Mediastinitis is defined as acute or chronic inflammation of the mediastinal structures and generally has a low incidence. The most frequent acute cause is sternotomy following cardiac revascularization surgery with both internal mammary arteries, with an incidence of 0.4% to 5% and a mortality of 16.5% to 47%. The most frequent vector is Staphylococcus aureus. Esophageal perforation, usually iatrogenic, is the second most frequent cause of acute mediastinitis, produced by common oropharyngeal flora, with a mortality rate of 20% to 60%, depending on the time of diagnosis. The third most freque

We studied 13 patients with mediastinal abscesses caused by oropharyngeal infections, who presented between April 2007 and June 2011. All patients were operated on after maxillofacial and ear, nose and throat surgeons had treated the primary source and drained all collections in the neck. Thoracic surgery was performed in the same session. Anterior mediastinal collections were drained via a small mediastinotomy. Posterior collections were approached via a thoracotomy. Chest computed tomography was essential to delineate the extent of disease. A thoracotomy approach was used in 7 patients; 2 of

Basing on the analysis of 143 cases of odontogenic mediastinitis it is shown that in the structure of acute odontogenic infectious diseases this severe complication accounts for 0.3% of cases. In 80% of the cases it develops on the background of chronic somatic diseases representing 51.7% of cases in the age group from 31 to 50 years. Outcome of mediastinitis depends on time of hospitalization, intensity and time of the treatment. Life span and quality of life of the patients depends first of all upon somatic disease compensation.

Mediastinitis is not a new disease, however, the occurrence was rare especially in children. Focal infection, which considered to be insignificant, can result in diffuse dissemination involving mediastinum leading to fatal condition. This paper reported a case of mediastinitis with very critical condition. Fast and accurate diagnosis as well as adequate antibiotic treatment saved the patient without surgical intervention.

A 69-year-old man with lymphocytic leukemia presented with fever, a pericardial friction rub, widening of the mediastinum, and left pleural effusion. Atrial fibrillation, refractory hypotension and acute paraplegia punctuated his hospital course. Invasion of the mediastinum, myocardium, mediastinal, coronary and spinal arteries with mucormycosis was present at post-mortem examination.

Although unfrequent, chronic mediastinites are not exceptional. Their clinical polymorphism results from diversity of the mediastinal content. The superior vena cava syndrome is the most frequent clinical sign. The essential examination is the angiopneumography. Mediastinitis can be associated to other fibrinogenous processes, particularly the retroperitoneal ones. Infectious processes (tuberculosis, various bacteria), parasitic or medicinal (methysergide), are found in the etiology of this affection. Very often the etiological research is negative: recurring lymphotropic respiratory infection

The authors give an outline of the issue of acute mediastinitis. It is a severe infection of connective tissue of the mediastinum. Timing of recognition and accuracy of drainage are the most essential conditions of successful treatment. Acute mediastinitis may be divided in the postoperative sternal infections with infection of the mediastinum, mediastinitis due to perforation of the aero-digestive tract and descending necrotizing mediastinitis. The authors describe etiopathogenesis, diagnostics and therapy in each particular type of mediastinitis.

Fibrosing mediastinitis is a rare entity either idiopathic or caused by granulomatous disease. Plain film findings mainly show widening of the upper half of the anterior mediastinum, whereas CT delineates more clearly the exact location of the mediastinal mass and the extent of compromise of mediastinal structures. Although vessels surrounded by the fibrous mass are typically smoothly bordered and only rarely displaced, diagnosis can only be suspected together with the patients history and clinical course.

BACKGROUND AND OBJECTIVE: Acute mediastinitis, mediastinal granuloma (MG), and chronic fibrosing mediastinitis (FM) are distinct disease processes affecting the mediastinal structures. These diseases have variable etiologies, management strategies, and outcomes. The purpose of the manuscript is to present a narrative review of these distinct, albeit rare conditions affecting the mediastinum.
METHODS: We searched various databases such as PubMed and Google Scholar from the last 10 years for the various benign mediastinal disorders we wanted to discuss. Relevant articles that were written in En

Fibrosing mediastinitis is rare in settings where histoplasmosis is not endemic. An idiopathic form of the disease may present with indistinguishable features and requires methodical exclusion of competing differential diagnoses. We report the case of a 30-year old female patient who presented with intermittent haemoptysis for the past 2 years with no constitutional symptoms. Computed tomography of the chest revealed a prominent right bronchial arterial circulation with a mass-like lesion, which encased and attenuated the right pulmonary trunk and adjacent structures. We treated of 3 patients with descending necrotizing mediastinitis that is often to be fatal. There are 3 important issues regarding the treatment of this disease. First, the precise sites of abscess should be determined by computed tomography (CT) scans from the neck to diaphragm. Second, effective drainage of the neck and mediastinal abscess should be carried out immediately when the sites of abscess are determined. Third, drainage under video-assisted thoracic surgery (VATS) is an appropriate treatment because VATS is less invasive and provides an easier placement of the drainage tubes at a

Descending cervical mediastinitis is an uncommonly reported presentation of infection originating in the head or neck and descending into the mediastinum, which is fraught with impressive morbidity and mortality rates of 30% to 40% or more. We present the INOVA-Fairfax-Alexandria Hospital experience with descending cervical mediastinitis, January 1, 1986, to April 1, 1997; in addition we review the English-language medical and surgical literature with regard to this entity. Computed tomography and magnetic resonance imaging serve to aid both diagnosis and management. The application of broad-s

The authors work is based on experience in the study of 56 patients with acute mediastinitis of various origin who were treated in the period from 1981 to 1990. There were 33 males and 23 females whose ages ranged from 16 to 65 years, Odontogenous mediastinitis was diagnosed in 23, tonsillogenous in 2, and mediastinitis resulting from injury to the esophagus in 31 patients. Instrumental injuries prevailed. All patients underwent operation. The character of the operation was determined by the duration, localization, and severity of the pyonecrotic process in the mediastinum. Diffuse anterior me

From January 1975 through July 1981, ten patients with mediastinitis complicating an oropharyngeal infection, that is, a form of mediastinitis best termed as DNM, were encountered at our institution. Based upon rather relatively stringent diagnostic criteria, 21 other instances were found in the literature from 1960 to 1980, a time period well into the antibiotic era. The predominant underlying oropharyngeal infection was of odontogenic origin, specifically, infection involving the mandibular molars. Bacteriologically, DNM is most frequently a polymicrobial process, with anaerobes playing a ma

Acute mediastinitis is a rare infection that carries high morbidity and mortality. They are complications seen most often with deep sternal wound infections from surgeries with median sternotomies, oropharyngeal and odontogenic infections and esophageal perforations. These conditions should be promptly recognized and treated. Mediastinal granulomas are focal, mass-like lesions commonly resulting from prior granulomatous infections. They are regarded as benign, self-resolving lesions however can cause complications by compression of adjacent mediastinal structures. Chronic fibrosing mediastinit

Descending necrotizing mediastinitis (DNM) is a rare but severe life threatening complication of oropharyngeal infections. We report a young lady who had severe pharyngitis complicated by anterior neck infection that descended to the mediastinum causing necrotizing mediastinitis. The course of her illness was complicated with septic shock, acute kidney injury, tracheo-esophageal fistula, and critical illness polyneuropathy. There was considerable delay in her diagnosis; however, she survived after aggressive surgical intervention, wide spectrum antibiotics, and appropriate intensive care manag

Parapharyngeal abscess secondary to quinsy is a well understood complication; however, its incidence has significantly declined following early use of effective antibiotics. Tracking of infection from the parapharyngeal space through the anatomical planes to cause mediastinitis has a significant mortality rate and requires early detection and aggressive management. A case of quinsy leading to mediastinitis, pericarditis and pleural effusions is presented. It highlights the potentially life-threatening complications of a commonly encountered ENT problem.

BACKGROUND: [corrected] Descending necrotizing mediastinitis (DNM) is an acute, serious, septic disease which results from a complication of oropharyngeal infection. The disease requires a prompt diagnosis and radical surgical treatment to reduce high mortality (40%). The optimal form of mediastinal drainage remains conroversial. The reason for publishing this report is both the fact that DNM is very rare and our experience prefering thoracotomy as an optimal approach to treating the disease.
CASE REPORT: We reported a 34-years-old woman with DNM. The disease began as a peritonsillar abscess.
