Randomized Controlled Trial of Self-Directed Care for Medically Uninsured Adults With Serious Mental Illness.

OBJECTIVE: Self-directed care (SDC) is a treatment model in which recipients self-manage funds designated for provision of services. The model is designed to cost no more than traditional services while achieving superior participant outcomes. The authors examined the model's impact on outcomes, service costs, and user satisfaction among medically uninsured, low-income individuals with serious mental illness. METHODS: Adults in the public mental health system (N=42) were randomly assigned (1:1) to receive SDC or services as usual and were assessed at baseline and 6- and 12-month follow-ups. O

[Research-action on medical and social management of uninsured pregnant women].

INTRODUCTION: Perinatal morbidity is increased among women without health insurance and/or receiving poor antenatal care. The objectives of this study were to estimate the prevalence of such situations, to analyze underlying social conditions, to identify possible means to secure health insurance and to assess the impact of the absence of health insurance on antenatal care and pregnancy outcomes. MATERIAL AND METHODS: All pregnant women without health insurance during one calendar year were included. Those who delivered a live child were compared with a control group of women with health insu

Factors Associated with Abnormal Mammogram Results Among Low-Income Uninsured Populations in Medically Underserved And Rural Texas Regions.

BACKGROUND: This study investigated the potential associations between neighborhood characteristics, rurality, ethnicity/race, and breast cancer screening outcomes in designated Health Professional Shortage Areas in Central Texas. Limited access to preventive medical care can impact screening rates and outcomes. Previous research on the effects of factors such as rurality, neighborhood socioeconomic status, and education level on cancer prevention behaviors has yielded inconsistent results. MATERIALS AND METHODS: We analyzed data from a state-funded breast and cervical cancer screening progra

The effect of health insurance expansion on price and demand for medications among the uninsured.

Social health insurance expansions can affect not only new beneficiaries but also those who remain outside the program. This paper studies these spillovers in the context of Medicare Part D. Increases in insured demand can raise or lower price depending on market conditions: they may increase price when pharmaceutical firms face less elastic demand, but reduce price when demand concentration strengthens bargaining and pass-through in competitive markets. I find that Part D lowered the out-of-pocket unit price paid by Americans under age 65 without prescription drug insurance. Price declines ar

Risk profile for breast carcinoma and tumour histopathology of medical uninsured patients in Pakistan.

BACKGROUND: Breast carcinoma is an unpredictable disease in the sense that some patients may die at early disease stage due to wide-spread metastasis within six months to one year, while others may survive longer. This study was aimed to evaluate the risk factors for breast carcinoma occurrence and histopathological features of breast carcinoma developed in the social and economical conditions of Pakistan. METHODS: A total of 224 female breast cancer diagnosed patients with uncovered medical insurance visiting at the Oncology clinic of a teaching hospital at Karachi, Pakistan were selected fo

Impact of a charitable medication program and social vulnerability index on clinical outcomes in uninsured adults with diabetes.

BACKGROUND: Adults with diabetes mellitus (DM) and poor social vulnerability index (SVI) have been shown to have worse clinical outcomes with uncontrolled DM. Patient assistance programs (PAPs), free clinics, and charitable medication programs have been shown to improve hemoglobin A1c (HbA1c) for uninsured adults. OBJECTIVES: The primary objective of this study was to evaluate the impact of a charitable medication program on HbA1c as well as other clinical outcomes (blood pressure [BP], low-density lipoprotein [LDL], and body mass index [BMI]) in adults with type 2 diabetes mellitus (T2DM). Coordinating eye and primary medical care in a low-income and uninsured population: the experience of the Vision of Hope Health Alliance.

PURPOSE: The Vision of Hope Health Alliance (VOHHA) was developed to provide an integrated system of eye and primary medical services to low-income and uninsured patients who were referred from nonmedical and medical agencies. METHODS: Partnerships were formed to facilitate patient referrals. Eye examinations and ophthalmic materials were provided at no cost to patients. Interpretation services and follow-up care were provided as needed. If applicable, patients were scheduled with a primary care physician at a Federally Qualified Health Center. Findings were documented by VOHHA team members o

Improving access to colorectal cancer screening through medical philanthropy: feasibility of a flexible sigmoidoscopy health fair for uninsured patients.

OBJECTIVES: Only half of eligible patients in the United States undergo colorectal cancer (CRC) screening as recommended. Hypothesizing that the medical philanthropy platform may be effective in improving access to CRC screening, we aimed to demonstrate the feasibility of a flexible sigmoidoscopy (FS)-based CRC screening "health fair" for uninsured patients. METHODS: Uninsured patients older than 50 years who had not undergone CRC screening in the preceding 10 years were recruited through local free clinics and health fairs. A standard medical clinic was transformed into a fully functional en

Uninsured South Florida vascular surgery patients are less likely to receive optimal medical management than their insured counterparts.

OBJECTIVE: Vascular disease is the most prevalent condition in patients aged >60 years, leading to increasing complications associated with their comorbid conditions. Poor medical compliance could be one reason why the rate of complications may be higher in this patient population, particularly the uninsured. This study was conducted to better assess rates of medication compliance in vascular surgical patients. METHODS: Consecutive patients seen in vascular clinics at a busy tertiary academic center were prospectively studied. Physicians and physician assistants used a standardized questionna

Perspectives of family physicians on the care of uninsured pregnant women.

OBJECTIVE: No official provisions are made for the medically uninsured under provincial public health programs in Canada. Studies have shown that uninsured pregnant women have inadequate access to prenatal and obstetrical services that favour healthy maternal and child outcomes. This qualitative study aimed to explore the perspectives of family physicians who provided care to uninsured pregnant women. METHODS: Eight family physicians affiliated with two Montreal-based primary-care clinics and one tertiary care hospital between 2004 and 2007 were interviewed using a semi-structured interview g

Medicaid's Role in Health Reform and Closing the Coverage Gap.

Medicaid coverage matters for millions of low-income Americans, and especially for those with ongoing and serious health challenges. A source of comprehensive and affordable coverage, Medicaid has long been a cornerstone of federal and state efforts to improve access and health outcomes for very poor and medically vulnerable populations. The Affordable Care Act (ACA) leveraged Medicaid's role in serving the poor to broaden the program's reach to millions of low-income uninsured adults, and positioned the program as a fundamental component of the newly established continuum of public and privat

Prevalence of vitamin D deficiency in uninsured women.

BACKGROUND: Vitamin D deficiency, an important risk factor for osteoporosis and other chronic medical conditions, is epidemic in the United States. Uninsured women may be at an even higher risk for vitamin D deficiency than others owing to low intake of dietary and supplemental vitamin D and limited sun exposure. OBJECTIVE: Our goal was to determine the prevalence of vitamin D deficiency in this vulnerable population. SETTING AND PARTICIPANTS: We enrolled 145 uninsured women at a County Free Medical Clinic in urban Michigan. Questionnaires were used to obtain information about demographics,

Cesarean Section Rates and Mobile Health's Role in Equitable Access to Prenatal Care.

Cesarean section (CS) rates have risen globally, and while an often lifesaving and necessary intervention, CS deliveries increase future maternal/neonatal risks and are costly to both patients and healthcare systems. The U.S. Department of Health and Human Services has set a national low-risk pregnancy CS (NTSV) target of 23.9% under the Healthy People 2030 initiative. This analysis compares NTSV rates of uninsured patients receiving prenatal care from a mobile clinic to the national target and also compares overall mobile health CS rates with national and state CS rates. Through reviewing 5 y
