Military Personnel Who Seek Health and Mental Health Services Outside the Military.

BACKGROUND: Although research conducted within the military has assessed the health and mental health problems of military personnel, little information exists about personnel who seek care outside the military. The purpose of this study is to clarify the personal characteristics, mental health diagnoses, and experiences of active duty U.S. military personnel who sought civilian sector services due to unmet needs for care. MATERIALS AND METHODS: This prospective, multi-method study included 233 clients, based in the United States, Afghanistan, South Korea, and Germany, who obtained care betwe

Predicting mental health problems in military personnel following deployment: The Military Mental Health Risk Screen.

Many military service members experience posttraumatic stress disorder (PTSD) and other mental health conditions following deployment; however, current methods for screening and identifying those at risk for developing mental health disorders are insufficient. The present study used archival, prospectively collected longitudinal data on risk factors to create a brief screening measure that could be used to proactively identify those most likely to develop longer-term PTSD and other mental health symptoms following deployment. Data were available for 512 military personnel, and the sample was r

Association of military life experiences and health indicators among military spouses.

BACKGROUND: The health and well-being of military spouses directly contribute to a robust military force by enabling the spouse to better support the active duty member's career. In order to understand the overall health and well-being of military spouses, we assessed health indicators among military spouses using the Healthy People 2020 framework and examined associations of these health indicators with military experiences and psychosocial factors. METHODS: Using data from the Millennium Cohort Family Study, a U.S. Department of Defense-sponsored survey of 9872 spouses of service members wi

Improving the Health and Readiness of Military Women.

WHY THE DEFENSE HEALTH HORIZONS DID THIS STUDY: Women comprise approximately one-fifth of the total force in the U.S. Military. Gynecologic and reproductive health issues not only affect the health and wellness of individual servicewomen but may also impact the mission of the DoD. Unintended pregnancies can result in adverse maternal and infant outcomes and can negatively impact the careers of military women and mission readiness. Gynecologic conditions such as abnormal uterine bleeding, fibroids, and endometriosis can also limit women's optimal health and performance, and a significant propor

Framework for the evaluation of military health systems.

The organisation of a military health system (MHS) differs from the civilian system due to the role of the armed forces, the unique nature of the supported population and their occupational health requirements. A previously published review of the Military Medical Corps Worldwide Almanac demonstrated the value of a standardised framework for evaluation and comparison of MHSs. This paper proposes such a framework which highlights the unique features of MHSs not covered by health services research of national health systems. These include: national context and summary; organisational structure;

The effect of military deployment on mental health.

Public concern about soldiers' mental health has increased over the last decade. Yet the large literature on the mental health problems of returning soldiers relies primarily on self-reported measures that may suffer from non-response bias, usually refers to older conflicts, and focuses mainly on specific diagnoses such as PTSD. Another challenge is that the differences between soldiers and non-soldiers are not necessarily causal, instead possibly reflecting an underlying propensity towards active military service. Using the objective measures of hospitalizations and the purchase of mental hea

Modernizing Sexual Health and Reproductive Health Education to Optimize Military Readiness.

Sexually transmitted infections (STIs) account for over half of all reportable medical events in the military health system. The military reported a 98% increase in syphilis cases among service members between 2015 and 2023. From 2015 to 2019, the rates of chlamydia and gonorrhea in the Army increased 31% and 49%, respectively. Despite the high morbidity attributed to STIs, there is no standardized, evidence-based sexual and reproductive health education curriculum throughout the military. However, civilian studies on interactive digital interventions (IDIs) as well as interactive computer-bas

Parental Mental Health Conditions and Infant Health Outcomes Among Military Families.

INTRODUCTION: Little is known about the effects of parental mental health burdens during pregnancy on infant health among military families, who are subject to various stressors unique to military life. The present study leveraged infant data from the DoD Birth and Infant Health Research (BIHR) program and self-reported parental survey data from the Millennium Cohort Study (MCS) to examine associations of parental mental health conditions with adverse infant health outcomes. MATERIALS AND METHODS: Subjects included singleton infants captured in BIHR program data, born between July 2001 and De

Environmental and health hazards of military metal pollution.

An increasing body of literature has demonstrated that armed conflicts and military activity may contribute to environmental pollution with metals, although the existing data are inconsistent. Therefore, in this paper, we discuss potential sources of military-related metal emissions, environmental metal contamination, as well as routes of metal exposure and their health hazards in relation to military activities. Emission of metals into the environment upon military activity occurs from weapon residues containing high levels of particles containing lead (Pb; leaded ammunition), copper (Cu; unl

Sarcoidosis in the United States Military Health System.

Rationale: Sarcoidosis is an inflammatory disorder of unclear etiology with historical significance in the U.S. Department of Defense (DoD). Objectives: This study sought to characterize the sarcoidosis population within the DoD Military Health System (MHS). Methods: Adult patients with sarcoidosis were identified in the DoD MHS database from 01-JAN-2004 through 31-DEC-2013. Patients required ≥3 encounters with a sarcoidosis diagnosis and continuous MHS eligibility. Index was defined as date of first sarcoidosis encounter. Comorbidities were assessed within the pre-index and follow-up periods.

Investigating the respiratory health of deployed military personnel.

Recent news media articles have implied a direct relationship between environmental exposures such as burn pits during current deployments and the development of serious and debilitating chronic pulmonary disease. These articles suggest that the military is superficially investigating evidence that establishes a link between deployment and development of chronic lung disease. Anecdotal cases of military personnel with lung disease are detailed to suggest a systemic problem with undiagnosed and untreated pulmonary disease in deployed service members. Despite these contentions, the U.S. Army Med

An Investigation of Mucosal Leishmaniasis in the Military Health System.

Leishmaniasis is a protozoal infection with an increased risk of transmission to those serving in the U.S. Military due to theaters of operation in endemic regions. There has, in recent decades, been robust experience with old-world leishmaniasis in the Military Health System (MHS); however, new-world leishmaniasis, which may result in mucosal leishmaniasis, has been less studied. A total of 88 patients from 2012 to 2022 with diagnosis codes for "mucocutaneous leishmaniasis" or "leishmaniasis, unspecified" were identified in the Military Data Repository and reviewed. Within this cohort, upon m

Blast Overpressures as a Military and Occupational Health Concern.

PURPOSE: This tutorial reviews effects of environmental stressors like blast overpressures and other well-known acoustic contaminants (continuous, intermittent, and impulsive noise) on hearing, tinnitus, vestibular, and balance-related functions. Based on the overall outcome of these effects, detailed consideration is given to the health and well-being of individuals. METHOD: Because hearing loss and tinnitus are consequential in affecting quality of life, novel neuromodulation paradigms are reviewed for their positive abatement and treatment-related effects. Examples of clinical data, resear

Defining Mental Health and Social Health for Public Health Practice in the United States Military.

Clinically diagnosed mental health (MH) disorders accounted for the largest total number of hospital bed days and the second highest total number of medical encounters for U.S. Armed Forces active component members from 2019 to 2023. MH is largely influenced by social health (SH) outcomes and factors which include conditions at the relationship, command, and community levels for the military. While MH and SH definitions have been standardized for the civilian population, the unique aspects of the military environment call for military-focused MH and SH definitions. This commentary reviewed the
