Revision mastoidectomy.

The first three priorities in surgery for chronic otitis media are (1) the elimination of progressive disease to produce a safe and dry ear, (2) modification of the anatomy of the tympanomastoid compartment to prevent recurrent disease, and (3) reconstruction of the hearing mechanism. The indications for revision following mastoidectomy for chronic otitis media thus involve failure to achieve any of these goals, including recurrent cholesteatoma, recurrent suppuration, recurrent perforation, or recurrent or residual conductive hearing loss. The focus of this article is the management of recurr

Mastoidectomy update.

The long-term results of 375 primary mastoidectomy operations for chronic otitis media and chronic mastoiditis are described and discussed. Two-thirds of these cases were open cavity mastoidectomies and 1/3 closed cavity mastoidectomies. Primary pathology was cholesteatoma in 1/3 and granulation tissue in 2/3 of the cases. The method of reducing the size of the mastoid cavity by sculpturing mastoid cavity by sculpturing mastoid cortical bone is most important in avoiding large postoperative cavity problems. Principles and methods are described. Healing (dry ear) is a more important management

The behavior of mastoidectomy cavities following modified radical mastoidectomy.

OBJECTIVES/HYPOTHESIS: An analysis of the frequency and intensity of postoperative aftercare required for modified radical mastoidectomy (MRM) and patterns of healing in the postoperative period. STUDY DESIGN: A retrospective review of all primary modified radical mastoidectomies carried out for cholesteatoma under the care of the senior author between the years of 2004 and 2009 with minimum follow-up of 2 years. METHODS: The time and number of interventions required to achieve a stable and dry mastoid cavity were collected. Cross-sectional and longitudinal analysis of the behavior of the ca

Mastoidectomy in the elderly.

OBJECTIVE: To present conditions requiring mastoid surgery in elderly patients. STUDY DESIGN: Retrospective. RESULTS: Twenty-seven patients (aged 65-90 years) who underwent mastoidectomy were enrolled. The etiologies included chronic otitis media (COM) with (n = 16) or without (n = 5) cholesteatoma, otogenic meningitis and malignant otitis externa (MOE; n = 3 each). Urgent mastoidectomy was indicated for 4 intracranial complications (3 meningitis and 1 brain abscess) and 14 extracranial complications of acute/COM or MOE (7 patients with labyrinthitis and 7 patients with facial nerve, FN, par

Modified-modified radical mastoidectomy.

PURPOSE: It is unusual to have communication from the external auditory canal (EAC) directly to the mastoid, totally sparing the tympanum. These patients need a different surgical approach, a modified canal wall-down procedure, to completely clear the disease but fully preserve the tympanum. We present one such exceptional case. CASE PRESENTATION: A 28-year-old lady presented with ear discharge for 1 year. Imaging confirmed the canal-mastoid fistula, but the entire tympanum was normal. We performed a modified-modified radical mastoidectomy. CONCLUSIONS: Canal-mastoid fistula is an infrequent

Intracranial complications following mastoidectomy.

Mastoidectomy is a common surgical procedure in otology. However, postoperative complications of various degrees of severity may occur. We present 4 children who underwent mastoidectomy for middle ear and mastoid disease and developed postoperative intracranial complications. One child was operated on for brain abscess 1 week after the initial mastoidectomy. Another child appeared with seizures 5 days after the initial mastoidectomy and a subdural empyema was drained during revision surgery. Large bone defects with exposed middle cranial fossa dura were found at revision surgery in both cases

[Application of the collagen in mastoidectomy].

OBJECTIVE: To evaluate the effect of the collage application in mastoidectomy. METHOD: Forty-one patients (41 ears) with chronic suppurative otitis media (cholesteatoma type) were surgically treated by mastoidectomy, in which 22 cases (22 ears) used collagen and dissociative flap to deal with mastoid cavity but 19 cases (19 ears) without collagen. All patients were followed up for one year to observe the situation after mastoidectomy comparatively. RESULT: The epithelization time of mastoid cavity was 3 to 7 weeks, averaging (4.5 +/- 0.7) weeks in the collagen-applying group and was 5 to 12

[Mastoidectomy and seromucous otitis].

Twenty-nine children presenting with chronically recurring serous otitis media, or with serous otitis media complicated by retracted eardrum, underwent either unilateral or bilateral mastoidectomy. For 23 of the children (26 surgical cases) whose follow-up extended over a sufficiently long period of time, results were assessed by otoscopic eardrum inspection, audiometry, and tympanometry. Long-standing recovery of serous otitis media, accompanied by a return to normal of the tympanic membrane, the tympanometric and audiometric parameters, was achieved in 2/3 of cases. These data compare well w

Pulsatile tinnitus cured by mastoidectomy.

Tinnitus of venous origin is a rare occurrence. It represents roughly half of cases of vascular tinnitus. The choice of treatment is not easy, even when the diagnosis is certain. Reassurance of the patient is often sufficient. Nevertheless, a surgical treatment is sometimes performed--usually ligature of the internal jugular vein. We present the case of a patient with a right venous pulsatile tinnitus and a history of 5 years of ineffective medical and surgical treatments. Ligature of the internal jugular vein was not chosen in this case: a computed tomographic scan showed filled mastoid cells

Canal wall down mastoidectomy with obliteration versus canal wall up mastoidectomy in primary cholesteatoma surgery.

OBJECTIVE: This study sought to compare disease recidivism rates between canal wall up mastoidectomy and a canal wall down with obliteration technique. METHODS: Patients undergoing primary cholesteatoma surgery at our institution over a five-year period (2013-2017) using the aforementioned techniques were eligible for inclusion in the study. Rates of discharge and disease recidivism were analysed using chi-square statistics. RESULTS: A total of 104 ears (98 patients) were included. The mean follow-up period was 30 months (range, 12-52 months). A canal wall down with mastoid obliteration tech

Usefulness of cortical mastoidectomy in myringoplasty.

OBJECTIVES: The usefulness of cortical mastoidectomy in myringoplasty remains an issue of controversy. We aimed to assess the effectiveness of mastoidectomy on outcomes of tympanoplasty performed in patients with persistent or intermittent discharging chronic suppurative otitis media without cholesteatoma. STUDY DESIGN: Prospective, randomized, single-blinded study. Level of evidence-1B. SETTING: Tertiary referral center. PATIENTS: Three hundred twenty consecutive adult patients presenting persistent or intermittent otorrhea during the preceding 6 months scheduled for myringoplasty were inc

Consent for mastoidectomy: a patient's perspective.

OBJECTIVES: The purpose of this study is to identify the difference between patients perspectives regarding the given information during the consenting procedure of mastoidectomy and the type of the provided information by the medical practitioners. This subject is becoming increasingly important because of the increase in the medico-legal litigation due to inadequate consenting process. METHODS: Nineteen patients who had undergone mastoid surgery were asked to identify which risks of mastoid surgery they felt were important to be informed of prior to surgery. This was compared to 20 ENT cons

Pneumocephalus as a delayed mastoidectomy complication.

We report the case of a 43-year-old woman who presented with a spontaneous pneumocephalus, 37 years after a mastoidectomy. Clinical examination showed a cerebrospinal fluid leak, meningeal herniation in the superior part of the middle ear, and an audible noise from her ear when she stood up due to the entrance of air into the cranium. A computed tomography scan and magnetic resonance imaging showed the complete destruction of the tegmen tympani and the pneumocephalus in the temporal lobe. The patient underwent an emergency operation via a double middle-ear and subtemporal approach. The meningo

[Follow-up of mastoiditis and mastoidectomy].

BACKGROUND: To analyse the late results of mastoiditis and operative intervention as well as critical classification of our own indication for operation we invited 298 operated patients to follow-up examinations. PATIENTS AND METHODS: 218 of 298 patients operated on, took part in these examinations. The follow-up time ranged between 2 and 18 years. RESULTS: Altogether 311 ears were operated. Out of these, 69 patients had presented with classical mastoiditis and 242 with "masked" type. Microscopic examination of the operated ears showed that 284 (91.3%) had normal tympanic membranes, 13 (4.2%

Vestibular nerve section following previous mastoidectomy.

Vestibular neurectomy is an effective procedure in the management of vertigo due to active labyrinthine disease. Various approaches have been developed for selectively sectioning the vestibular nerve, in order to preserve serviceable hearing and avoid facial nerve injury. In patients who have a mastoid cavity, from previous surgery for chronic otitis media, the approach to the vestibular nerve has to be modified. Considerations taken into account are cavity infection, hearing status, and the presence of associated loud tinnitus. Vestibular or vestibulo-cochlear nerve section has been undertake

Epithelial migration in open mastoidectomy cavities.

Twenty patients who had undergone open cavity mastoidectomy for chronic middle ear disease were selected for study. Following initial microscopic aural toilet, each cavity was painted in its medial aspect with a marker solution of Bonney's Blue dye. Each patient was assessed at weekly intervals for a minimum of three months following application of dye, and a photographic record made of the cavity. Information was also sought regarding the clinical behaviour of the mastoid cavities, with particular regard to degree of otorrhoea, and accumulation of debris. Migration of epithelium inferred by v
