI) were included. Data were collected over an almost 7-mont

BACKGROUND: Mass casualty incidents (MCIs) are events where the number of patients may exceed the resources available for emergency management. In emergency departments (ED), MCIs go along with high patient allocation in a short time period. For ED staff, simulation-based immersive scenario training can be an effective tool to develop communicative and leadership competencies to manage these situations.
APPROACH: A novel simulation-based immersion experience was developed for senior residents in emergency care. To our knowledge, this is the first scenario training focusing on in-hospital tria

Dicentric chromosome assay (DCA) is routinely used for estimating the absorbed radiation dose in exposed humans. Optimal lymphocyte viability is crucial for reliable dose estimation and most cytogenetic laboratories prefer the receipt of blood samples within 24 to 36 hours after collection. Delays in the shipment/receipt of samples can occur sometimes under certain unforeseen circumstances: (1) Adverse weather conditions, (2) distant location of blood collection sites, and (3) shipping and handling of a large number of samples after radiological/nuclear mass casualty incident(s). To circumvent
