Severe vaginal evisceration at a military treatment facility.

Vaginal evisceration is a rare extrusion of pelvic and/or abdominal viscera through a defect or rupture of the vaginal wall, and it is a true surgical emergency. With less than 100 cases reported in the literature, we present a case of an elderly woman in her 80s who was brought to a military treatment facility with vaginal evisceration after sustaining a ground-level fall. She was activated as a level 1 trauma by the emergency department, which led to efficient evaluation by the trauma team and subsequent emergent surgery to reduce the evisceration and repair the defect. She went on to recove

Infection prevention and control in deployed military medical treatment facilities.

Infections have complicated the care of combat casualties throughout history and were at one time considered part of the natural history of combat trauma. Personnel who survived to reach medical care were expected to develop and possibly succumb to infections during their care in military hospitals. Initial care of war wounds continues to focus on rapid surgical care with debridement and irrigation, aimed at preventing local infection and sepsis with bacteria from the environment (e.g., clostridial gangrene) or the casualty's own flora. Over the past 150 years, with the revelation that pathoge

Pediatric Mortality in Deployed United States Military Treatment Facilities: 2001-2022.

OBJECTIVE: Pediatric mortality from conflict-related injuries exceeds that of adults. Understanding pediatric injury patterns and outcomes is critical for guiding resource allocation and training. This study characterizes pediatric injuries in conflict zones and examines relationships between injury patterns, resource allocation, and survival. METHODS: A retrospective cohort analysis of the Department of Defense Trauma Registry from 2001 to 2022 was conducted for children younger than 18 years treated at deployed military treatment facilities in conflict zones with documented injuries and dis

Standardizing Early Ambulation Practices to Improve Mobility across Military Treatment Facilities.

Background: Hospital-acquired immobility contributes to functional decline, prolonged hospital stays, and preventable complications. Within the Military Health System (MHS), inconsistent mobility practices and lack of a standardized protocol hinder effective care. Purpose: This evidence-based practice project aimed to implement and evaluate the Johns Hopkins Activity and Mobility Promotion program across adult inpatient units at Tripler Army Medical Center to standardize mobility, improve outcomes, and enhance interdisciplinary collaboration. Methods: Using a four-phase approach-planning, educ

Examination of Gallbladders at Military Treatment Facilities: Is Histologic Analysis Necessary?

INTRODUCTION: Gallbladder specimens from cholecystectomy procedures are a common specimen evaluated by military pathologists. These are often removed for inflammatory causes. Previous studies show that the incidence of gallbladder carcinoma (GBC) is around 3%. Incidentally identified GBC is even less common at 0.36%. Incidentally found GBCs are of little clinical consequence as most are treated by cholecystectomy alone. We hypothesize that a selective approach to histologic evaluation of gallbladders could save time for pathologists to focus on more complex cancer cases and save money for the

Analysis of 3,294 cases of upper gastrointestinal bleeding in military medical facilities.

OBJECTIVES: Upper gastrointestinal bleeding (UGIB) remains a commonly encountered medical emergency with significant morbidity and mortality. Most large studies detailing the specific incidence, demographic, and mortality data were performed more than a decade ago. This study analyzes 3,294 cases of UGIB from 139 military medical treatment facilities over a 12-month period. METHODS: A retrospective chart review of Department of Defense military medical treatment facilities for UGIB was performed from October 1990 through September 1991. Core data such as demographic information were analyzed,

Lung Cancer Screening at a Military Treatment Facility: A Retrospective Review.

INTRODUCTION: Lung cancer is the leading cause of cancer death among men and women, accounting for more fatalities than colon, breast, and prostate cancers combined. Smoking causes about 85% of all lung cancers in the United States and is the single greatest risk factor. In 2013, the US Preventive Services Task Force (USPSTF) published initial guidelines for low-dose computed tomography lung cancer screening (LCS) among patients 55-80 years old, with a 30-pack-year history, who are current smokers or who quit within the previous 15 years. Smoking prevalence is higher among military personnel c

Persons tested for SAR-CoV-2 at a military treatment facility in Hawaii.

Health inequalities based on race are well-documented, and the COVID-19 pandemic is no exception. Despite the advances in modern medicine, access to health care remains a primary determinant of health outcomes, especially for communities of color. African-Americans and other minorities are disproportionately at risk for infection with COVID-19, but this problem extends beyond access alone. This study sought to identify trends in race-based disparities in COVID-19 in the setting of universal access to care. Tripler Army Medical Center (TAMC) is a Department of Defense Military Treatment Facilit

Pneumonia outbreak associated with group a Streptococcus species at a military training facility.

BACKGROUND: Although group A streptococci (GAS) infections are a major cause of morbidity and mortality, outbreaks of associated pneumonia are rare. We report an outbreak of GAS pneumonia that occurred at a US military training camp. METHODS: Standard epidemiologic and laboratory procedures were used to characterize the outbreak and causative organism(s). A case-control study and determination of the prevalence of GAS infection among camp personnel were also performed. RESULTS: A total of 162 of 4500 Marine Corps personnel were hospitalized for respiratory symptoms during the period of 1 Nov

Impact of beneficiary status on bariatric surgery outcomes in a military treatment facility.

BACKGROUND: Retirees and dependents represent demographically and clinically distinct beneficiary groups within the Military Health System (MHS). We assessed whether beneficiary status influences perioperative outcomes, healthcare utilization, and weight loss after bariatric surgery. METHODS: We conducted a retrospective cohort study of adults undergoing sleeve gastrectomy or Roux-en-Y gastric bypass from 01/01/2022 to 12/31/2024 at a single military treatment facility. Outcomes were stratified by beneficiary status (retiree versus dependent). Propensity scores incorporating demographic, clin

Quality and Patient Safety Review Processes in Military Treatment Facilities and Operational Clinical Services.

RAND researchers assessed the quality and patient safety review processes for care delivered in military treatment facilities (MTFs) and via operational clinical services (OCSs). MTFs are fixed or permanent medical facilities. OCSs are often provided in austere or operational environments that can be temporary, mobile, or permanent. OCS settings vary widely in their infrastructure and capabilities depending on the operational needs, resources, and location. The authors conducted an assessment of seven quality and patient safety review processes: credentialing and privileging; quality assurance

Surgical Management of Endometrial Intraepithelial Neoplasia at Military Treatment Facilities: A Multicenter Retrospective Study.

INTRODUCTION: Endometrial intraepithelial neoplasia (EIN), also known as atypical endometrial hyperplasia (AEH), is a precursor lesion of endometrial carcinoma (EC). In endometrial cancer patients, lymph node assessment with biopsy during hysterectomy is part of surgical staging. However, routine lymph node assessment for EIN is inconsistently utilized. This study aims to investigate the surgical management of EIN in the military to inform best-practice guidelines tailored for the Military Health System to avoid delays in care, manage cost, ensure military readiness and optimize clinical outco

Risk determination for patients with direct access to physical therapy in military health care facilities.

STUDY DESIGN: Nonexperimental, retrospective, descriptive design. OBJECTIVES: This study was designed to ascertain whether direct access to physical therapy placed military health care beneficiaries at risk for adverse events related to their management. BACKGROUND: Military health care beneficiaries have the option at most US military hospitals and clinics to first enter the health care system through physical therapy by direct access, without referral from another privileged health care provider. This level of autonomous practice incurs broad responsibilities and raises concern regarding t

Characterization of Humanitarian Trauma Care by US Military Facilities During Combat Operations in Afghanistan and Iraq.

OBJECTIVE: To characterize humanitarian trauma care delivered by US military treatment facilities (MTFs) in Afghanistan and Iraq during combat operations. BACKGROUND: International Humanitarian Law, which includes the Geneva Conventions, defines protections and standards of treatment to victims of armed conflicts. In 1949, these standards expanded to include injured civilians. In 2001, the Global War on Terror began in Afghanistan and expanded to Iraq in 2003. US MTFs provided care to all military forces, civilians, and enemy prisoners. A thorough understanding of the scope, epidemiology, res

Purchased Behavioral Health Care Received by Military Health System Beneficiaries in Civilian Medical Facilities, 2000-2014.

INTRODUCTION: Behavioral health conditions are a significant concern for the U.S. military and the Military Health System (MHS) because of decreased military readiness and increased health care utilization. Although MHS beneficiaries receive direct care in military treatment facilities, a disproportionate majority of behavioral health treatment is purchased care received in civilian facilities. Yet, limited evidence exists about purchased behavioral health care received by MHS beneficiaries. This longitudinal study (1) estimated the prevalence of purchased behavioral health care and (2) identi
