Pulmonary metastasectomy: role of pulmonary metastasectomy and type of surgery.

Patients with untreated metastatic disease have a less than 5 % to 10 % 5-year survival, and for the patient who has metastatic disease isolated to the lungs, pulmonary metastasectomy remains the best hope for cure. Pulmonary metastasectomy has been performed for decades. However, despite hundreds of studies spanning several decades, randomized control data in support of pulmonary metastasectomy is still lacking, and the evidence upon which we base this commonly accepted surgical practice is for the most part weak. While well-accepted surgical selection criteria exist, controversies related to

Lymphadenectomy in metastasectomy.

Although surgery for pulmonary metastases does not benefit a significant number of patients, PM should continue to be offered to patients whose primary tumor is controlled and who have acceptable operative risks. For a survival benefit to be achieved, all extrathoracic and pulmonary metastases must be amenable to complete surgical resection. We have shown that the presence of metastatically involved lymph nodes discovered during PM adversely effects survival in patients undergoing curative PM. We therefore continue to recommend complete mediastinal lymphadenectomy at the time of PM to define t

Disparities in Pulmonary Metastasectomy.

Pulmonary metastasectomy can significantly improve survival in carefully selected patients with metastatic disease, but disparities in access exist based on race, ethnicity, socioeconomic status, and insurance coverage. Factors influencing the likelihood of undergoing the procedure include age, hospital characteristics, and socioeconomic conditions. Implicit biases among health care providers and systemic barriers contribute to these disparities. To address these issues, it is crucial to increase awareness of biases, establish standardized criteria for surgical selection, and improve access to

Pulmonary Metastasectomy for Melanoma.

Early-stage melanoma has a favorable prognosis with a 94% 5-year survival rate, but survival declines significantly with metastasis. Traditional treatments like metastasectomy have evolved due to advancements in targeted therapies and immune checkpoint inhibitors, improving 5-year survival rates from 18% in 2009 to 52% in 2018. The lung is the most common site for metastasis, and the criteria for pulmonary metastasectomy have changed, allowing for surgical intervention even with extrapulmonary metastasis if complete resection is possible. This highlights the role of thoracic surgeons in select

Preoperative imaging for metastasectomy.

For most solid neoplasms, medical imaging is a vital component of tumor staging and surveillance. Imaging strategies vary according to the type and grade of primary neoplasm, tumor stage at diagnosis, tumor markers, previous therapies, and patient symptoms. In this article, we address imaging of individual organs (lung, liver, adrenals) and outline imaging strategies for specific types of neoplasms.

Pulmonary Metastasectomy in Sarcoma.

Sarcoma has a predilection to metastasize to the lungs, with pulmonary metastases occurring in up to half of patients with this disease. Resection of metastatic disease is a commonly utilized tool in the management of these patients due to limitations in systemic therapies. Prospective data are absent and recommendations for metastasectomy in the setting of sarcoma are derived from retrospective series. We describe factors associated with survival that should be evaluated in considering surgical candidacy. Approaches to resection along with management of recurrent pulmonary disease are also di

Metastasectomy in cutaneous melanoma.

Metastasectomy remains the only treatment in malignant melanoma to offer complete pathologic response within a few days of in-hospital stay. It has been historically associated with the highest survival rates in the literature reported for patients of this stage. However, only a minority of patients are amenable to curative resection of distant metastatic disease. This patient group exhibit slow growing oligometastases as indicated by: a. Long disease free interval after treatment of their primary tumours and b. An exhaustive preoperative work up with the use of PET/CT and MRI scans. Only comp

Prognostic factors in pulmonary metastasectomy and efficacy of repeat pulmonary metastasectomy from colorectal cancer.

BACKGROUND: The rate of pulmonary metastasectomy from colorectal cancer (CRC) has increased with recent advances in chemotherapy, diagnostic techniques, and surgical procedures. The purpose of this study was to investigate the prognostic factors for response to pulmonary metastasectomy and the efficacy of repeat pulmonary metastasectomy. METHODS: This study was a retrospective, single-institution study of 126 CRC patients who underwent pulmonary metastasectomy between 2000 and 2019 at the Gifu University Hospital. RESULTS: The 3- and 5-year survival rates were 84.9% and 60.8%, respectively.

[Pulmonary metastasectomy: indication and technique].

Distant metastases of solid tumours are most frequently located in the lung. Most patients with lung metastases suffer from multiple pulmonary lesions or metastases in other organs, which makes these patients unsuitable for surgical treatment. However, several studies suggest a survival benefit if complete resection of all pulmonary metastases is possible. In some patients pulmonary metastasectomy may even be the only curative treatment option. If pulmonary metastases are suspected contrast-enhanced computed tomography is the diagnostic procedure of first choice. Generally accepted rules for i

Pulmonary metastasectomy by median sternotomy.

The results of pulmonary metastasectomy by a median sternotomy approach were retrospectively studied in 38 patients. This approach enabled the exploration of both thoracic cavities and, subsequently, the detection of other metastases. Such occult metastases were present in 11 of the 38 patients, four patients had unsuspected metastases at the contralateral side. There was no postoperative mortality, the morbidity associated with the median sternotomy and bilateral exploration was low. Pneumothorax occurred in three patients, pulmonary infection in four patients and a superficial wound infectio

Metastasectomy in Head and Neck Cancers.

Pulmonary metastases occur relatively commonly in head and neck cancer patients. Careful evaluation to establish whether a lung nodule in head and neck cancer patients is a metastasis or a separate primary lung cancer is critical in providing optimal care. Surgical resection of pulmonary metastases is appropriate in select patients and is associated with reasonable long-term survival.

The role of extended pulmonary metastasectomy.

BACKGROUND: The role of extended pulmonary resection for lung metastases is still unclear, and little information is available in the literature. This study was performed to analyze the outcomes and prognostic factors of patients who underwent extended resections for pulmonary metastases. METHODS: From 1998 to 2013, 1027 patients underwent lung metastasectomy procedures. Twenty-nine patients had extended pulmonary resections: three resections of the chest wall, one azygos, one diaphragm, four vascular resections/reconstructions, six sleeve resections, and 14 pneumonectomies. RESULTS: Extende

Pulmonary Metastasectomy in Renal Cell Carcinoma.

Recent advances in immunotherapy and targeted therapy have resulted in survival rates as high as 90% at 1 year in metastatic renal cell carcinoma patients; however, sustained response and ultimate cure is rarely achieved with systemic therapy alone (complete response rates remain <5%), and progression of disease at distant sites is common. Pulmonary metastasectomy is recommended as a component of multimodal management in patients with favorable-risk or intermediate-risk classification and can be associated with excellent survival if complete resection is obtained.

Reoperative Pulmonary Metastasectomy: Outcomes and Indications.

Recurrent pulmonary metastasis after initial pulmonary metastasectomy is common and poses a clinical dilemma. Reoperative pulmonary metastasectomy is an option that in select patients has been shown to be safe and technically feasible resulting in encouraging survival results from non-randomized retrospective studies. As with metastasectomy in general, there is ongoing debate if observed benefit of repeat pulmonary resection is a result of surgery or a reflection of selection bias. This review summarizes available evidence regarding reoperative pulmonary metastasectomy, discusses selection bia

Bilateral pulmonary metastasectomy through right thoracotomy.

Bilateral thoracotomies spaced at least 4 weeks apart are often required for the resection of bilateral pulmonary metastases. The anatomic distribution of the metastatic disease may rarely permit excision of bilateral pulmonary deposits through one thoracotomy incision. We demonstrate a successful bilateral pulmonary metastasectomy in the right upper lobe, right middle lobe, and left upper lobe through a right thoracotomy in a 40-year-old man with a past history of rectal adenocarcinoma and of left open pulmonary metastasectomy 1 year previously. The left upper lobe was approached by opening t

Metastasectomy for stage IV colorectal cancer.

Metastatic colorectal cancer traditionally has been considered incurable. Over the past 3 decades, however, resection of low-volume hepatic disease has been recognized as beneficial in some cases. More recently, resection of isolated pulmonary metastases has been shown to offer long-term survival in carefully selected patients. Resection of metastases to more unusual sites (ovary, brain, peritoneal cavity) is more controversial; nevertheless, retrospective data suggest that a few patients may be cured with resection of these tumors. In this article, we review the history and current status of

Pulmonary metastasectomy for pediatric solid tumors.

Most pediatric thoracic malignancy is pulmonary disease secondary to solid tumors of childhood. The management of isolated pulmonary metastases in adulthood is well documented. Little has been published to document the long-term outcome of pulmonary metastasectomy in childhood. A retrospective study was undertaken to assess the results of surgery for isolated pulmonary metastases. Twenty children underwent surgery over 12 years (mean follow-up 8 years). Five had Wilms' tumor (mean age 51 months), eight had osteogenic sarcoma (mean age 141 months), three had rhabdomyosarcoma (mean age 92 months
