Cesarean section surgical site infections in sub-Saharan Africa: a multi-country study from Medecins Sans Frontieres.

BACKGROUND: Surgical site infections (SSI) are a significant cause of post-surgical morbidity and mortality and can be an indicator of surgical quality. The objectives of this study were to measure post-operative SSI after cesarean section (CS) at four sites in three sub-Saharan African countries and to describe the associated risk factors in order to improved quality of care in low and middle income surgical programs. METHODS: This study included data from four emergency obstetric programs supported by Medecins sans Frontieres, from Burundi, the Democratic Republic of Congo (DRC), and Sierra

Cesarean section rates and indications in sub-Saharan Africa: a multi-country study from Medecins sans Frontieres.

OBJECTIVES: The World Health Organization considers Cesarean section rates of 5-15% to be the optimal range for targeted provision of this life saving intervention. However, access to safe Cesarean section in resource-limited settings is much lower, estimated at 1-2% reported in sub-Saharan Africa. This study reports Cesarean sections rates and indications in Democratic Republic of Congo, Burundi, and Sierra Leone, and describe the main parameters associated with maternal and early neonatal mortality. METHODS: Women undergoing Cesarean section from August 1 2010 to January 31 2011 were includ

The Medecins Sans Frontieres intervention in the Marburg hemorrhagic fever epidemic, Uige, Angola, 2005. I. Lessons learned in the hospital.

When the epidemic of Marburg hemorrhagic fever occurred in Uige, Angola, during 2005, the international response included systems of case detection and isolation, community education, the burial of the dead, and disinfection. However, despite large investments of staff and money by the organizations involved, only a fraction of the reported number of cases were isolated, and many cases were detected only after death. This article describes the response of Medecins Sans Frontieres Spain within the provincial hospital in Uige, as well as the lessons they learned during the epidemic. Diagnosis, m

The Medecins Sans Frontieres intervention in the Marburg hemorrhagic fever epidemic, Uige, Angola, 2005. II. lessons learned in the community.

From 27 March 2005 onwards, the independent humanitarian medical aid agency Medecins Sans Frontieres, together with the World Health Organization, the Angolan Ministry of Health, and others, responded to the Marburg hemorrhagic fever (MHF) outbreak in Uige, Angola, to contain the epidemic and care for those infected. This response included community epidemiological surveillance, clinical assessment and isolation of patients with MHF, safe burials and disinfection, home-based risk reduction, peripheral health facility support, psychosocial support, and information and education campaigns. Catalyst for change: Lessons learned from overcoming barriers to providing safe abortion care in Médecins Sans Frontières projects.

CONTEXT: Despite instituting a policy in 2004, Médecins Sans Frontières (MSF) continuously struggled to routinely provide safe abortion care (SAC). In 2016, the organization launched an initiative aimed at increasing availability of SAC in MSF projects and increasing understanding of abortion-related dynamics in humanitarian settings. METHODOLOGY: From March 2017 to April 2018, MSF staff conducted support visits to 10 projects in a country in sub-Saharan Africa. Each visit followed a systematic approach with six key components and related tools that were later shared with teams worldwide. Dat

Tailored intervention to prevent relapses and hospitalization in a migrant suffering from paranoid schizophrenia.

Adherence issues combined with inequitable access to healthcare may increase the risk of discontinuation of care for undocumented migrants with severe mental health illness. An Ethiopian man with paranoid schizophrenia who relapsed several times after hospitalization was identified by a humanitarian outreach team in Brussels. The team built a relationship with him by offering him access to services including accommodation and mental health care. A treatment buddy was identified to support him adhering to his treatment and accompany him while hospitalized. Effective collaboration between Medeci

[The concept of neglected disease].

"Neglected disease", "neglected population" and more generally "public health negligence" are emerging concepts being put forward by numerous humanitarian groups. Sleeping sickness provides a typical example to illustrate these concepts. After causing a major epidemic in the 1930s, sleeping sickness had been practically eradicated by the end of decolonization. Because of more urgent priorities, independent national governments relinquished control activities thus allowing the disease to return insidiously. By the beginning of the 1990s the situation was comparable to that prevailing in 1930 wi

Mental health of older adults in humanitarian settings in low- and middle-income countries: a retrospective analysis from Médecins sans Frontières-supported mental health services, 2019-2024.

BACKGROUND: More complex humanitarian emergencies have a profound impact on a rapidly growing ageing population. There are few data available on the mental health conditions of older people in humanitarian settings. This study describes mental health symptoms among older adults to inform the development of age-adapted and gender-adapted mental health services in humanitarian contexts. METHODS: This multicountry study includes data from adults aged 50 years or older that accessed Médecins sans Frontières-supported mental health services in humanitarian settings across 20 countries between July

Health conditions of older adults in complex humanitarian settings in low- and middle-income countries: a retrospective analysis of 2019-2025 data from Médecins Sans Frontières-supported inpatient departments.

BACKGROUND: Inpatient admissions of older adults in humanitarian settings in low-income and middle-income countries remain poorly documented, likely leading to gaps in the delivery of age-appropriate health services. This analysis aims to contribute to age-adapted and gender-adapted healthcare strategies in humanitarian settings. METHODS: This multicountry study includes adults who were admitted at Médecins Sans Frontières-supported inpatient departments in humanitarian settings across four regions between July 2019 and April 2025. Obstetric fistula in Burundi: a comprehensive approach to managing women with this neglected disease.

BACKGROUND: In Burundi, the annual incidence of obstetric fistula is estimated to be 0.2-0.5% of all deliveries, with 1000-2000 new cases per year. Despite this relatively high incidence, national capacity for identifying and managing obstetric fistula is very limited. Thus, in July 2010, Medecins Sans Frontieres (MSF) set up a specialised Obstetric Fistula Centre in Gitega (Gitega Fistula Centre, GFC), the only permanent referral centre for obstetric fistula in Burundi. A comprehensive model of care is offered including psychosocial support, conservative and surgical management, post-operativ

A Public health wound: health and work among children engaged in the worst forms of child labour in the informal sector in Dhaka, Bangladesh: a retrospective analysis of Médecins Sans Frontières occupational health data from 2014 to 2023.

BACKGROUND: Bangladesh has the second highest burden of child labour in South Asia. The informal sector employs most of the children however, data on health including injuries and place of work for children are limited. As the deadline for the Sustainable Development Goals to end child labour is upon us, it is paramount to document the impact of child labour on health. This study aims to contribute to this knowledge gap by presenting medical data from occupational health clinics (OHCs) set up by Médecins Sans Frontières (MSF) in a peri-urban area of Dhaka, Bangladesh. METHODS: We did a retros

Epidemiological characteristics and outcomes of the 2023-2024 cholera outbreak in South Yemen: a retrospective descriptive study from MSF-supported centres.

BACKGROUND: The cholera burden in Yemen is the highest in the world, with the country facing recurrent outbreaks. It is worsened by the ongoing conflict, limited access to clean water, and the deteriorated sanitation and health systems. In November 2023, a cholera outbreak started in Shabwah governorate and seemed to be controlled in January 2024. However, in March 2024, cholera spread westward, causing 40,000 suspected cases in South Yemen. METHODS: Routinely collected data were used to provide descriptive statistics of the 2023-2024 cholera outbreak of patients managed in the Médecins Sans

Incidence and characteristics of candidemia in hospitalised patients with advanced HIV in the Médecins Sans Frontières (MSF) hospital in Kinshasa, Democratic Republic of Congo (DRC).

Candidemia is an underrecognised complication among patients with advanced HIV disease (AHD), particularly in low‑resource settings. We conducted a retrospective study at an MSF-supported hospital in Kinshasa (2022-2025) to describe the incidence, characteristics, and outcomes of candidemia. Among 3974 AHD admissions, 46 candidemia cases (1.2%) were identified, more than 80% of which were healthcare‑associated. Mortality was high (76.1%) and many deaths occurred before microbiological confirmation. Classical risk factors, such as indwelling catheters and parenteral nutrition, were uncommon, su

Distribution of household disinfection kits during the 2014-2015 Ebola virus outbreak in Monrovia, Liberia: The MSF experience.

During the initial phase of the 2014-2016 Ebola virus disease (EVD) outbreak in Monrovia, Liberia, all hospitals' isolation capacities were overwhelmed by the sheer caseload. As a stop-gap measure to halt transmission, Medecins sans Frontieres (MSF) distributed household disinfection kits to those who were at high risk of EVD contamination. The kit contained chlorine and personal protective materials to be used for the care of a sick person or the handling of a dead body. This intervention was novel and controversial for MSF. This paper shed the light on this experience of distribution in Monr

Nifurtimox-eflornithine combination therapy for second-stage Trypanosoma brucei gambiense sleeping sickness: a randomized clinical trial in Congo.

BACKGROUND: Human African trypanosomiasis caused by Trypanosoma brucei gambiense is a fatal disease. Current treatment options for patients with second-stage disease are either highly toxic or impracticable in field conditions. We compared the efficacy and safety of the nifurtimox-eflornithine drug combination with the standard eflornithine regimen for the treatment of second-stage disease. METHODS: A randomized, open-label, active-control, phase III clinical trial comparing 2 arms was conducted at the Sleeping Sickness Treatment Center, which was run by Medecins Sans Frontieres, in Nkayi, Bo

Nutrition indicators as potential predictors of AIDS-defining illnesses among ARV-naïve HIV-positive adults in Kapiri Mposhi, Zambia 2008-2009.

Early changes in nutritional status may be predictive of subsequent HIV disease progression in people living with HIV (PLHIV). In addition to conventional anthropometric assessment using body mass index (BMI) and mid-upper arm circumferences (MUAC), measures of strength and fatigability may detect earlier changes in nutrition status which predict HIV disease progression. This study aims to examine the association between various nutritional metrics relevant in resource-scarce setting and HIV disease progression. The HIV disease progression outcome was defined as any occurrence of an incident A
