Medicaid Expansion and Rural-Urban Disparities in Postpartum Medicaid Coverage.

OBJECTIVE: Rural residents experience higher rates of postpartum uninsurance than their urban counterparts, contributing to rural-urban disparities in maternal health outcomes. We examined how Medicaid expansion under the Affordable Care Act affected rural-urban disparities in the rates of postpartum health insurance coverage among low-income women. METHODS: We used nationally representative data from the 2010-2012 and 2015-2019 American Community Survey to compare rates of postpartum health insurance coverage among low-income women in states that expanded Medicaid and states that did not exp

Medicaid and non-Medicaid case mix differences in Colorado nursing homes.

Profiles of case mix and related variables were compared for 1,064 Medicaid and 459 non-Medicaid residents of 65 freestanding nursing homes in Colorado in 1980. The results point to substantial case-mix differences, with Medicaid residents typically characterized by fewer and less intense long-term care problems as well as greater independence in functioning (ADLs). Policy impacts of these findings in Colorado have included a legislatively mandated home and community-based care program as well as a more stringent preadmission certification program for Medicaid clients. The method of comparing

The Association of Medicaid Estate Recovery with Homeownership, Home Equity, and Medicaid Enrollment.

UNLABELLED: Policy Points This study examines the association between the implementation of Medicaid estate recovery and homeownership, home equity, and Medicaid enrollment among low-income adults. Estate recovery is associated with a decrease in Medicaid enrollment among unmarried, low-income older adults and a decrease in home equity overall and among Black respondents, White respondents, and adults over 74 years of age. These findings suggest that low-income adults may behave as intended, avoiding Medicaid and extracting housing wealth to cover care costs; still, it is worth reconsidering a

Homelessness and Medicaid Churn.

OBJECTIVES: To identify ICD-10-CM diagnostic codes associated with the social determinants of health (SDOH), determine frequency of use of the code for homelessness across time, and examine the frequency of interrupted periods of Medicaid eligibility (ie, Medicaid churn) for beneficiaries with and without this code. DESIGN: Retrospective data analyses of New York State (NYS) Medicaid claims data for years 2006-2017 to determine reliable indicators of SDOH hypothesized to affect Medicaid churn, and for years 2016-2017 to examine frequency of Medicaid churn among patients with and without an in

Medicaid Expansion Through Ballot Initiatives Increased Medicaid Enrollment And Reduced Uninsurance Among Adults.

Between 2019 and 2021, six states expanded Medicaid through voter-approved ballot initiatives, overcoming legislative opposition. We used 2016-23 data from the American Community Survey to compare health insurance coverage outcomes in these six ballot initiative states with those in ten nonexpansion states. We estimated stacked event study regression models to account for variation in expansion timing and control for socioeconomic differences. Overall, ballot initiative expansions increased Medicaid enrollment by 3.2 percentage points and decreased uninsurance by 2.2 percentage points among no

Associations Between Medicaid Insurance, Facility Medicaid Share, and Guideline-concordant Prostate Cancer Care.

OBJECTIVE: To investigate the dynamics between both patient-level Medicaid status and facility-level Medicaid share and receipt of guideline-concordant care for localized PCa. METHODS: Using the National Cancer Database, we identified a cohort of individuals aged 21-64 diagnosed with localized PCa from 2014 to 2021. The primary outcome was receipt of guideline-concordant management, defined as active surveillance for "low risk," and either radical prostatectomy or androgen deprivation therapy and radiation therapy for "unfavorable intermediate" and "high risk" PCa. We evaluated the associatio

Medicaid's role in critical care after Medicaid expansion: evidence from Virginia.

INTRODUCTION: Medicaid provides access to care for low-income patients facing life-threatening illnesses who are cared for in intensive care units (ICUs). Despite the growth of Medicaid coverage with the Affordable Care Act, little is known about Medicaid's role in critical care for the Medicaid Expansion population, adults ages 19-64. METHODS: Using hospital discharge data from Virginia, we examined payer composition between 2016 and 2023 and analyzed 2023 demographic and clinical data for adults ages 19-64. RESULTS: Medicaid's share of ICU stays more than doubled from 2016 (14.1%) to 2023

The Impact of Increased Medicaid Eligibility During Pregnancy on Medicaid Utilization and Gestational Age.

OBJECTIVE: To assess the impact of increased Medicaid income eligibility during pregnancy on payment source for prenatal care and birth and on gestational age at birth (GAb). STUDY SETTING AND DESIGN: We performed a quasi-experimental, difference-in-differences study comparing two increases in Medicaid income eligibility during pregnancy to two control states with data from 2007 to 2010: (Dyad 1) Ohio (expanded from 150% to 200% of the Federal Poverty level [FPL]) versus Pennsylvania and (Dyad 2) Wisconsin (185% to 250% FPL) versus Michigan. Medicaid's Cranio-Cap: Medicaid Reimbursement for Craniosynostosis Repair is Not Trending With the Rate of Inflation.

BACKGROUND: Craniosynostosis requires early diagnosis and intervention to mitigate the risks of impaired cranial growth; however, patients with Medicaid face treatment delays for craniosynostosis care. Medicaid reimbursement rates have failed to match the inflation rate in medicine, remaining relatively static in many surgical fields. This study investigates Medicaid reimbursement and the longitudinal impact of economic inflation on reimbursement for craniosynostosis care. METHODS: Using the Current Procedural Terminology (CPT) codes for operations for craniosynostosis, the authors analyzed M

Effect of Medicaid expansion on cancer treatment and survival among Medicaid beneficiaries and the uninsured.

BACKGROUND: The Affordable Care Act expanded Medicaid coverage for people with low income in the United States. Expanded insurance coverage could promote more timely access to cancer treatment, which could improve overall survival (OS), yet the long-term effects of Medicaid expansion (ME) remain unknown. We evaluated whether ME was associated with improved timely treatment initiation (TTI) and 3-year OS among patients with breast, cervical, colon, and lung cancers who were affected by the policy. METHODS: Medicaid-insured or uninsured patients aged 40-64 with stage I-III breast, cervical, col

Variation in the interpretation and application of the Medicaid sterilization consent form among Medicaid officials.

OBJECTIVE: The Medicaid consent policy has been identified as a major barrier to desired permanent contraception, particularly for low-income communities and communities of color. As each state may modify their state Medicaid sterilization consent form, variation in the form has been reported. This study aims to characterize state-level variation in Medicaid Title XIX consent form interpretation and application. STUDY DESIGN: We aimed to collect primary data from Medicaid officials in all 50 United States from January to May 2020 via a 25-question electronic survey regarding state-level conse

Medicaid Coverage Disruptions Among Children Enrolled in North Carolina Medicaid From 2016 to 2018.

IMPORTANCE: Brief disruptions in insurance coverage among eligible participants are associated with poorer health outcomes for children. OBJECTIVE: To describe factors associated with coverage disruptions among children enrolled in North Carolina Medicaid from 2016 to 2018 and estimate the outcome of preventing such disruptions on medical expenditures. DESIGN SETTING AND PARTICIPANTS: This was a retrospective cohort study using North Carolina Medicaid claims data. All enrolled individuals were aged 1 to 20 years on January 1, 2016, and with 30 days of prior continuous enrollment. Children we

Medicaid 'Unwinding:' Much Of The Reduction In Medicaid-Paid Prescriptions Was Offset By Increased Commercial Coverage.

With the expiration of the Medicaid continuous coverage condition on March 31, 2023, states began returning to regular eligibility renewals in Medicaid and the Children's Health Insurance Program (CHIP). Because of incompleteness of administrative data and lags in the availability of survey data, there is limited understanding of how this "unwinding" process has affected insurance coverage or access to care. Using data from IQVIA PayerTrak, a large, nationally representative, all-payer pharmacy transactions database, we examined the trends in prescription drug use during the unwinding period.

The Effect of Medicaid Expansion on Delivery of Finger and Thumb Replantation Care to Medicaid Beneficiaries and the Uninsured.

BACKGROUND: Despite advances in replantation, over 80 percent of finger and thumb amputation injuries in the United States result in revision amputation. Although numerous factors contribute to this, disparities in access and delivery of replantation care play a substantial role. With ongoing Medicaid expansion under the Affordable Care Act, it is prudent to understand whether expansion of coverage changes use of replantation care. METHODS: The authors used the 2001 Medicaid expansion in New York State to evaluate changes in replantation for Medicaid beneficiaries and the uninsured. Data for

Treatment of Acute Cholecystitis: Do Medicaid and Non-Medicaid Enrolled Patients Receive the Same Care?

BACKGROUND: Nationally, Medicaid enrollees with emergency surgical conditions experience worse outcomes overall when compared with privately insured patients. The goal of this study is to investigate disparities in the treatment of cholecystitis based on insurance type and to identify contributing factors. METHODS: Adults with cholecystitis at a safety-net hospital in Central Massachusetts from 2017-2018 were included. Sociodemographic and clinical characteristics were compared based on Medicaid enrollment status (Medicare excluded). Univariate and multivariate analyses were used to compare t

Updated Medicaid-To-Medicare Fee Index: Medicaid Physician Fees Still Lag Behind Medicare Physician Fees.

Medicaid plays a critical role in the US health insurance system, but a history of low physician fees has limited physicians' participation in the program. Recent Centers for Medicare and Medicaid Services rules have sought to encourage states to increase their Medicaid physician fees to at least 80 percent of Medicare fees, based on the methods used in the Urban Institute's periodic surveys of Medicaid physician fees. In this article, we show that between 2019 and 2024, Medicaid fees increased slightly, from 72 percent of Medicare fees to 75 percent of Medicare fees for a basket of twenty-sev
