"Forward psychiatry" in the military: its origins and effectiveness.

"Forward psychiatry" was devised in World War I for the treatment of shell shock and today is the standard intervention for combat stress reaction. It relied on three principles: proximity to battle, immediacy, and expectation of recovery, subsequently given the acronym "PIE." Both US and UK forces belatedly reintroduced PIE methods during World War II to return servicemen to active duty and made confident claims for its efficacy. Advanced treatment units also appeared to have minimized psychiatric battle casualties during Korean and Vietnamese Wars. Evaluations of its use by Israeli forces in

The military, psychiatry, and "unfit" soldiers, 1939-1942.

This article examines the psychiatric screening of U.S. soldiers during the Second World War, established by psychiatrist Harry Stack Sullivan (1892-1949), as a key moment in the public application of clinical psychiatry, as well as a turning point in Sullivan's intellectual and professional career. After a brief look at the ideas and expectations Sullivan brought to the screening system, I discuss a major problem of the screening: the mismatch between the medical concept of disease prevention and the realities of the mass screening as a public policy. As a way to highlight this mismatch, I fo

Military traumatic brain injury: a challenge straddling neurology and psychiatry.

Military psychiatry, a new subcategory of psychiatry, has become an invaluable, intangible effect of the war. In this review, we begin by examining related military research, summarizing the related epidemiological data, neuropathology, and the research achievements of diagnosis and treatment technology, and discussing its comorbidity and sequelae. To date, advances in neuroimaging and molecular biology have greatly boosted the studies on military traumatic brain injury (TBI). In particular, in terms of pathophysiological mechanisms, several preclinical studies have identified abnormal protein

Battle for the mind: World War 1 and the birth of military psychiatry.

The 100th anniversary of the outbreak of World War 1 could be viewed as a tempting opportunity to acknowledge the origins of military psychiatry and the start of a journey from psychological ignorance to enlightenment. However, the psychiatric legacy of the war is ambiguous. During World War 1, a new disorder (shellshock) and a new treatment (forward psychiatry) were introduced, but the former should not be thought of as the first recognition of what is now called post-traumatic stress disorder and the latter did not offer the solution to the management of psychiatric casualties, as was subseq

Moral injury in modern warfare: clinical reflections and implications for military psychiatry.

Moral injury has emerged as a significant dimension of psychological suffering among individuals exposed to ethically compromising situations in war. Unlike post-traumatic stress disorder (PTSD), which is primarily organized around fear-based responses to threat, moral injury centers on violations of deeply held moral expectations involving responsibility, agency, and trust. In this clinical reflection, moral injury is approached not as a psychiatric diagnosis, but as a clinical-moral condition arising from difficulty reconciling actions or survival with an internalized moral framework. [What do we really know about how lance-corporal Adolf Hitler was treated by german military psychiatry?].

OBJECTIVE: This paper inquires the hypothesis that Hitler's rise to power was in part due to a hypnotic therapy he had undergone when being treated for hysterical blindness at an army hospital in the town of Pasewalk in October 1918 - as recent contributions have argued. Edmund Forster, his psychiatrist at that time, is supposed to have suggested to Hitler that he would be ordained as Germany's redeemer in times of defeat, thus causing a profound change in his patient's personality. METHODS: Following three lines of argument, this paper examines if such an assumption can be made plausible. Fi

"One more front in the dirty war": The political instrumentalisation of psychiatry and the involvement of mental health professionals in state-sponsored human rights abuses during Argentina's civil-military dictatorship (1976-1983).

This paper examines the instrumentalisation of psychiatric procedures and the complicity of physicians and mental health professionals in the state-sponsored terror of Argentina's civil-military dictatorship (1976-1983). Hitherto, these episodes have remained largely underexplored in broader historiography, as have their implications for modern psychiatric ethics. Drawing on archival records, victim testimonies, and secondary sources, this paper outlines how mental health expertise and psychiatric institutions were implicated in the junta's campaigns to subjugate perceived "subversive" tendenc

Psychiatric considerations in military aerospace medicine.

Military aerospace medicine requires a psychiatric selection and certification process that determines not only the absence of significant mental disorders, but also the presence of positive qualities in the realms of motivation, ability and stability: not all normal people are fit to fly. Other issues of aerospace psychiatry involve maintenance of mental resilience and hardiness during a flying career, aeromedical decisions about when to remove from flight duties and when to return, criteria for waivers for psychiatric conditions, use of medications for treatment of psychiatric symptoms, ques

[Specificities of the EMDR psychotherapeutic approach in the military environment].

In the French armed forces, the treatment of post-traumatic stress disorder is an integral part of military psychiatry, with an expertise that is applied both in the field, close to the wounded, and in France. Eye Movement Desensitization and Reprocessing is a psychotherapy recognized as a first-line treatment for these disorders. Its application to military personnel is proving to be a valuable therapeutic tool, but it can sometimes run up against obstacles to the integration of traumatic memories. These difficulties may be due to the patient's own resistance, to his or her past life history,
