BACKGROUND: Electronic Patient Medication Record (ePMR) systems have important safety features embedded to alert users about potential clinical hazards and errors. To date, there is no synthesis of evidence about the effectiveness of these safety features and alerts at the point of pharmacy order entry. This review aims to systematically explore the literature and synthesise published evidence about the effectiveness of safety features and alerts in ePMR systems at the point of pharmacy order entry, in primary and secondary care.
METHODS: We searched MEDLINE, EMBASE, Inspec, International Pha

OBJECTIVES: Technology-related prescribing errors curtail the positive impacts of computerised provider order entry (CPOE) on medication safety. Understanding how technology-related errors (TREs) occur can inform CPOE optimisation. Previously, we developed a classification of the underlying mechanisms of TREs using prescribing error data from two adult hospitals. Our objective was to update the classification using paediatric prescribing error data and to assess the reliability with which reviewers could independently apply the classification.
MATERIALS AND METHODS: Using data on 1696 prescri

BACKGROUND: Improving our understanding of the association between medication errors and health information technology (health IT) usability has the potential to reduce errors and improve patient safety. This study used patient safety event reports (PSEs) to investigate the contribution of usability challenges associated with the electronic medication administration record (eMAR) to medication errors.
METHODS: Free-text descriptions of 849 medication-related PSEs selected from 2.3 million reports were analyzed. Coders identified the specific health IT components, usability challenge categorie

The pharmacologic treatment for cancer must adhere to complex, finely orchestrated treatment plans, including not only chemotherapy medications, but pre/post-hydration, anti-emetics, anti-anxiety, and other medications that are given before, during and after chemotherapy doses. The treatment plans specify the medications and dictate precise dosing, frequency, and timing. This is a challenge to most Computerized Physician Order Entry (CPOE), Pharmacy and Electronic Medication Administration record (eMAR) Systems. Medications are scheduled on specific dates, referred to as chemo days, from the o

The number of medication errors reported to The Danish National Board of Health in Denmark exceeds 5000 per year. It is well known that computerized physician order entry (CPOE) with addition of decision support tools may reduce the frequency of medication errors. The primary scope of the work in Denmark has been to help health care professionals avoiding harmful errors. Using data primarily from The Danish National Board of Health, based on the reports of errors from Danish hospitals, and with our previous foundation in the international literature, we analyzed the errors which led to harmful

OBJECTIVE: High-risk medicine (HRM) prescribing errors in pediatrics are a safety concern. Technology-related errors (TREs) occur when the design of computerized provider order entry (CPOE) facilitates errors. We aimed to measure the proportion of HRM errors that were TREs and the HRM TRE rate at 2 pediatric hospitals, describe TRE types, and examine their underlying mechanisms.
METHODS: We identified 7014 HRM orders from retrospective medical record review of 18 951 medication orders for 2192 pediatric inpatients at 2 hospitals with the same CPOE in 2020. HRM prescribing errors were classifi

BACKGROUND: Healthcare overuse is a widespread issue, accounting for approximately 10–30% of total healthcare costs. This study examines the impact of a computerized alert system designed to reduce inappropriate laboratory tests across five public hospitals in southern Switzerland. METHODS: A 32-month observational study - between July 2021 and March 2024 - was conducted to assess the effectiveness of the alert system in discouraging unnecessary laboratory testing. Data were collected from five public hospitals where 12.4% of laboratory tests performed were identified as non-compliant with rec

BACKGROUND: Blood product ordering is a complex process and mistakes can harm patients and lead to poor outcomes. Orders and order sets can be designed to help mitigate errors, but major changes in design can unintentionally cause new errors.
OBJECTIVES: Our objective was as follows: (1) utilize formative in situ usability testing to iteratively improve the design of a redesigned blood product order set prior to go-live, (2) implement changes based on feedback derived from this testing, and (3) compare the error rate, system usability scale (SUS) score, time to task completion, and click coun

Healthcare IT leaders nationwide are moving forward steadily to introduce evidence-based care tools into clinician workflows, both with regard to the development of order sets in computerized physician order entry (CPOE) systems, as well as in the context of the medical diagnosis process. As they advance in these areas, they are finding that building everything from scratch is simply too labor-intensive a prospect. And as hospitals and medical groups turn increasingly to the commercially sponsored offerings available, they are finding the insertion of evidence-based decision support to be a pl

UNLABELLED: Inpatient sleep loss can worsen health outcomes, including delirium and falls. Sleep disruptions in the hospital often originate from provider-patient interactions ordered electronically through computerized provider order entry. These orders contain clinical decision support systems with default schedules. These defaults are often around-the-clock, may not align with patients' needs, and cause iatrogenic sleep loss. Optimizing clinical decision support in the electronic health record can decrease unnecessary sleep disruptions and influence sleep-friendly decision-making.

Alerts in potassium(K+)-increasing drug-drug interactions (DDIs) are often ignored due to their low specificity. Although different approaches have been implemented to address DDIs, subsequent clinical studies revealed poor adherence to such alerts. We therefore suggest a novel alert concept currently being evaluated in a randomized clinical trial in a large teaching hospital. Highly specific reminders (to monitor K+) and alerts (of hyperkalaemia) are displayed to the physicians of the intervention group, whereas reminders and alerts are suppressed in the control group. Preliminary analysis sh

INTRODUCTION: Duplicate prescribing clinical decision support alerts can prevent important prescribing errors but are frequently the cause of much alert fatigue. Stat dose prescriptions are a known reason for overriding these alerts. This study aimed to evaluate the effect of excluding stat dose prescriptions from duplicate prescribing alerts for antithrombotic medicines on alert burden, prescriber adherence, and prescribing.
MATERIALS AND METHODS: A before (January 1st, 2017 to August 31st, 2022) and after (October 5th, 2022 to September 30th, 2023) study was undertaken of antithrombotic dup

BACKGROUND: Therapeutic duplication, the presence of multiple agents prescribed for the same indication without clarification for when each should be used, can contribute to serious medical errors. Joint Commission standards require that orders contain clarifying information about when each order should be given. In our system, as needed (PRN) acetaminophen and ibuprofen orders are major contributors to therapeutic duplication.
OBJECTIVE: The objective of this study is to design and evaluate effectiveness of clinical decision support (CDS) to reduce therapeutic duplication with acetaminophen

BACKGROUND: Clinical decision support systems are implemented in many hospitals to prevent medication errors and associated harm. They are however associated with a high burden of false positive alerts and alert fatigue. The aim of this study was to evaluate a drug-drug interaction (DDI) clinical decision support system in terms of its performance, uptake and user satisfaction and to identify barriers and opportunities for improvement.
METHODS: A quantitative evaluation and end-user survey were performed in a large teaching hospital. First, very severe DDI alerts generated between 2019 and 20

BACKGROUND: Potentially inappropriate prescribing in outpatient care contributes to adverse outcomes and health care inefficiencies. Clinical decision support systems (CDSS) offer promising solutions, but their effectiveness is often constrained by incomplete medical records.
OBJECTIVE: This study aims to evaluate the effectiveness of a machine learning-based CDSS for enhancing diagnostic recommendations, which are system-suggested diagnoses, ensuring that each prescribed medication has a corresponding diagnosis documented and meets medication appropriateness.
METHODS: This prospective singl

Despite the proven usefulness of appropriate clinical decision support (CDS) alerts, many CDS systems fire excessive, clinically irrelevant alerts that are often ignored by clinicians. We have developed a method to suppress false-positive alerts based on prior drug tolerance but encountered substantial barriers to integrating the method into widely adopted commercial electronic health record (EHR) systems. This study aimed to describe the challenges faced while attempting to integrate our method into the CDS infrastructure of two commercial EHR systems and provide recommendations for future res

BACKGROUND: Automated dispensing cabinets (ADCs) are used to store and dispense medications at the point of care. Medications accessed from an ADC before pharmacist order verification are removed using override functionality. Bypassing pharmacist verification can lead to medication errors; therefore, The Joint Commission considers overrides acceptable only in limited scenarios. During an 18-month period, the override rate in our perianesthesia care unit (PACU) was 17%, with oral midazolam accounting for roughly 40% of overrides. A multidisciplinary quality improvement (QI) project was initiate

INTRODUCTION/AIMS: High-risk medication exposure is a modifiable risk factor for myasthenic exacerbation and crisis. We evaluated whether real-time electronic clinical decision support (CDS) was effective in reducing the rate of prescribing potentially high-risk medications to avoid or use with caution in patients with myasthenia gravis.
METHODS: An expert panel reviewed the available drug-disease pairings and associated severity levels to activate the alerts for CDS. All unique alerts activated in both inpatient and outpatient contexts were analyzed over a two-year period. Clinical context,

OBJECTIVES: Knowledge of the prevalence and characteristics of medication errors in pediatric and neonatal patients is limited. This study aimed to evaluate the incidence and medication error characteristics in a pediatric hospital over 5 years and to determine whether serial error prevention programs to optimize a computerized physician order entry (CPOE) system reduce error incidence.
METHODS: We retrospectively reviewed medication errors documented between January 2015 and December 2019.
RESULTS: A total of 2,591,596 prescriptions were checked, and 255 errors were identified. Wrong dose p
