Mast cell stabilizers.

The precise mode of action of the well-studied cromolyn sodium and the newer nedocromil sodium has not been completely elucidated. Because the drugs do not pass the cell membrane and enter the cell, they are virtually not metabolized, do not exert a systemic action, and therefore are associated with only minimal systemic toxicity. To be effective either drug must be applied topically and directly to the nasal mucosa. Proper contact with the nasal mucosa is essential for efficacy; in patients with nasal congestion and secretions, vasoconstrictors or saline lavages are indicated before cromolyn

Twenty-first century mast cell stabilizers.

Mast cell stabilizing drugs inhibit the release of allergic mediators from mast cells and are used clinically to prevent allergic reactions to common allergens. Despite the relative success of the most commonly prescribed mast cell stabilizer, disodium cromoglycate, in use for the preventative treatment of bronchial asthma, allergic conjunctivitis and vernal keratoconjunctivitis, there still remains an urgent need to design new substances that are less expensive and require less frequent dosing schedules. In this regard, recent developments towards the discovery of the next generation of mast

Ocular allergy and mast cell stabilizers.

The role of the mast cell in ocular allergy is becoming understood. As a result, the therapeutic effects of agents that stabilize the mast cell have been evaluated in the treatment of seasonal allergic conjunctivitis, vernal keratoconjunctivitis, atopic keratoconjunctivitis, and giant papillary conjunctivitis. At present, cromolyn sodium is the only available mast cell stabilizer of known effectiveness. Clinical and laboratory investigations of the effectiveness of cromolyn sodium in the treatment of ocular allergy are reviewed in the present article.

Synthesis of dehydroabietic acid derivatives as mast cell stabilizers.

Mast cell degranulation plays a central role in IgE-driven allergic diseases, yet the therapeutic options for stabilizers remain limited. Rosinane diterpenoids, a class of natural polyphenols known for their anti-inflammatory properties, represent an attractive source for novel anti-allergic compounds. Dehydroabietic acid (DHAA), a member of this class, exhibits diverse biological activities, yet its potential in mast cell regulation remains unexplored. In this study, we designed and synthesized a series of novel DHAA derivatives. We evaluated their anti-allergic effects in IgE/antigen-stimula

Topical antihistamines, mast cell stabilizers, and dual-action agents in ocular allergy: current trends.

PURPOSE OF REVIEW: To address the current trends of therapeutic mechanisms for treatment of allergic conjunctivitis (AC), based on topical antihistamines and mast cell stabilizers (MCS). RECENT FINDINGS: The antihistamine drug alcaftadine has H4 receptor inverse agonism, anti-inflammatory and MCS activities. The antihistamines levocabastine and azelastine are more effective than placebo in treatment of AC symptoms in randomized controlled trials (RCTs). The topical dual-action antihistamines/MCS olopatadine, azelastine, ketotifen, and epinastine are commonly used in Europe and in the United S

Mast Cell Stabilizers in the Treatment of Rosacea: A Review of Existing and Emerging Therapies.

Rosacea is a chronic inflammatory skin disease characterized by centrofacial erythema, papules, pustules, and telangiectasias. The onset of rosacea typically occurs after 30 years of age. It is estimated that approximately 2-5% of adults worldwide are affected. While the exact etiology of rosacea remains unknown, its pathogenesis is thought to be multifactorial with both environmental and genetic factors implicated. Ultraviolet radiation, heat, steam, ingested agents, including spicy foods and alcohol, host vasculature, dermal matrix degeneration, genetic susceptibility, and microbial organism

Anti-vasospastic mast cell stabilizers: a novel therapeutic approach to anaphylaxis-induced acute coronary syndrome.

Kounis syndrome is an acute myocardial infarction caused by severe allergic reactions or anaphylaxis following vaccination and other triggers, with the associated release of chemical mediators from mast cells causing coronary artery vasospasm. However, treatment with adrenaline is controversial as it paradoxically aggravates cardiac ischemia. Among the many a1-adrenergic receptor blockers, calcium channel blockers, endogenous vasodilators, and antioxidants that can ameliorate coronary artery vasospasm are some (e.g., prazosin, verapamil, diltiazem, magnesium and vitamin C) that can also stabil

Adrenaline and Salbutamol Rapidly Exert Mast Cell-Stabilizing Properties in Rat Peritoneal Mast Cells.

BACKGROUND/AIMS: Adrenaline is the first-choice drug for the treatment of anaphylaxis. Previous studies have revealed its prophylactic effects on the release of chemical mediators from mast cells. However, adrenaline quickly reverses the symptoms and signs of anaphylaxis after injection, indicating its rapid effects on mast cells. METHODS: We examined the effects of adrenaline on rat peritoneal mast cell degranulation using differential-interference contrast (DIC) microscopy. To examine the rapid effects of adrenaline and salbutamol, a selective β2-adrenergic receptor agonist, we treated mast

Inhibitors of chymase as mast cell-stabilizing agents: contribution of chymase in the activation of human mast cells.

There has long been evidence that inhibitors of chymotryptic proteinases can inhibit the degranulation of rodent mast cells, but their actions on human mast cells and the contribution of mast cell chymase itself have received little attention. We investigated the ability of the selective chymase inhibitor Z-Ile-Glu-Pro-Phe-CO(2)Me and other proteinase inhibitors to inhibit chymase and cathepsin G activity, and we examined their potential to modulate the responsiveness of mast cells dispersed from human skin, lung, and tonsil tissues. IgE-dependent histamine release from skin mast cells was inh

Prazosin Potentiates Mast Cell-Stabilizing Property of Adrenaline.

BACKGROUND/AIMS: Adrenaline quickly inhibits the release of histamine from mast cells. Besides β2-adrenergic receptors, several in vitro studies also indicate the involvement of α-adrenergic receptors in the process of exocytosis. Since exocytosis in mast cells can be detected electrophysiologically by the changes in the membrane capacitance (Cm), its continuous monitoring in the presence of drugs would determine their mast cell-stabilizing properties. METHODS: Employing the whole-cell patch-clamp technique in rat peritoneal mast cells, we examined the effects of adrenaline on the degranulati

Progress of Research and Comparative Analysis on the Combination of Mast Cell Stabilizers and Antihistamines in Allergic Conjunctivitis Treatment.

Allergic conjunctivitis (AC) is a common ophthalmological condition primarily associated with type I and IV allergic responses. Although AC typically does not cause visual impairments, it can significantly reduce the quality of life of individuals, regardless of age, with severe cases possibly affecting the cornea and potentially impairing vision. However, the mechanisms of action of primary therapeutic drugs-antihistamines, mast cell stabilizers, and their combination-remain unclear. This review introduces the pathogenesis of allergic conjunctivitis, including seasonal, perennial, and vernal

Clarithromycin Dose-Dependently Stabilizes Rat Peritoneal Mast Cells.

BACKGROUND: Macrolides, such as clarithromycin, have antiallergic properties. Since exocytosis in mast cells is detected electrophysiologically via changes in membrane capacitance (Cm), the absence of such changes due to the drug indicates its mast cell-stabilizing effect. METHODS: Employing the whole-cell patch clamp technique in rat peritoneal mast cells, we examined the effects of clarithromycin on Cm during exocytosis. Using a water-soluble fluorescent dye, we also examined its effect on deformation of the plasma membrane. RESULTS: Clarithromycin (10 and 100 μM) significantly inhibited d

Mast Cell Stabilizer Ketotifen Inhibits Gouty Inflammation in Rats.

Gout, an extremely painful arthritis with relapsing inflammatory attacks, is a common inflammatory joint disease in adults. We examined the therapeutic effect of ketotifen, a mast cell stabilizer, on monosodium urate (MSU) crystal-induced acute inflammation. Eight-week-old male Wistar rats were injected with MSU crystals (5 mg per rat) into air pouch. Ketotifen (0, 0.1, 03, and 1 mg/kg) was given 1 hour before MSU crystal injection. Lavage histamine, leukocyte counts, mast cell counts, nitric oxide, and proinflammatory mediator levels were assessed 12 hours after MSU injection. Ketotifen signi

Misoprostol prevents damage to the gastric mucosa by stabilizing the mast cells.

Prostaglandins have been shown to prevent the damage to the gastric and intestinal mucosa which has been induced by diverse necrotizing substances. These damaging stimuli increase the liberation of histamine from mast cells. Because of its well known effects on cellular permeability, histamine may serve the initial stimulus for mediating cellular damage. The aim of our study was to investigate the effect of misoprostol, a synthetic PGE1 analog, on tissue histamine concentration and mast cell counts after damage to the gastric mucosa induced by stress, histamine, aspirin and concentrated ethano

The role of mast cell stabilization in treatment of postoperative ileus: a pilot study.

OBJECTIVES: Although postoperative ileus (POI) is considered multifactorial, intestinal inflammation resulting from manipulation-induced mast cell activation is recognized as an important pathophysiological mechanism. Therefore, mast cell stabilization may represent a new therapeutic approach to shortening POI. The aim of this paper was to study the effect of ketotifen, a mast cell stabilizer, on postoperative gastrointestinal transit in patients who underwent abdominal surgery. METHODS: In this pilot study, 60 patients undergoing major abdominal surgery for gynecological malignancy with stan

Gastroprotective and ulcer healing profile of the mast cell stabilizer quazolast in rats.

Quazolast, a mast cell stabilizer, was evaluated for efficacy against acid independent (alcohol, HCl), or dependent (aspirin, indomethacin) gastric damage in rats. Its gastroprotective profile was compared to that of ranitidine. In addition, the antisecretory and gastric ulcer (acetic acid induced) healing capabilities of these agents were examined. Quazolast, in direct contrast to ranitidine, protected the rat gastric mucosa from acid-independent, but not acid-dependent gastric damage. Quazolast lacked antisecretory activity in rats; however, it did heal acetic acid induced gastric ulcers in
