Effects of Tibial Rotational-guided Growth on the Geometries of Tibial Plateaus and Menisci in Rabbits.

BACKGROUND: There are studies on the use of oblique plate over epiphyses for rotational deformities but the effects of this method on bone are not yet known. The purpose of this study was to determine the effect and rebound effect of rotational-guided growth on the geometries of the tibial plateaus and menisci in a rabbit model of tibial axial rotation. METHODS: Thirty male rabbits were 6 weeks old when medial and lateral plates were applied to the proximal tibias of the left side. After 4 weeks, 15 rabbits were euthanized (group 1), and the plates from the tibias of the remaining 15 rabbits

Evaluation of the Effect of Menisci on Tibial Slope and the Correlation With Body Mass Index.

Background One of the important factors affecting the biomechanics of the knee joint is the posterior tibial slope which is the tibial plateau's anatomical inclination toward the posterior of the sagittal plane. This inclination, which affects anterior-posterior stability, is important for the kinematics of the knee joint. Changes in the tibial slope may cause a deficit in the stability and function of the knee joint. We aimed to examine the inclination of the posterior horn of the meniscus and posterior tibial slope in healthy individuals and investigate the effect of body mass index on these

Comparison of the Visibility of Canine Menisci before and after Tibial Plateau Leveling Osteotomy: 3D-Printed Model Study.

The aim of this study was to compare the degree of visibility of the lateral and medial menisci before and after tibial plateau leveling osteotomy (TPLO) on 3D-printed models created after laser scanning of the right tibia with menisci derived from a fresh cadaver of a 4-year-old adult male golden retriever. The models were produced of white polylactic acid, and the menisci were filled with light-curing red resin. The models showed a similar conformation as the natural specimen harvested from the cadaver, maintaining the same length and width, in addition to reproducing the anatomical structur

The effect of proximal tibial corrective osteotomy on menisci, tibia and tarsal bones: a finite element model study of tibia vara.

BACKGROUND: Proximal tibial open wedge osteotomy (PTO) is a corrective operation used in the surgery of lower extremities and is applied to patients with varus deformities for sufficient correction. The aim of the study was to evaluate whether the PTO can achieve decreased stress-bearing on the tibia and tarsal bones in addition to correcting the mechanical axis of the lower limb in patients with tibia vara. METHODS: Three-dimensional (3D) solid modelling of the lower extremity was carried out using computed tomography (CT) and magnetic resonance (MR)-containing images of all of the bony elem

Evaluation of meniscal and articular cartilage lesions in femoral and tibial condyles in dogs without cranial cruciate ligament insufficiency: cadaveric study.

BACKGROUND: The menisci of the stifle joint are essential for impact absorption, joint stability, and load support. Meniscal injuries, particularly of the medial meniscus, can lead to osteoarthritis and are generally associated with cranial cruciate ligament instability. Lateral meniscal injuries are less frequent. The aim of this study was to describe the frequency of medial or lateral meniscal injuries and cartilage lesions on the tibial and femoral condyles in cadaveric dogs with intact cranial cruciate ligaments, and to correlate the meniscal injuries with the tibial plateau angle and the

Current concepts on structure-function relationships in the menisci.

The menisci are intricately organized structures that perform many tasks in the knee. We review their structure and function and introduce new data about their tibial and femoral surfaces. As the femur and tibia approach each other when the knee is bearing load, circumferential tension develops in the menisci, enabling the transmission of compressive load between the femoral and tibial cartilage layers. A low shear modulus is necessary for the tissue to adapt its shape to the changing radius of the femur as that bone moves relative to the tibia during joint articulation. The organization of th

A Morphological Study of the Meniscus, Cartilage and Subchondral Bone Following Closed-Joint Traumatic Impact to the Knee.

Post-traumatic osteoarthritis (PTOA) is a debilitating disease that is a result of a breakdown of knee joint tissues following traumatic impact. The interplay of how these tissues influence each other has received little attention because of complex interactions. This study was designed to correlate the degeneration of the menisci, cartilage and subchondral bone following an acute traumatic event that resulted in anterior cruciate ligament (ACL) and medial meniscus tears. We used a well-defined impact injury animal model that ruptures the ACL and tears the menisci. Subsequently, the knee joint

Tibial plateau fractures: a study of associated soft tissue injuries.

Tibial plateau fractures can occur concomitant with injuries to the collateral and cruciate ligaments, menisci, and surrounding nerves and arteries. The purpose of this article is to record the frequency of these soft tissue injuries with tibial plateau fractures. Patients with plateau fractures were entered into a prospective protocol consisting of radiographic evaluation, Schatzker classification, pre- and postfixation stress testing, and diagnostic arthroscopy. Thirty tibial plateau fractures were evaluated in this study. There was a 56% (17 of 30) frequency of associated soft tissue injuri

The Influence of Obesity and Meniscal Coverage on In Vivo Tibial Cartilage Thickness and Strain.

BACKGROUND: Obesity, which potentially increases loading at the knee, is a common and modifiable risk factor for the development of knee osteoarthritis. The menisci play an important role in distributing joint loads to the underlying cartilage. However, the influence of obesity on the role of the menisci in cartilage load distribution in vivo is currently unknown. PURPOSE: To measure tibial cartilage thickness and compressive strain in response to walking in areas covered and uncovered by the menisci in participants with normal body mass index (BMI) and participants with high BMI. Femoro-tibial and menisco-tibial translation patterns in patients with unilateral anterior cruciate ligament deficiency--a potential cause of secondary meniscal tears.

OBJECTIVE: To analyze menisco-tibial and femoro-tibial translation patterns in healthy and ACL-deficient knees in different knee flexion angles under muscle activity. METHODS: The ACL-deficient and contralateral healthy knees of 10 patients were examined with an open MRI system at 30 degrees and 90 degrees of knee flexion, under isometric contraction of the extensors or flexor muscle groups. Translations between the tibia, the femoral condyles and the menisci were analyzed by three-dimensional image postprocessing. RESULTS: Posterior translation of the femur and menisci relative to the tibia

The influence of meniscal lesions on reflex activity in the hamstring muscles.

Meniscal lesions are among the most common knee injuries and substantially affect the mechanical function of the knee joint. In addition, the presence of mechanoreceptors in the meniscus suggests that the menisci are also involved in the sensorimotor control of the knee. The objective of this study was to investigate the influence of an isolated meniscal lesion on the reflex behaviour of the hamstring muscles, which are involved in the sensorimotor control of the knee joint. For this purpose, we mechanically induced tibial translation in 19 patients in a standing position and analysed the biph

[Treatment concept in complex fractures of the head of the tibia].

Complex tibial condylar fractures are intraarticular fractures with associated lesions of capsule and ligaments, menisci, soft tissue envelope and neurovascular structures. They are in general produced by a high-energy trauma and are usually part of a polytraumatized patient. The frequency for the associated lesions are up 50% for the collateral ligaments, and up to 40% for the cruciate ligaments and the menisci, respectively. The paper presents the crucial steps of clinical and radiological assessment and outlines a concept of treatment, documented by two illustrative cases.

Intra-articular risks of suprapatellar nailing.

To determine the risks to local anatomy near the starting point for tibial nailing during suprapatellar nailing, 15 fresh-frozen hemipelvis specimens were nailed using a suprapatellar technique. After nail passage, the menisci and articular surfaces, anterior cruciate ligament (ACL) insertion, intermeniscal ligament, and fat pad were assessed for injury. The distance from the entry portal to the menisci, articular surfaces, and ACL insertion was determined. Medial meniscus injury occurred in 1 (6.7%) specimen and medial articular injury in 2 (13%). Nails passed through the fat pad in all speci

Avulsion of the tibial tuberosity with a lateral plateau rim fracture: case report.

An avulsion fracture of tibial tuberosity with an unusual articular involvement was treated with open reduction and internal fixation. Although the mechanism of tibial tuberosity fracture is described as passively flexing the knee while active quadriceps femoris contraction, the weight transmitted from the menisci on the lateral side of the knee joint may cause an epiphyseal disruption. We report a case of tibial tuberosity fracture with lateral plateau rim fracture in a young male adolescent, which has not been published previously in the literature.

Anterior-posterior stability of the knee by an MR image subtraction method.

The purpose of our study was to test the hypothesis that when a shear force was applied posteriorly to the loaded knee in vivo, there would be no relative motion between the tibia and the medial femoral condyle. Siemens 7 Tesla high-resolution MRI machine was used to scan eight healthy male volunteers with the knee at 15° of flexion. Two scans were obtained: the first with a compressive force of 660 N along the tibial long axis and a second with the compressive force and a posterior shear force of 36 N applied to the tibia. Solid models were created of the femur, tibia, and menisci for both lo

Biomechanical considerations of the menisci of the knee.

The menisci of the knee are important structures for maintaining a healthy joint since they absorb shock and prevent rotatory instability. The shapes of the menisci and the orientations of their collagen fibres are naturally suited for bearing weight of the joint. Their shape also allows the tibial plateau contact area to be three times greater, thereby decreasing the stress by up to seven times. The medial and lateral menisci are able to move to different extents as a result of the variable laxities of the adjoining ligaments. Often a meniscus becomes injured and must be surgically removed by

[Meniscus transplantation using a cryopreserved allograft--an experimental study in the rabbit].

In twenty rabbits with completion of bone growth, the medial meniscus was replaced with an allograft meniscus that had been preserved at -80 degrees C for 6 to 14 days. As a control, the medial meniscus was removed and reattached without cryopreservation in 14 rabbits. Replaced menisci were examined histologically and microangiographically. Allografted menisci were found to attach to the capsule at 4 weeks. However, the attachments of the anterior and posterior horns were not stable in the majority of rabbits, because the nylon thread for anchoring was broken in these cases. There was osteophy
