A 15 year experience using wide flange tube for long term ventilation is reported with emphasis on complications and is compared in some respects to a small series of Paparella tube implants. At one time or another 70% of the wide flange tube implants were complicated, as were 100% of the Paparella tubes. The incidence of infection over a long period is high, at times only manageable with removal of the tube. However, longevity when tubes are removed for uncontrolled complication is still almost 3 years and as long as 12 years. The average longevity for electively removed tubes exceeds 4 years

OBJECTIVE: Long-term tympanostomy tubes are associated with a significant rate of complications, particularly persistent perforation. We describe the outcomes of 57 subannular ventilation tube insertions in 45 consecutive patients.
DESIGN: Retrospective case series.

The results of a retrospective study of the effect and outcome of middle ear ventilation by Goode's tubes are presented. 83 ears from 50 patients were analyzed both as a group and in age-related sub-groups over a mean follow-up period of 1.83 years. The mean period of ventilation by Goode's tubes before removal or extrusion was 18.4 months. The tubes became infected in 70.4% and were spontaneously extruded in 44.9% of patients. Patients with chronic otitis media that did not respond to conventional treatment were studied for the types and incidence of complications from the use of large-bore tubes inserted into their ears. The most common complication was otorrhea (41%, 50 of 123 ears). Permanent perforation occurred in 25% (17/68 ears). Use of large-bore tubes proved effective and continues to be indicated in certain conditions of chronic otitis media that are refractory to other forms of treatment.

The middle ear (ME) steady state gas composition resembles that of mixed venous blood. We changed arterial and venous blood gases by artificially ventilating anesthetized guinea pigs and measured simultaneous ME gas changes during spontaneous breathing, hyperventilation, and hypoventilation. During hyperventilation, PaCO2 and PvCO2 (a = arterial, v = venous) decreased from 46.0 and 53.0 mm Hg to 17.9 and 37.5 mm Hg, respectively, while PaO2 and PvO2 (85.6 and 38.2 mm Hg) did not change. METHODS: A prospective longitudinal cohort study included all children under 2 years of age with a tracheostomy placed at a tertiary care children's hospital between 2015 and 2020 that obtained at least one tympanometry exam. The tympanic membrane was perforated (n = 19) or a tube was inserted into the middle ear through the wall of the tympanic bulla (n = 6) immediately after blockage of the endolymphatic duct in guinea pigs.
