To review the meniscus from a historical perspective especially on surgical management and general guidelines for arthroscopic meniscectomy procedures for various types of meniscal tears. One hundred sixteen patients who had undergone partial arthroscopic meniscectomy were reviewed in a short-term follow-up study (mean follow-up: 16 months). Their results were compared with those of a group of 402 patients who had been treated previously by conventional open meniscectomy in the same hospital. Average age in the arthroscopic group was 40 years, in the open group 34. Average sick leave was 4 weeks in the arthroscopic group, 10 weeks in the arthrotomy group. A series of 264 endoscopic meniscectomies has been analysed. In 8% the endoscopic operation failed, and arthrotomy was performed at the same session. During the latter part of the study, this figure decreased to 4% owing to improved skill and instrument design. 92% of the patients were followed up by scoring-scale evaluation 13.8 +/- 7.7 (2-51) months after treatment. A technique is described for doing arthroscopic meniscectomies using short curved instruments passed through flexible, sealable Teflon sleeves. The goal, with few exceptions, was to achieve en bloc subtotal resection of the posterior two-thirds of the meniscus. The first known operations on the meniscus were done in the late 1800s. Since that time, as the architectural and biomechanical aspects of the knee became known, the tide swung from advocacy of total meniscectomy regardless of the damage involved, to a procedure that removed only the damaged portion of the meniscus. INDICATIONS: Symptomatic, irreparable lesions of the meniscus due to trauma or degeneration.
CONTRAINDICATIONS: Reparable lesions of the meniscus. Local skin affections.
SURGICAL TECHNIQUE: Introduction of the arthroscope through an anterolateral or central portal. The instrument portal is positioned in accordance with the situation of the meniscal lesion to be treated.

Lesion of the meniscus is the most frequently occurring lesion in the region of the knee joint. Opinions are still divided as to the effects of partial, subtotal or total meniscectomy in provoking and/or promoting arthrosis. In this study, 70 patients with isolated injury of the meniscus were followed up on an average 11 years after meniscectomy. INTRODUCTION: The theoretical advantages of the Ho-Yag laser make it the ideal instrument for current use in arthroscopic meniscectomy. Short term results showed less patient discomfort, rapid recovery and less post operative pain and swelling. The purpose of this single blind prospective randomized study was to compare the results of arthroscopic meniscectomy performed mechanically to those obtained with the Ho-Yag laser, with a minimum follow up of one year.

Arthroscopic treatment of meniscal pathology has become a routine procedure within the last 10 years. In our patients who underwent arthroscopic surgery for multiple reasons we found 226 cases with combined lesions of the medial and lateral menisci. Eliminating the patients with instabilities, chondral lesions grade IV and general diseases like rheumatoid arthritis and those who had had surgery previously, 71 patients were left. It would appear that arthroscopic partial menisectomy offers as good, if not better, results compared with open meniscectomy. Authors who have compared patients undergoing open meniscectomy with those undergoing arthroscopic meniscectomy have also shown that the other variables and parameters so important to the patient in the early postoperative period weight very heavily in favor of arthroscopic meniscectomy.

The results of 100 arthroscopic partial meniscectomies are reported. Hospitalisation was on average 1.5 days; return to work 3 weeks and to sports activities 7 weeks. The clinical and roentgenological data on 30 patients with unsatisfactory recovery or recurrent symptoms after meniscectomy were analysed. The meniscectomy was regarded as having been the wrong choice of therapy in four patients and as ineffective therapy in 11. A case of arteriovenous fistula between the popliteal artery and vein after injury during medial meniscectomy is presented. The diagnosis was made on the fifth postoperative day by angiography, and a successful reconstruction of the artery and vein was accomplished by primary anastomosis of the artery following resection of the damaged part; the vein was closed by lateral venorrhaphy.

A technique of arthroscopic excision of a torn meniscus has evolved during two years' experience of therapeutic arthroscopy of the knee joint. The same method is used for both medial and lateral meniscus lesions under full visual control throughout the procedure, with a standard arthroscope inserted centrally through the patellar tendon.
