Assessing mentalization in psychotherapy: first validation of the Mentalization Imbalances Scale.

The aim of this study was to provide data on the preliminary validation of a clinician-report multidimensional assessment measure of mentalization (Mentalization Imbalances Scale, MIS). A random national sample of psychotherapists (N=190) completed the MIS to identify mentalization imbalances, and the Personality Disorder Checklist to assess the personality disorders (PDs) of randomly selected patients currently in their care. Factor analysis confirmed the presence of six factors that represented different imbalances of mentalization: cognitive, affective, automatic, external, imbalance toward

Enhancing mentalization by specific interventions within mentalization-based treatment of adolescents with conduct disorder.

OBJECTIVE: Mentalization is discussed as a mechanism of change in psychotherapy due to its positive effects on psychological functioning. In order to specifically apply mentalization-based interventions, a better understanding of the relationship between interventions and in-session mentalization is needed. The study aimed to explore the association between interventions and effective mentalizing. METHOD: Fifteen therapy sessions of three therapies with male adolescents with conduct disorder were transcribed and rated with the Reflective Functioning (RF) Scale and a newly developed Mentalizat

A multifaceted approach to assessment of mentalization: the mentalization profile in patients with eating disorders.

Purpose: The existing literature, however sparse, suggests an association between eating disorders (ED) and mentalization ability. The aim of this study was to investigate the mentalization profile (MP) in patients with ED. It was hypothesized that patients with ED would have a lower degree of mentalization ability compared to healthy controls (HC).Materials and methods: The study is based on a cross-sectional survey on a sample of patients diagnosed with ED compared to a HC group. Results and conclusion: A total of 88 participants, distributed between patients with ED (N = 30) and HC (N = 58)

Mentalization-Based Treatment for Adolescent Depression.

Depression is a very common mental health problem in adolescence. Although over the past decades a number of psychological interventions for depression in adolescence have been developed and empirically evaluated, recent systematic reviews and meta-analyses suggest that there is considerable room for improvement of their effectiveness. This is particularly true for the treatment of adolescents with "complex" depression, that is, those where depression is embedded within broader personality and relational problems, often related to a history of attachment trauma. Mentalization-based treatment (

Pedagogical stance in mentalization-based treatment.

BACKGROUND: A common aspect of evidence-based treatments for people with borderline personality disorder (BPD) is pedagogical interventions and formats. In mentalization-based treatment (MBT) the introductory course has a clear pedagogical format, but a pedagogical stance is not otherwise defined. METHODS: Treatment integrity was quantitatively assessed in a sample of 346 individual MBT sessions. Nine group sessions and 24 individual MBT sessions were qualitatively subjected to interpretative phenomenological analysis (IPA). RESULTS: The dominating intervention type was MBT Item 16-therapist

Mentalization-Based Treatment for Adolescents (MBT-A).

In this article we describe the structure and technique of mentalization-based therapy for adolescents (MBT-A), an evidence-based intervention that has shown effectiveness in helping young people with self-harm, borderline personality, and depression. We describe also the differences between MBT with adults and MBT-A, which primarily focuses on the developmental aspects of adolescence. The developmental trajectory of adolescence culminates in a coherent and consolidated sense of self. Mentalizing provides the main supporting socio-cognitive-emotional process for achieving a coherent sense of s

[Crisis-MBT: Mentalization-based brief hospitalization intervention].

Context Our team works in a psychiatric hospital unit at the University Hospitals of Geneva (Switzerland). We welcome there for 7 days people in crisis situations who have either suicidal thoughts or suicidal behavior. As factors precipitating the suicidal crisis, these people go through life events that are accompanied by intense interpersonal difficulties or that threaten the image they have of themselves. In our clinical population, approximately 35% of patients suffer from borderline personality disorder (BPD). In these patients, repeated crises and suicidal behavior lead to frequent and d

Mentalization-based psychotherapy interventions with mothers-to-be.

In recent years, the theory of mentalization has been applied to a number of clinically relevant areas including psychotherapy for patients with borderline personality disorder, therapy with adolescents and children, treatment of self-harm in young people, parent-infant early interventions, and even community outreach (see Midgley & Vrouvam, 2012. Minding the child: Mentalizing interventions with children, young people, and their families. London and New York: Routledge). Extending on this body of work, the present article applies the theory of mentalization to psychotherapies that aim to help

[Mentalization--the ability to keep mind in mind].

Mentalization is the ability to keep mind in mind, both one's own and another person's mind. Without mentalization ability, man is unable to anticipate personal and other person's reactions in different situations and to control his/her own emotional states. Insufficient mentalization ability makes life erratic and uncontrollable. It is associated with emotionally unstable personality and other psychic and neuropsychiatric disorders. At least five evidence-based forms of psychotherapy have been developed for the treatment of emotionally unstable personality disorder, all promoting the patient'

Mentalization-based therapy for personality disorder in adolescents.

PURPOSE OF REVIEW: Mentalization-based therapy for adolescents (MBT-A) is a treatment approach that aligns well with dimensional models of personality disorders. This article reviews recent literature on mentalizing and MBT, proposing ways in which findings may inform future research and clinical innovations. RECENT FINDINGS: There have been few new treatment studies and meta-analyses, and the evidence generally does not support the superiority of MBT-A over other well structured treatments for personality disorder in adolescents. Long-term follow-up studies suggest heterotypic continuity of

[A mentalization-based perspective on childhood autism treatment].

Mentalization is a process by which a subject makes sense of both his own mental representations and of those around him. Disturbances in the mentalization process are found in several psychiatric disorders, notably borderline personality disorders for which mentalization-based treatments (MBT) have been developed and evaluated. Children with Autism Spectrum Disorder (ASD) display a theory of mind impairments, which corresponds to disturbances in the mentalization process. Although no MBT protocol for patients with ASD has been described in the literature, such treatment appears promising to i

Mentalization in Asylum Seekers with Post-Traumatic Stress Disorder.

This study explores the relationship between post-traumatic stress disorder (PTSD) and the two major dimensions of mentalization (self and other-oriented) in asylum seekers resettled in Italy. It is important because it is the first study addressing the role of mentalization in asylum seekers with PTSD. Twenty asylum seekers scoring above the cut-off for the PTSD Checklist for DSM-5 (PCL-5) were contrasted to twenty asylum seekers without PTSD on mentalization and somatization. The Certainty About Mental States Questionnaire (CAMSQ) and the Bradford Somatic Inventory (BSI-21) were used to asse

The construction of sadomasochism: Vicissitudes of attachment and mentalization.

This paper integrates empirical developmental research with the clinical theory and treatment of sadomasochism. Three strands of argument illustrate the thesis that sadomasochistic relatedness develops from relational procedures originating in the mother-infant dyad. The first strand involves attachment research including data from infancy to adulthood and videotaped microanalyses of infant-mother dyads. The second strand addresses research on mentalization and its relationship to attachment. These strands suggest that sadomasochistic relatedness exists on continua from minor to severe as atta

Minding mentalizing - convergent validity of the Mentalization Breakdown Interview.

OBJECTIVES: Mentalizing difficulties are central to borderline personality disorder (BPD), have severe consequences, and are an explicit focus in mentalization-based treatment. The significance of mentalizing capacity as a predictor or mediator of change is however still uncertain due to a scarcity of research. The Mentalization Breakdown Interview (MBI) was developed as a time saving tool for studying psychotherapy processes and outcome in borderline pathology. This study aimed to investigate the convergent validity of reflective functioning (RF) ratings based on the MBI (MBI-RF) by a compari

Mentalization and the left inferior frontal gyrus and insula.

OBJECTIVE: To determine if an interpersonal attribution bias associated with self-perception, the externalizing bias, was related to neural activations during mentalization. METHODS: A functional magnetic resonance imaging task involving verbal appraisals measured neural activations when thinking about oneself and others in 59 adults, including healthy women as well as women with and recovered from anorexia nervosa. Whole-brain regressions correlated brain function during mentalization with the externalizing bias measured using the Internal, Personal, and Situational Attributions Questionnair

Interplay of mentalization, emotion regulation and identity integration among adolescents.

Mentalization, emotion regulation (ER) and identity integration are important transdiagnostic concepts contributing to mental health. Yet, their interconnections are relatively poorly understood. Adolescence is a critical period for developing these skills, as well as for risk of developing mental health problems. In this study, we used network analysis among adolescents with and without mental health problems to examine how mentalization, ER, and identity integration overlap and interplay with each other, and which aspects of them are the most central in mental health problems. A non-clinical

Different epistemic stances for different traumatic experiences: implications for mentalization.

Traumatic experiences may impair reflective functioning (RF), making it difficult for individuals to understand their own and others' mental states. Epistemic trust (ET), which enables evaluating social information as reliable and relevant, may vary in association with RF. In this study, we explored the implications of different ET stances (i.e., trust, mistrust, and credulity) in the relation between different childhood traumatic experiences (i.e., emotional abuse, physical abuse, sexual abuse, emotional neglect, and physical neglect) and different types of RF impairments (uncertainty and cer
