Mastoid emissary vein, mastoid emissary foramen, and mastoid emissary canal: anatomy, variability, imaging, and clinical implications.

PURPOSE: The mastoid emissary vein (MEV), mastoid emissary canal (MEC), and mastoid emissary foramen (MEF) are common but variably reported structures of the mastoid-posterior fossa region. Existing literature is fragmented across osteological, imaging-based, and clinical studies, with inconsistent prevalence and morphometric estimates, and with limited integration of anatomical variation into surgical and radiological decision-making. This SANRA-guided narrative review aimed to synthesise the anatomy, variability, imaging assessment, and clinical implications of MEV/MEC/MEF. METHODS: This SA

Mastoiditis and brain hernia (mastoiditis cerebri).

Ten patients with brain hernia occurring as an insidious complication of chronic mastoiditis coincidentally discovered at the time of surgery are described. Four had previous surgery, six did not. A history of trauma was othwise absent. Cerebrospinal fluid otorrhea did not occur. Our method of management is described. Pathology and pathogenesis are discussed. Three patients had pedunculated brain hernias, while seven had diffuse (fungoid) herniation. Granulation tissue appeared as an important part of the disease process. These may represent abortive attempts at brain abscess formation since t

Congenital Mastoid Cholesteatoma.

BACKGROUND: Congenital cholesteatomas account for just up to 5% of all cholesteatomas and most commonly arise in the petrous apex and middle ear. Congenital cholesteatomas arising in the mastoid are rare and typically present late. METHODS: In this study, we report a case series of 3 cases managed in our department between 2006 and 2021 and present a summary of the current literature. RESULTS: Congenital cholesteatomas arising in the mastoid is a rare finding and even among reported cases, not all are clearly mastoid in origin. Their location allows for considerable growth before symptoms de

Primary mastoid cyst.

OBJECTIVE: To report an unusual case of a primary mastoid cyst (congenital or developmental) in a patient without otological symptoms. METHOD: Case report and review of the English language literature. RESULTS: Primary mastoid cyst is a newly reported and very rare pathological entity. Mastoid cysts usually occur secondary to chronic infection, inflammation or trauma. Review of the medical literature highlights the rarity of this condition. CONCLUSION: This report describes the experience gained by the diagnosis and management of this patient. It emphasises the importance of clinical vigila

Mastoid subperiosteal abscess.

Subperiosteal abscess was the indication for surgery in every child with acute mastoiditis who underwent mastoidectomy at our institution since 1972. Nineteen such patients are discussed herein. Positive bacteriologic findings included Streptococcus pyogenes in three patients, Streptococcus pneumoniae in three patients, Mycobacterium tuberculosis in two patients, and enterococcus in the one patient with cholesteatoma. Cultures were more often positive for causative bacteria when taken directly from the abscess before or shortly after beginning antibiotic therapy.

Mastoiditis.

Mastoiditis still occurs as a nonmeningeal complication of otitis media. Fever, postauricular swelling, tenderness of the ear pinna, severe otalgia and ear drainage are all typical clinical findings. Most commonly, the causative organisms are the same aerobic bacteria that are responsible for otitis media; however, anaerobic organisms must also be considered. Treatment for acute mastoiditis includes appropriate intravenous antibiotics and myringotomy for culture and drainage. Mastoidectomy is indicated if medical therapy fails.

Mastoid Dermoid Cyst.

A case of mastoid dermoid cyst (DC) was presented, and differences with cases of other temporal bone DCs were analyzed. The mastoid DC was also compared with mastoid congenital cholesteatoma. We reported a case of a patient with mastoid DC, evaluating her clinical, radiological, and surgical findings. A review of the literature was performed to compare our findings with those reported. The preoperative radiological evaluation prompted us to plan a surgical approach to the lesion, suspecting the presence of a mastoid congenital cholesteatoma. The surgical findings were in line with the presence

Postauricular cutaneous mastoid fistula.

Postauricular cutaneous mastoid fistula is a rare condition. The cutaneous mastoid fistula is a very rare complication of chronic suppurative otitis media. The fistula tracts are typically difficult to manage because of the surrounding necrotic skin edges. We describe an unusual case of a postauricular cutaneous mastoid fistula and outline the surgical technique used for closure.

Mastoid surgery in childhood.

The presenting features and operative findings in 105 patients aged 16 years or less undergoing mastoid surgery are reviewed. In 94% surgery was for chronic otomastoiditis, usually acquired, and associated with cholesteatoma in 64%. Post-operative otorrhoea persisted beyond 6 months in 44% and contralateral disease required some form of surgical procedure in 20%. Post-operative hearing thresholds were improved or unchanged in the majority of patients, most of whom underwent cortical or modified radical mastoidectomy. An improvement in the therapeutic results of mastoid surgery in childhood is

Solitary plasmacytoma of the mastoid bone presenting as coalescent mastoiditis.

Acute mastoiditis is a complication of acute otitis media that produces air cell opacification and coalescence on computed tomographic (CT) imaging. This appearance, however, is nonspecific and may be seen in patients with infection and tumor. Magnetic resonance imaging (MRI) can aid in differentiating the two on the basis of differences in tissue signal characteristics and by showing the presence or absence of an associated soft-tissue mass. In this report, we demonstrate the utility of MRI as a complementary imaging modality to CT in the diagnosis of a solitary plasmacytoma of the temporal b

Mastoid cortex osteoma associated with middle-ear and mastoid cholesteatoma: a case report.

Osteomas of the temporal bone are rare benign tumors that, in a small minority of cases, may give rise to secondary cholesteatoma. We report a case in which two independent pathological mechanisms appeared to coexist, resulting in a configuration not previously described in the literature. A 51-year-old woman presented with one year of right-sided otorrhoea and progressive mixed hearing loss unresponsive to antibiotic therapy. High-resolution computed tomography (HRCT) demonstrated a giant sessile osteoma arising from the anterior mastoid cortex with complete external auditory canal (EAC) obst

Tuberculoma of the mastoid.

Tuberculoma is a rare but known complication of tuberculosis. It rarely occurs in the mastoid bone. When it occurs in the middle ear cleft, it can lead to intracranial complications if there is a delay in the diagnosis and management. A rare case of tuberculosis of the middle ear cleft which presented as tuberculoma of the mastoid bone with infranuclear facial palsy is described. The tuberculoma was removed and a canal wall down tympanomastoidectomy was performed. Post-operatively the patient was kept on antituberculous therapy. The various problems encountered in the diagnosis and management

Three 'silent' mastoid abscesses.

Acute mastoiditis with abscess formation is uncommon, its incidence being modified by modern antibiotic usage for acute suppurative otitis media (ASOM). It tends to occur in young children who have no previous history of ear disease. The diagnosis is a clinical one, and treatment requires hospital admission and some form of surgical drainage procedure, in order to prevent intracranial complications, eg, meningitis and intracerebral abscess.

Mastoid pneumatization in otosclerosis.

The extent of mastoid pneumatization in 150 otosclerotic ears was compared with that of 150 healthy control ears. The size of mastoid pneumatization was measured by use of the Schüller lateral x-ray projection with the help of computed planimetry. The measurements showed the average pneumatized area in otosclerotic ears to be 17.4 +/- 5 cm2, in contrast to 12.9 +/- 4 cm2 for the healthy control ears. The difference between the two groups was highly significant (p less than .0001). While both groups showed a bell-shaped distribution of the measured pneumatized area, the curve of the otoscleroti

Mastoiditis Masquerade.

We report a case of primary temporal bone diffuse B-cell non-Hodgkin lymphoma, which is a rare entity. A 71-year-old male with a history of dementia and hemicraniectomy presented due to 1 month of a pronounced left ulcerative mastoid lesion. Strikingly, there were no cranial nerve deficits which was unexpected due to the degree of the lesion. Initially, infectious mastoiditis was suspected based on physical examination alone. Due to the patient being a poor historian, it was difficult to determine whether this was an acute or chronic issue. Temporal bone squamous cell carcinoma, infectious mas
