Medical debt in America, part III: Debt collection.

In the first two parts of the series, we described the sources of high prices in health care and the severe constraints faced by low-income patients when seeking to access care without incurring medical debt. In this third and final installment, we examine the origins of aggressive medical debt collection practices, their deleterious consequences for patients, and potential solutions.

Medical Debt, Credit Card Debt, and Other Financial Debt Among Veterans Who Receive Homeless Assistance in the Veterans Affairs Healthcare System.

The US Department of Veterans Affairs (VA) healthcare system is dedicated to preventing veteran homelessness, but there is limited research on the specific sources of financial debt among homeless veterans. We investigated the proportion of veterans seeking VA homeless services reporting any debt and specific types of outstanding debt, associations between homelessness experiences and debt, and whether these associations vary by sex. Using VA administrative data, we cross-sectionally evaluated various types of outstanding debt among 118 594 homeless and at-risk veterans in the VA healthcare sy

Medical Debt and Advanced Cancer Stage at Diagnosis.

IMPORTANCE: Medical debt affects millions of US residents and has been associated with higher cancer mortality. Whether medical debt burden is associated with incidence of advanced stage cancer, a key determinant of survival, remains unclear. OBJECTIVE: To examine the association between medical debt and cancer incidence in the US. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study used ecological county-level data linking 2016 medical debt with age-adjusted incidence rates of 9 cancers from January 1, 2017, to December 31, 2021. The study included 2958 US counties with available

The Association of Childbirth with Medical Debt in the USA, 2019-2020.

BACKGROUND: Medical debt affects one in five adults in the USA and may disproportionately burden postpartum women due to pregnancy-related medical costs. OBJECTIVE: To evaluate the association between childbirth and medical debt, and the correlates of medical debt among postpartum women, in the USA. DESIGN: Cross-sectional. PARTICIPANTS: We analyzed female "sample adults" 18-49 years old in the 2019-2020 National Health Interview Survey, a nationally representative household survey. MAIN MEASURES: Our primary exposure was whether the subject gave birth in the past year. We had two family-l

Financial Assistance Policy, Hospital Charity Care, and Medical Debt in Collections.

IMPORTANCE: Medical debt is a public health crisis. Hospital bills represent the majority of medical debt and present an opportunity for policy intervention. State financial assistance policies (FAPs) require hospitals to expand patient eligibility. However, it is unknown whether these policies alleviate medical debt. OBJECTIVE: To estimate the association of Oregon's FAP with medical debt in collections and hospitals' charity care (ie, free or discounted care for patients qualified for FAPs) and bad debt (ie, total amount of uncollectible patient accounts) expenditures. Medical debt in America, Part II: Financial support for the most vulnerable.

In this second part of a series on medical debt and the costs of care, we examine the past and present financial protections for low-income patients at nonprofit hospitals. Born of almshouses and religious orders, nonprofit hospitals were devoted at their founding to the care of the poor. However, over the course of the twentieth century, they became more focused on high-priced care for paying patients. Federal regulations surrounding tax exemption and charity care have been loosened to allow hospitals to spend relatively little on financial assistance, contributing to inequitable and inadequa

Medical Debt Among US Veterans: Structural Barriers, Financial Strain, and Policy Recommendations.

OBJECTIVES: Medical debt is a challenge for US veterans. Depending on eligibility, some veterans receive care outside the US Department of Veterans Affairs (VA) system through private health care providers, where out-of-pocket costs may be higher. We examined whether access to VA health care and TRICARE coverage (vs other types of health coverage) was associated with lower rates of medical debt and cost avoidance among veterans. METHODS: This cross-sectional study used data from the 2022 National Health Interview Survey and binomial logistic regression models to examine whether access to VA c

Increased Medical Debt in Collections 1 Year After Emergency General Surgery Admission.

BACKGROUND: Emergency general surgery (EGS) conditions account for 4 million unplanned hospital admissions annually, but whether these acute, usually curable conditions are associated with lasting medical debt is unknown. We hypothesized that post-EGS increases in medical debt would be more strongly associated with patient demographics than treatment factors. STUDY DESIGN: We evaluated 4,216 adults admitted for appendicitis, gallbladder disease, small bowel obstruction, or emergency laparotomy in a statewide EGS collaborative (April 2020-June 2021), linking clinical registry data to quarterly

Beyond past-due bills: The varieties of medical debt used to finance healthcare.

The high cost of medical care in the United States has led to widespread medical indebtedness in the population. While previous work has highlighted the reciprocal relationship between debt and health, we know less about how families use and experience diverse kinds of debt resulting from medical care. This article draws on in-depth interviews with women in the Midwest having trouble paying for healthcare to understand the ways they use debt to finance care for themselves and their families. I find that in addition to conventional medical debt-past-due bills or payment plans from providers-the

County-level medical debt and treatment initiation among individuals newly diagnosed with cancer.

BACKGROUND: The high costs of cancer care may lead to medical debt for patients and families. This study examined the association of county-level medical debt and timely treatment initiation among individuals newly diagnosed with cancer. METHODS: Individuals aged 19 years and older who were newly diagnosed with acute leukemias, diffuse large B-cell lymphoma, Hodgkin lymphoma, female breast cancer, colorectal cancer, and lung cancer with consecutive enrollment in the same insurance type from the month of diagnosis through 90 days afterward were identified from the 2012-2021 Colorado Central Ca

The Association between Medical Debt and Food Insecurity among United States Families with Children.

OBJECTIVE: To characterize the association between medical debt and food insecurity in a recent nationally representative sample of US families with children, and to assess whether family income or health insurance type modifies this association. STUDY DESIGN: We conducted a cross-sectional study of US parents using the 2019-2023 National Health Interview Survey. Multivariable logistic regression models, adjusted for sociodemographic characteristics and survey year, assessed the association between medical debt, defined as self-reported inability to pay medical bills, and food insecurity. Medical Debt Among Insured Adults: The Impact of Insurance and Transportation Barriers by Metropolitan Status.

PURPOSE: Medical debt is a significant barrier to health equity in the United States, disproportionately affecting rural populations. Understanding how insurance and transportation barriers contribute to this burden is critical for informing interventions. METHODS: Using 2022 Health Reform Monitoring Survey data (N = 7761), we applied generalized linear models (GLMs) to examine associations of insurance-related and transportation barriers with medical debt among insured adults. Analyses assessed whether these relationships varied by metropolitan statistical area (MSA) status. FINDINGS: Overa

Presence of Any Medical Debt Associated With Two Additional Years of Homelessness in a Seattle Sample.

Although medical debt has been associated with housing instability, almost no research has connected homelessness to medical debt. We interviewed 60 individuals experiencing homelessness in Seattle, selected from those participating in self-governed encampments organized by a homeless advocacy organization. Most respondents reported having at least one kind of debt, with two-thirds reporting current medical debt. Almost half reported trouble paying medical bills for themselves or family members. Almost one-third believed medical debt was in part responsible for their current housing situation.

Impact of Medical Debt on the Financial Welfare of Middle- and Low-Income Families across China.

BACKGROUND: Medical debt is a persistent global issue and a crucial and effective indicator of long-term family medical financial burden. This paper fills a research gap on the incidence and causes of medical debt in Chinese low- and middle-income households. METHOD: Data were obtained from the 2015 China Household Finance Survey, with medical debt measured as borrowings from families, friends and third parties. Tobit regression models were used to analyze the data. The concentration index was employed to measure the extent of socioeconomic inequality in medical debt incidence. RESULTS: We f

Prevalence and Correlates of Anxiety and Depression Symptoms Among Medical Students: The Role of Student Debt Burden.

BACKGROUND: This cross-sectional study examined whether anxiety and depression symptoms decrease as medical students progress through training (suggesting adjustment disorder) or persist (suggesting chronic conditions), while identifying associated risk factors, including student debt burden and treatment effectiveness. METHODS: We surveyed medical students (years 1-4) at VHS-ODU using a sociodemographic questionnaire and validated screening instruments (GAD-7 for anxiety and PHQ-9 for depression) via RedCap from September to November 2024. RESULTS: Among 126 students, Scores were higher in

Medical debt during epidemics: A case for resolving the situation in low- and middle-income countries such as Kenya.

This paper evaluates the problem of medical debt in Kenya during the COVID-19 pandemic. The medical debt problem is compounded during pandemics such as COVID-19 when patients seek treatment and end up in insurmountable debt because illnesses related to the pandemic are not covered by the Kenyan National Health Insurance Fund (NHIF), the public health coverage body under government control. As a result, discharged patients may be detained in hospitals and dead bodies are locked away in mortuaries, until relatives and friends fundraise and clear the bills. Apart from causing vulnerability, fear,

The Financial Burden of Nonoperatively Treated Pediatric Distal Radius Fractures: Medical Debt in Privately Versus Publicly Insured Patients.

BACKGROUND: Patients with public health insurance have greater difficulty obtaining orthopaedic care than their privately insured counterparts because of lower reimbursements. However, the relationship between insurance status and financial burden for patients and treating institutions is unknown. We compared patient medical debt and uncompensated hospital costs by insurance type for pediatric patients who received nonoperative treatment for distal radius fractures (DRFs). METHODS: We reviewed medical records of 100 pediatric patients (above 18 y) treated nonoperatively at our US academic hos
