Human Milk Banking.

Human milk banks play an essential role by providing human milk to infants who would otherwise not be able to receive human milk. The largest group of recipients are premature infants who derive very substantial benefits from it. Human milk protects premature infants from necrotizing enterocolitis and from sepsis, two devastating medical conditions. Milk banks collect, screen, store, process, and distribute human milk. Donating women usually nurse their own infants and have a milk supply that exceeds their own infants' needs. Donor women are carefully selected and are screened for HIV-1, HIV-2

New alternatives to holder pasteurization in processing donor milk in human milk banks.

Infectious and toxicological risks are the main potential hazards that operators of Human Milk Banks (HMBs) encounter and must eliminate. HMBs are trying to implement procedures that allow to manage and sanitize human milk without altering significantly its nutritional and biologically protective components, obtaining a product characterized by a valid balance between safety and biological quality. The history of human milk processing is linked to the origins of HMBs themselves. And although other forms of sterilization were used originally, pasteurization soon became the recognized most effec

Macronutrient and bioactive profiles of donor milk differ in commercial vs non-profit milk banks.

OBJECTIVE: Characterize variation of macronutrient and bioactive composition between levels of human milk-derived human milk fortifier (HM-HMF), and between donor human milk (DHM) from commercial vs non-profit banks. STUDY DESIGN: Macronutrients, Immunoglobulin A (IgA), and cortisol levels were measured in 10 lots of each HM-HMF product (20, 24, 26, 28, and 30 (n = 5) kcal/oz), including both "ready to feed" products and fortifiers reconstituted in donor milk. RESULTS: Caloric density, protein, fat, carbohydrate, and total IgA concentration generally increased with the level of HM-HMF labele

High-Temperature Pasteurization Used at Donor Breast Milk Banks Reduces Melatonin Levels in Breast Milk.

Background and Objective: Donor human milk banks are used when breast milk directly from mothers is unavailable or insufficient. Breast milk contains melatonin, which exhibits a 24-hour pattern. Melatonin promotes sleep onset and is barely detected in daytime milk but rises in the evening and peaks early in the morning. Melatonin supports the development of an infant's own circadian rhythm and is important for neurodevelopment. Currently, donor banks pasteurize breast milk using a Holder Pasteurization (HoP) technique where breast milk is treated at a high temperature (+62°) for 30 minutes bef

Composition and Variation of Macronutrients, Immune Proteins, and Human Milk Oligosaccharides in Human Milk From Nonprofit and Commercial Milk Banks.

BACKGROUND: When human milk is unavailable, banked milk is recommended for feeding premature infants. Milk banks use processes to eliminate pathogens; however, variability among methods exists. Research aim: The aim of this study was to compare the macronutrient (protein, carbohydrate, fat, energy), immune-protective protein, and human milk oligosaccharide (HMO) content of human milk from three independent milk banks that use pasteurization (Holder vs. vat techniques) or retort sterilization. METHODS: Randomly acquired human milk samples from three different milk banks ( n = 3 from each bank)

The Fourth Survey on the Activity of Human Milk Banks in Italy.

BACKGROUND: Scientific literature confirms the benefits of mother's own milk (MOM) for both term and preterm infants. The feeding of pathological newborns, in particular the very low birth weight infants (VLBWIs), is dependent on human milk. When MOM is not available, pasteurized donor human milk obtained from a recognized Human Milk Bank (HMB) is the best alternative. RESEARCH AIMS: This survey aims to evaluate the activity of human milk banks (HMBs) in Italy in the years 2023-2024. METHODS: Following the previous three surveys performed in 2012, 2016, and 2022, a fourth survey related to 2

The data and characteristics of the human milk banks in mainland China.

BACKGROUND: Human milk banks (HMB) have been established for over 100 years in North America and Europe. This study aimed to describe and summarize the operation and characteristics of the HMBs in mainland China since the first nonprofit HMB operated in 2013. METHODS: Operation of HMB in mainland China is based on the standards and guidelines of the Human Milk Banking Association of North America and some countries in Europe and was modified to meet the needs and circumstances in China such as donation only in the local HMB by medical staff. We reviewed the descriptive data of these 14 HMBs a

Knowledge, Attitudes, and Perceptions of Medical and Paramedical Students Toward Human Milk Banks and Breast Milk Donation.

Background and objectives Human milk benefits for both mothers and babies are widely acknowledged. Human milk banks (HMBs) are essential in providing newborns who would not otherwise have access to pasteurized and stored human milk. The objective of this research was to investigate the understanding, opinions, and outlooks of medical and paramedical students regarding breast milk donation and the concept of an HMB. Material and methods A total of 398 professional female students specializing in physiotherapy, nursing, and medicine were given an online self-administered questionnaire as part of

Human milk banking services: an umbrella review.

OBJECTIVE: Donor human milk (DHM) is excess breast milk a woman donates to a human milk bank (HMB) for use by another infant. HMB requires operational sustainability to ensure consistent and equitable DHM supply. This review synthesises evidence from systematic reviews on how HMB services have been developed and scaled internationally. Results will inform the co-design of an evidence framework that incorporates governance, quality and sustainability criteria for HM banking. DESIGN: An umbrella review was conducted following the JBI Manual for Evidence Synthesis. CINAHL, MEDLINE, PsycINFO, Emb

Donor and recipient safety in human milk banking.

AIM: Australian Red Cross Lifeblood supplies pasteurised donor human milk (PDHM) to more than 30 partner hospitals across Australia. Preterm infants who receive PDHM are a highly vulnerable population but formal biovigilance programs are rare in human milk banking. Lifeblood Milk performs ongoing surveillance for both donor and recipient adverse events. This study aimed to formally review adverse events reported to Lifeblood Milk since 2018. METHODS: Milk donor infectious diseases testing outcomes and donor adverse events (DAEs) are prospectively recorded at Lifeblood. Infant recipient advers

Characteristics of the first human milk bank in Taiwan.

BACKGROUND: The benefits of feeding human milk to infants, even in prematurity, have been well documented. Well-organized donor milk processing has made the milk bank a good source of nutrition for premature or sick infants if their own mother's milk is not sufficient or suitable. The Taipei City Hospital Milk Bank was established in 2005 and is the first nonprofit human milk bank to operate in Taiwan. METHODS: The milk bank has adopted standards of practice laid down by the Human Milk Banking Association of North America and United Kingdom Association for Milk Banking. The clinical character

Developing global guidance on human milk banking.

Donor human milk is recommended by the World Health Organization both for its advantageous nutritional and biological properties when mother's own milk is not available and for its recognized support for lactation and breastfeeding when used appropriately. An increasing number of human milk banks are being established around the world, especially in low- and middle-income countries, to facilitate the collection, processing and distribution of donor human milk. In contrast to other medical products of human origin, however, there are no minimum quality, safety and ethical standards for donor hu

Bacterial Multiresistance and Microbial Diversity of Milk Received by a University Hospital Milk Bank.

Breastfeeding is fundamental for the development and protection of the newborn, and microorganisms present in breast milk are associated with the development of the infant's intestinal microbiota. However, there are factors that interfere with breastfeeding, resulting in the need to supply donated milk to milk banks for these children. Even though there is a restriction on medications prescribed for pregnant and breastfeeding women, some antimicrobials are accepted, as long as they are used correctly and as they can increase the selection pressure for resistant bacteria. The microorganisms pre
