Mapping Cognitive Trajectories and Detecting Early Dementia Using the Mini-Mental State Examination Cognitive Charts: Application to the French Three-City Cohort.

The Cognitive Quotient (QuoCo) classification algorithm monitoring decline on age- and education-adjusted Mini-Mental State Examination (MMSE)-derived cognitive charts has proved superior to the conventionally-used cut-off for identifying incident dementia; however, it remains to be tested in different settings. Data were drawn from the Three-City Cohort to 1) assess the screening accuracy of the QuoCo, and 2) compare its performance to that of serial MMSE tests applying different cut-offs. For the QuoCo, sensitivity was 74.2 (95% CI: 71.4-76.8) and specificity 84.1 (83.6-84.7) and for the MMS

Accuracy of immediate self-assessment of neurocognitive test performance: Associations with psychiatric diagnosis and longitudinal psychotic symptoms.

Participants with schizophrenia (SCZ) and bipolar disorder (BD) have challenges in self-evaluation of their cognitive and functional abilities, referred to as introspective accuracy (IA). Although psychotic symptoms are commonly found to be uncorrelated with cognitive performance, many models of the development of delusions focus on failures in self-assessment and responses biases during momentary monitoring. We performed a single 4-test cognitive assessment on 240 participants (schizophrenia n = 126; bipolar disorder n = 114) and asked them to make a judgment about their performance immediate

Evaluation of Suspected Dementia.

Dementia is a major neurocognitive disorder involving deficits that interfere with daily function. Age is the greatest risk factor for developing the disease. Other risk factors include family history, cardiovascular disease, uncontrolled diabetes, and lower education levels. The initial evaluation for dementia involves recognizing subtle signs that are often missed or mistaken for normal aging. Screening tools include the Mini-Cog, Memory Impairment Screen (MIS), and questionnaires that are completed by caregivers or friends. If cognitive impairment is suspected, a more detailed evaluation sh

Cognitive Neurology Continuing Medical Education: History Taking and Bedside Mental Status Examination in a Patient with Dementia.

In a patient presenting with forgetfulness, history taking comprises asking questions pertaining to specific cognitive domains namely memory, language, executive function, visuospatial functions, and social cognition to characterize the clinical phenotype. The next step is to administer a standardized screening test for cognitive assessment, namely the Montreal Cognitive Assessment (MoCA)/mini mental status examination (MMSE). These have been validated in five Indian languages. Detailed lobar function tests to assess functions of frontal, temporal, parietal, and occipital lobes namely planning

Monetary value of informal caregiving in dementia from a societal perspective.

BACKGROUND: Dementia is a condition which results in a high cost of care, a significant proportion of which is the cost associated with informal care. In previous studies, informal caregiving has been challenging to assess due to difficulties in estimating the true time spent on caregiving work and how to value caregivers' time. The aim of this study was to compare the costs of dementia among patients living alone and among those living with a caregiver to show the monetary value of informal caregiving from a societal perspective. METHODS: Data from our four dementia trials using the same mea

Digital quantification of the MMSE interlocking pentagon areas: a three-stage algorithm.

The Mini-Mental State Examination (MMSE) is a widely employed screening tool for the severity of cognitive impairment. Among the MMSE items, the pentagon copying test (PCT) requires participants to accurately replicate a sample of two interlocking pentagons. While the PCT is traditionally scored on a binary scale, there have been limited developments of granular scoring scale to assess task performance. In this paper, we present a novel three-stage algorithm, called Quantification of Interlocking Pentagons (QIP) which quantifies PCT performance by computing the areas of individual pentagons an

Brief Cognitive Screening Tools for Dementia in Low-Educated Population from South America: A Systematic Review.

INTRODUCTION: Despite less education being common in Latin America, there is no systematic review on the use of brief cognitive screening (BCS) tools in illiterate and low-educated adults in the region. We systematically reviewed brief cognitive tests used to identify dementia in illiterate or low-educated adults from South America (SA). METHODS: A systematic review was conducted according to the PRISMA and Cochrane guidelines. We searched four major databases: PubMed, Scopus, Web of Science, and Embase, for studies up to September 2023, and included observational studies that reported at lea

Neuropsychiatric symptoms in genetic frontotemporal dementia: developing a new module for Clinical Rating Scales.

BACKGROUND: Current clinical rating scales in frontotemporal dementia (FTD) often do not incorporate neuropsychiatric features and may therefore inadequately measure disease stage. METHODS: 832 participants from the Genetic FTD Initiative (GENFI) were recruited: 522 mutation carriers and 310 mutation-negative controls. The standardised GENFI clinical questionnaire assessed the frequency and severity of 14 neuropsychiatric symptoms: visual, auditory, and tactile hallucinations, delusions, depression, anxiety, irritability/lability, agitation/aggression, euphoria/elation, aberrant motor behavio

Estimating Parkinsonism Severity in Natural Gait Videos of Older Adults With Dementia.

Drug-induced parkinsonism affects many older adults with dementia, often causing gait disturbances. New advances in vision-based human pose- estimation have opened possibilities for frequent and unobtrusive analysis of gait in long-term care settings. This work leverages spatial-temporal graph convolutional network (ST-GCN) architectures and training procedures to predict clinical scores of parkinsonism in gait from video of individuals with dementia. We propose a two-stage training approach consisting of a self-supervised pretraining stage that encourages the ST-GCN model to learn about gait

Prediction of Parkinsonian Gait in Older Adults with Dementia using Joint Trajectories and Gait Features from 2D Video.

Older adults with dementia have a high risk of developing drug-induced parkinsonism; however, formal clinical gait assessments are too infrequent to capture fluctuations in their gait. Camera-based human pose estimation and tracking provides a means to frequently monitor gait in nonclinical settings. In this study, 2160 walking bouts from 49 participants were recorded using a ceiling-mounted camera. Recorded color videos were processed using AlphaPose to obtain 2D joint trajectories of the participant as they were walking down a hallway of the unit. A subset of 324 walking bouts from 14 partic

Neuropsychological screening tests in African Americans.

Neuropsychological tests are instruments used to diagnose a variety of cognitive conditions. This article will review a few of the brief scales commonly used in screening for dementia. It will also discuss the properties of and problems with some of the brief scales that are commonly used to screen African Americans for dementia, highlighting the various biases. The Mini-Mental State Examination (MMSE) is the most widely known and utilized cognitive impairment instrument in the United States. Whether or not it is biased to race after adjusting the scores for educational attainment remains cont

A systematic review of the efficacy of donepezil hydrochloride combined with nimodipine on treating vascular dementia.

BACKGROUND: Vascular dementia (VaD) is a comprehensive syndrome related to the damage of cognitive function and various cerebral vascular illnesses. VaD is also generally recognized as the second most common type of dementia after Alzheimer disease, contributing to 30% of the dementia population in Asia and developing countries. The ability of donepezil hydrochloride and nimodipine had been respectively proven in improving cognitive function in vascular dementia. However, whether the combined application of both drugs contribute to better efficacy remains as a research hotspot. Studies had sho
