Arthrodesis of the first metatarsophalangeal joint (MTPJ) is used primarily for end-stage hallux rigidus whereby pain, crepitus, and limitation of motion is noted at the joint. Arthrodesis at the first MTPJ also has it uses as a primary procedure for rheumatoid arthritis when severe deformity is present, as well as for salvage procedures for failed joint arthroplasties with or without implant, fractures with intra-articular extension, avascular necrosis, and infection management. A first MTPJ arthrodesis should provide stable fixation, attain suitable positioning for a reasonable gait, maintai

First MTP joint arthrodesis continues to be a time-honored, effective, and valuable procedure as a primary or secondary surgery for various pathologies afflicting the first ray segment. Though commonly thought of as a salvage procedure, it has proven beneficial in the management of primary hallux limitus and rigidus, geriatric hallux valgus deformity, severe arthritis of any etiology, and conditions in which joint instability or deformity are not readily correctable by more traditional approaches. Since its initial description in the 1800s, the procedure has continued to be popular among ortho

Koehler-Freiberg's disease is found to most commonly manifest itself in the metatarsal head of the second ray. Occasionally it is observed to occur in the metatarsal head of the third and less commonly it is seen to appear in the other rays of the foot. An investigation into probable causes of the disorder calls for consideration of the biomechanical conditions in the region of the metatarsophalangeal joints. A review of the literature pertaining to this matter has failed to reveal any detailed information in this respect and therefore a series of experiments have been conducted involving dete

Joint replacement of the hallux metatarsophalangeal joint has not enjoyed the same success as hip and knee arthroplasties. Silastic implants have achieved a high patient satisfactory rate but have caused many complications, including silicone synovitis and lymph node inflammation. Metal and polyethylene hemiarthroplasties and total toe replacements seem to be more promising although results are preliminary. Problems with these implants seem to be related to soft tissue instability of the joint; patients who have hallux rigidus have more success than patients who have hallux valgus or rheumatoi

The first metatarsophalangeal (MTP1) joint fusion is a very useful procedure in forefoot surgery and is still the gold standard for the treatment of severe and painful hallux rigidus. Normal walking and running are possible after MTP1 fusion, the first ray mobility being essentially in the interphalangeal (IP) joint with a compensatory hypermobility in dorsal flexion. Percutaneous MTP1 fusion is a simple procedure providing comparable results to fusions performed with open techniques. Postoperative cares are simplified with an immediate full weight bearing on rigid flat shoes and quick return

UNLABELLED: The aim of this work is to describe a percutaneous technique of first metatarsophalangeal (MTP1) joint fusion and to assess its preliminary results. Thirty-two percutaneous MTP1 joint arthrodeses were analysed in a prospective continuous series including 30 patients of mean age 66 years old. The indications for arthrodesis of the MTP1 joint were symptomatic hallux rigidus or hallux rigido-valgus in most of the cases. All patients underwent the same percutaneous procedure, as a one-day surgery for 26 cases. Clinical results were assessed using the functional AOFAS forefoot scoring s

Arthroscopic treatments for the first metatarsophalangeal joint have not spread fully owing to a number of factors. First, in the common arthroscopic view, the joint is observed from above, which makes the structures difficult to grasp and the dorsal aspect of the joint difficult to visualize and treat. Second, techniques for widening the view have not yet been reported. Third, the portals necessary for observation and treatment with hallux sesamoids have not yet been established. Finally, techniques for detecting the sesamoids have not been previously reported. We have developed a method for

Arthrodesis of the first metatarsophalangeal joint is indicated in patients with symptomatic arthrodesis or advanced hallux valgus deformities that are unresponsive to nonsurgical treatment. Several fixation techniques have been described, including intefragmentary compression screws and/or dorsal plate fixation. Using these modern fixation techniques, the rate of fusion is between 94% to 98%, with high patient satisfaction. Appropriate positioning of the fusion is important for satisfactory outcome.

The two most common disorders of the first metatarsophalangeal (MTP) joint are hallux valgus and hallux rigidus. The hallux valgus deformity has been the subject of numerous clinical studies in the past decade. This information has enabled the creation of an algorithm to assist the clinician in evaluating the patient with hallux valgus and selecting the appropriate surgical procedure. The technical aspects of various operative procedures and the most common complications are reviewed. The other major disorder of the first MTP joint is arthrosis, which results in hallux rigidus. As the arthrosi

A double-stemmed metatarsophalangeal joint silicone implant has been designed to act as a flexible weightbearing spacer for a number of conditions that affect the metatarsophalangeal joint. The preliminary results show that surgical treatment is feasible for conditions that were previously only marginally successfully treatable. With the exception of lesser metatarsal osteotomies, the prognosis for metatarsophalangeal joint procedures has always been poor. This implant spacer warrants an expanded evaluation for the treatment of lesser metatarsal maladies. Longer-term results are necessary to e

INTRODUCTION: Arthrodesis of the first metatarsophalangeal joint is the most reliable surgical option, with a low complication rate, for hallux rigidus from end-stage osteoarthritis.
STEP 1 SURGICAL APPROACH: Make a medial approach, following the mid-axis of the joint.
STEP 2 JOINT PREPARATION: Using a cup-cone configuration provides excellent bone exposure, construct stability, and metatarsophalangeal joint congruity.
STEP 3 POSITIONING OF ARTHRODESIS: Fix the toe in 5° to 10° of valgus and elevated 5 mm from the floor to achieve desired dorsiflexion.
STEP 4 APPLICATION OF IMPLANTS: Achie

Arthrodesis of the first metatarsophalangeal joint is a useful technique for the surgical treatment of hallux rigidus, complicated hallux valgus and rheumatoid forefoot deformity, and for revisions following resection arthroplasty or explantation of a prosthesis combined with interposition of a tricortical bone graft. In primary operations, good or excellent results can be achieved in 80-90% of patients. The articular surface and any exostoses or osteophytes are first resected aiming for a 15-30 degrees extension of the metatarsophalangeal angle and a 5-15 degrees valgus angle. Pseudarthroses

Arthrodesis has emerged as the primary salvage procedure for severe osteoarthritis of the first metatarsophalangeal (MTP) joint. Forty-three patients underwent arthrodesis of the first MTP joint with stabilization provided by either 2 crossed lag-screws or a dorsal plate and screws. First MTP joint arthrodesis was the primary procedure for 46 of the 54 treated feet. Joint surfaces were shaped into a ball-and-socket configuration to augment joint surface contact and facilitate alignment for arthrodesis. Postoperative care involved using a compressive bandage, a surgical shoe, and a cane, crutch

Pain in the region of the lesser metatarsophalangeal joints (often termed metatarsalgia) is a common complaint. It can be due to a variety of causes, and accurate diagnosis is essential for effective treatment. Understanding the anatomy and functions of the extrinsic and intrinsic musculature and the plantar plate, ligaments, and fat pad is important in evaluating metatarsophalangeal joint disorders. Claw toe is a hyperextension deformity of the metatarsophalangeal joint in combination with a hammer toe. Pathologic changes involving an isolated metatarsophalangeal joint may be due to monarticu

The kinematics of both the first metatarsophalangeal joint and the articulation of the hallux sesamoid bones with the metatarsal head were investigated with fifteen fresh-frozen below-the-knee amputation specimens using a radiographic technique. Six feet were of normal structural anatomy, six displayed hallux valgus, and three had hallux rigidus. Normal specimens demonstrated an average total range of motion in the sagittal plane of 111 degrees, with about 76 degrees of dorsiflexion and 34 degrees of plantar flexion. The abnormal specimens revealed a decreased total arc of motion, with a limit

Metatarsophalangeal (MTP) instability is a common cause of metatarsalgia of the lesser toes. Instability often presents as pain in the plantar forefoot, progressing to coronal and transverse plane malalignment. MTP joint dislocation can develop in some cases. The plantar plate plays an integral role in MTP joint stabilization, and injury to the plantar plate is the main contributor to the instability noted at the MTP joint.
