Superficial temporal artery-middle cerebral artery bypass for ischemic atherosclerotic middle cerebral artery disease.

Most recent studies on the effectiveness of cerebral revascularization have focused on the treatment of atherosclerotic internal carotid artery occlusive disease. The goal of the present study was to assess neurological function in 11 severe atherosclerotic middle cerebral artery (MCA) disease patients with transient ischemic attacks (TIAs) and hemodynamic compromise and determine the efficacy of superficial temporal artery-middle cerebral artery (STA-MCA) bypass. There were eight patients with MCA occlusion and three with severe MCA stenosis. After the bypass procedure, all 11 patients experi

Duplicated middle cerebral artery.

Duplicated middle cerebral artery (DMCA) is an anomalous vessel arising from the internal carotid artery. The incidence DMCA is relatively law, and an association between this anomaly and cerebral aneurysms has been documented. There is a controversy whether DMCA may have perforating arteries. This is an important fact to consider in aneurysm surgery. We report the case of a 34-year-old black woman who suffered a subarachnoid hemorrhage and the angiography a left DMCA, and an aneurysm in an inferior branch of the main MCA. The DMCA and the MCA had perforating arteries. The aneurysm was clipped

Middle cerebral artery dissection.

A 12-year-old girl had minor head trauma, with resultant mild headache, one day prior to onset of an apparently generalized tonic clonic seizure, right hemiparesis and dysphasia. A cranial CT scan showed evidence of a left middle cerebral artery infarct. Despite ventilation, fluid restriction, mannitol, thiopental infusion and intracranial pressure monitoring she deteriorated and died. Autopsy demonstrated dissection of the left middle cerebral artery with an intact internal carotid artery and no evidence of vasculitis. Middle cerebral artery dissection in children is very rare. Most reported

Unruptured middle cerebral artery aneurysm associated with a duplicated middle cerebral artery and a dolichoectasic anterior cerebral artery.

Duplicated middle cerebral artery (DMCA) has been reported in 0.7%-2.9% of autopsy cases, and sme reports have described aneurysms in the MCA. Both ruptured and unruptured aneurysms in the MCA have been reported, often requiring surgical intervention. During surgery for aneurysm in DMCA, care must be taken to avoid damaging the MCA, which supplies the territory of the temporopolar artery and the anterior and/or middle temporal artery. Regarding dolichoectasia of intracranial arteries, a relationship between dolichoectasia and ischemic brain lesions, especially lacunar infarctions, has been not

Ruptured aneurysm at a duplicated middle cerebral artery with accessory middle cerebral artery.

Although many cerebral vascular anomalies are widely recognized, others are less well known or unclassified. Accessory middle cerebral artery (MCA) and duplicated MCA are among uncommon anomalies. We present a very rare case of subarachnoid haemorrhage due to rupture of a saccular aneurysm arising from a duplicated middle cerebral artery which was associated with an accessory middle cerebral artery.

Posterior cerebral artery infarction from middle cerebral artery infarction.

BACKGROUND: While it is known that posterior cerebral artery (PCA) infarction may simulate middle cerebral artery (MCA) infarction, the frequency and localization of this occurrence are unknown. OBJECTIVE: To determine the frequency of PCA infarction mimicking MCA infarction and the territory of the PCA most commonly involved in this simulation. DESIGN: We studied 202 patients with isolated infarction in the PCA admitted to our stroke center to determine the frequency of PCA infarction simulating MCA infarction, the involved PCA territory, and the patterns of clinical presentation. RESULTS:

Superficial temporal artery-middle cerebral artery bypass for the treatment of complex middle cerebral artery aneurysms.

OBJECTIVES: Direct microsurgical clipping for complex middle cerebral artery (MCA) aneurysms may require a long ischemic time. Sacrifice of the parent artery with trapping or endovascular coiling also may lead to ischemic stroke. We described our institutional experience with the treatment of complex MCA aneurysms using extracranial-intracranial (EC-IC) (superficial temporal artery [STA]-MCA) bypass. MATERIALS AND METHODS: We retrospectively reviewed patients who had treatment of IC aneurysms with the assistance of STA-MCA bypass from July 2002 to December 2016. Six patients with complex MCA

The Protective Effect of Middle Cerebral Artery Calcification on Symptomatic Middle Cerebral Artery Infarction.

BACKGROUND AND PURPOSE: The presence of intracranial artery calcification is associated with an increased risk for stroke. However, calcified atherosclerotic plaques are also known to be less vulnerable to rupture. Given this discrepancy, we investigated whether the vulnerability of intracranial arterial atherosclerosis differed based on the presence or absence of calcification. METHODS: We considered consecutive patients with acute stroke in the unilateral middle cerebral artery (MCA) territory. Patients with any stenotic MCAs were included in this study. Symptomatic MCA was defined as the o

Proximal posterior cerebral artery occlusion simulating middle cerebral artery occlusion.

We describe 12 cases of acute stroke in which clinical features of proximal posterior cerebral artery occlusion simulated the clinical syndrome of middle cerebral artery occlusion. The majority of patients developed contralateral hemiparesis, homonymous hemianopia, hemispatial neglect, and sensory loss or sensory inattention. All 8 patients with dominant hemisphere lesions were aphasic. Accurate diagnosis in each case was achieved only after a head CT, showing occipital lobe, thalamic, and inferomesial temporal lobe infarction. "Cortical" signs are probably explained by thalamic involvement. R

[Traumatic middle cerebral artery occlusion].

A 16-year-old boy developed motor aphasia and right hemiplegia after head trauma caused during Rugby football club activity. About 2 hours after trauma, these neurological deficits improved dramatically. CT scan on admission revealed no abnormal finding. But, on the next day, CT scan revealed an irregular low density area at the left basal ganglia. Cerebral angiography showed a slight narrowing at the left carotid siphon, severe stenosis at M1 portion of the left middle cerebral artery with vasospasm of its distal branches and occlusion of the left anterior temporal artery. A ruptured internal carotid artery aneurysm located at the origin of the duplicated middle cerebral artery associated with accessory middle cerebral artery and middle cerebral artery aplasia.

BACKGROUND: Intracranial vascular anomalies involving the middle cerebral artery (MCA) are relatively rare, as such knowledge will be helpful for planning the optimal surgical procedures. CASE DESCRIPTION: We herein present the first case of a ruptured internal carotid artery aneurysm arising at the origin of the hypoplastic duplicated MCA associated with accessory MCA and main MCA aplasia, which was revealed by angiograms and intraoperative findings. CONCLUSION: In practice, this case highlights the urgent need to preoperatively recognize such vascular anomalies as well as understand the co

Anastomosis between accessory middle cerebral artery and middle cerebral artery diagnosed by magnetic resonance angiography.

The accessory middle cerebral artery (MCA) is a common variation of the MCA that arises from the anterior cerebral artery. We report a patient with anastomosis of the accessory MCA with the main MCA, an extremely rare variant that we diagnosed by magnetic resonance (MR) angiography. Both partial maximum-intensity-projection and partial volume-rendering MR angiographic images obtained at 3 T are useful to identify such rare vascular variation.

Influence of superficial temporal artery to middle cerebral artery bypass on cerebral blood flow in dogs with middle cerebral artery occlusion.

Eight dogs had a superficial temporal artery to middle cerebral artery (STA-MCA) anastomosis, followed immediately by ligation of the ipsilateral middle cerebral artery. Subsequently, utilizing the radioactive microsphere technique, regional cerebral blood flow determinations were made both before and after ligation of the superficial temporal artery. A significant reduction in blood flow of 20-35% was noted in both cerebral hemispheres, caudate and thalamic nuclei, brain stem and cerebellum following occlusion of the bypass. It is our impression that this generalized reduction in flow is due

Accessory middle cerebral artery associated with an accessory anterior cerebral artery.

PURPOSE: Although both accessory middle cerebral artery (MCA) and accessory anterior cerebral artery (ACA) are not rare anatomical variations, their combination is rare and there are only a limited number of reports of such combinations. METHODS: We report a case of accessory MCA associated with an accessory ACA diagnosed by magnetic resonance angiography (MRA). RESULTS: A 39-year-old woman visited our hospital because of right facial discomfort. She underwent 1.5-Tesla magnetic resonance imaging and MRA. MRA showed a left accessory MCA originating from the left ACA A1 segment. MRA also show

Unruptured aneurysm at bifurcation of super short middle cerebral artery mimicking duplicated middle cerebral artery aneurysm.

BACKGROUND: Duplicated middle cerebral artery (DMCA) is a normal variation of the middle cerebral artery (MCA), and distinguishing DMCA from other arteries based only on the angiographical findings is sometimes difficult. CASE DESCRIPTION: Preoperative angiography of a 60-year-old woman revealed two MCAs originating near the top of the right internal carotid artery and a 6-mm-sized aneurysm located between these two MCAs. The ipsilateral A1 segment of the anterior cerebral artery was not visualized due to hypoplasia. The patient was diagnosed with an unruptured aneurysm at the bifurcation bet

Distal anterior cerebral artery mirror aneurysms and middle cerebral artery aneurysms.

A 52-year-old woman presented with bilateral distal anterior cerebral artery (ACA) mirror aneurysms, in addition to two right middle cerebral artery (MCA) aneurysms. The left distal ACA and right MCA aneurysms were clipped through right interhemispheric and right pterional craniotomies. The right distal ACA aneurysm was thrombosed. Distal ACA aneurysms tend to be multiple, but mirror aneurysms are rare, especially in the absence of congenital vascular anomaly. Single stage unilateral craniotomy is recommended for surgical treatment.
