Case Report of Dysphoric Milk Ejection Reflex.

Dysphoric milk ejection reflex (D-MER) is a dysregulation of emotions (dysphoria) that can occur during the milk ejection reflex and during breastfeeding or expressing breast milk. Symptoms of D-MER present suddenly and can include hopelessness, sadness, nervousness, irritability, nausea, dread, palpitations, and a hollow feeling in the stomach. Although D-MER was first reported in 2007, it remains understudied to date and should not be confused with postnatal depression or anxiety disorders. Knowing that they are experiencing symptoms of a named condition is very supportive for women who expe

Dysphoric Milk Ejection Reflex: Prevalence, persistence, and implications.

BACKGROUND: Dysphoric Milk Ejection Reflex (D-MER), characterized by an abrupt wave of negative emotions during milk-letdown, affects a relevant proportion of lactating parents and may significantly impact feeding choices, but remains poorly understood. METHODS: We conducted a cross-sectional study using an online survey of lactating parents of children 18 months and younger who were nursed for any period of time. RESULTS: The prevalence of D-MER in our cohort was 14.2 % (n = 209/1469). Among the 209 women who reported having experienced D-MER, the most frequently selected descriptions were:

Milk ejection in mice LG/J x SM/J.

In mammals, milk provision is crucial to offspring survival and growth from birth to weaning. Milk deficiency early in life may cause death or changes in the progeny metabolism that later may lead to obesity and metabolic disorders. This study investigates milk ejection (ME) the first day after birth (D1) in F(2) females from the intercross of LG/J and SM/J inbred mice strains. The absence of milk in F(3) pups' stomach at D1 is directly associated with their survival (p < 0.001) and growth pattern (p < 0.001) in the early stages of life. Furthermore, late growth pattern is also affected by thi

Blood pressure responses to milk ejection in the young rat.

When young rats receive milk ejections from their mothers they exhibit abrupt 30-50% increases in blood pressure. In our first experiment, we recorded, via carotid cannulae, blood pressure and heart rate during natural nursing bouts. Blood pressure changes coincident with behaviorally defined milk ejections were twice as large as the changes associated with other behaviors observed. In addition, the increases in blood pressure during milk ejection were significantly greater when pups were separated from their mothers for 18-20 hours prior to testing. A second experiment showed that these respo

Dysphoric Milk Ejection Reflex: The Psychoneurobiology of the Breastfeeding Experience.

Breastfeeding, given its biochemical and physiological basis, is known for its many benefits for both the lactating mother and the infant. Among the many challenges new breastfeeding mothers experience is the feeling of aversion in response to their newborn's suckling which has been termed dysphoric milk-ejection reflex (D-MER). Characterized by intense feelings of dysphoria which may eventually interfere with the mother's ability to breastfeed regularly, evidence suggests both the neurobiological and psychological basis of D-MER in an attempt to explain its complexity. Biologically, breastfee

Experiences of Women with Dysphoric Milk Ejection Reflex: A Qualitative Study.

BACKGROUND: Dysphoric milk ejection reflex (D-MER) is a physiologic condition in which some women experience brief but intense negative emotions immediately before or during milk release. Awareness and understanding of D-MER remain limited among both mothers and healthcare providers, and qualitative evidence on women's experiences of dysphoric D-MER remains limited. RESEARCH AIM: The aim of this study was to explore the experiences and coping strategies and support the needs of women with D-MER during breastfeeding and/or expressing milk. METHODS: A qualitative descriptive design was employe

[Pineal peptides in the regulation of milk-ejection reflex in lactating rats].

The effects of pineal peptides (mol. mass 1-4 kDa) intranasal infusions on some parameters of milk-ejection reflex were investigated. Peptides were extracted from dairy-cattle pineal glands. Pineal peptides increase body weight, levels of water intake, plasma prolactin concentration and milk yield in rats when infusing daily in dose 1 microgram/kg from the third day of lactation. On 9th and 12th days of lactation during 1-hour nursing seance the significantly greater number of reflective milk ejections were found. When 1 hour before the peptides infusion rats were intraperitoneally injected by

Excitatory effects of intraventricular injections of oxytocin on the milk ejection reflex in the rat.

The effects of oxytocin, administered into the 3rd cerebral ventricle, on the milk ejection reflex, were studied in urethane-anaesthetized lactating rats. Intramammary pressure, electrical activity of oxytocinergic neurones and EEG were recorded simultaneously. Intraventricular injection of oxytocin (6, 60 and 600 microU) significantly increased both milk ejection frequency and the number of spikes in the oxytocinergic neurones characteristic bursts. The amount of hormone released for each milk ejection was higher after intraventricular oxytocin injection than before. The strongly excitatory e

Dysphoric milk ejection reflex: prevalence and associations with self-reported mental health history.

This study sought to determine prevalence of Dysphoric Milk Ejection Reflex (D-MER) and mental health correlates in a cohort of patients from a breastfeeding medicine clinic. We determined prevalence of D-MER symptoms and performed bivariate analyses to explore associations between D-MER symptoms, demographics, and self-reported mental health history. Among 271 patients, 42 (15.5%) reported D-MER. Women with D-MER symptoms were more likely to report a history of panic attacks (28.6 vs. 12.2%, OR 2.87, 95% CI 1.23-6.25).

Dysphoric Milk Ejection Reflex: Report of Two Cases and Postulated Mechanisms and Treatment.

Background: The symptoms of Dysphoric Milk Ejection Reflex (D-MER) occur just before milk release, last no more than a few minutes, and then subside spontaneously, which are characterized by a sudden and transient feeling of dysphoria, depression, sadness, or other negative emotions. These emotions may adversely affect the mother's lactation behavior and mental health, negatively influence the mother-child relationship, and even result in self-harm or suicidal tendencies in lactating women. Case Presentation: We reported two cases of breastfeeding mothers with D-MER who experienced unpleasant

Oxytocin: From milk ejection to maladaptation in stress response and psychiatric disorders. A psychoneuroendocrine perspective.

Oxytocin (OT) is implicated in stress reduction as well as in social behavior. It inhibits the stress-induced activity of the hypothalamic-pituitary adrenal axis responsiveness. OT is involved in social affiliation, sexual and maternal-infant binding, anxiety, mood, feeding control and memory. Several lines of evidence suggest a role of OT in psychiatric disorders. Various psychiatric disorders are strongly influenced by social variables, such as panic attacks, depression and early childhood autism, and seem to exhibit a particularly close connection with the brain dynamics that underlie socia

[Failure of milk ejection reflex in primiparous cows from the differential diagnostic and therapeutic aspects].

A particular failure of the milk-ejection reflex in dairy cows is described on the base of own experiences in farms and reports from literature. Four cases are reported more in detail. Quantity and fat content of cisternal, alveolar and residual milk fractions, oxytocin content in blood serum, milk-ejection after manual teat stimulation, after massage of the clitoris or rectal stimulation of the vagina and cervix were investigated. This failure of the milk-ejection is observed mostly in primiparous cows and always at the begin of the lactation period. It is complete and permanent unless succes

Dysphoric milk ejection reflex: an overlooked syndrome affecting up to one in five breastfeeding women.

Dysphoric milk ejection reflex (D-MER) is a syndrome characterized by a sudden onset of negative emotional experiences occurring a few seconds prior to each ejection of breast milk. These sensations arise and resolve abruptly, typically lasting only several minutes, and recur with every individual milk let-down. As a result, women affected by D-MER may experience dozens of such episodes throughout the day and night. According to available studies, D-MER affects approximately one in ten to one in five breastfeeding women. The specific emotional experiences vary among individuals in type, intens

[Effects of PGF2 alpha prostaglandins on milk ejection in the ewe. Consequences of ovariectomy with or without estroprogestative complementation].

The intrajugular administration of 200 micrograms of Dinolytic (tromethamine salt of PGF2 alpha) to lactating ewes caused an intramammary pressure (IMP) increase only during the luteal phase of the sexual cycle (group A). The increase in progesteronemia, induced by three daily injections of 2.5 micrograms of LH-RH, did not modify response amplitude (group B). On the other hand, bilateral ovariectomy (groupe C) led to the suppression of those responses, but supplementary oestroprogestative (table 1) did not re-establish them. It is probable that the milk ejection caused by PGF2 alpha resulted f

Defining Dysphoric Milk Ejection Reflex: using Premenstrual Dysphoric Disorder as a framework for proposing preliminary diagnostic criteria.

Dysphoric Milk Ejection Reflex (D-MER) is a distinct neurobiological condition characterized by negative alterations in mental state in response to milk letdown during lactation. Symptoms vary by patient and can include feelings of sadness, anxiety or agitation. Importantly, the symptoms are brief, typically lasting no more than 5 minutes. Prevalence has been found between 6 and 27% of lactating women, but studies show heterogeneity, due in part to inconsistent definition. D-MER is not currently classified in the Diagnostic and Statistical Manual of Mental Disorders (DSM) or the International

Dysphoric Milk Ejection Reflex: Characteristics, Risk Factors, and Its Association with Depression Scores and Breastfeeding Self-Efficacy.

Background: Dysphoric milk ejection reflex (D-MER) is a phenomenon that occurs before milk letdown and is described as a wave of negative or devastating emotions, ranging from mild to severe and lasting for seconds to minutes. To date, there has been little research regarding this phenomenon. This study aims to determine the prevalence of D-MER in our population as well as its association with postnatal depression scores and breastfeeding self-efficacy. Methods: Lactating persons between 4 and 12 weeks postpartum at our institution were invited to complete an anonymous 59-question survey via a

Effect of presynchronization using prostaglandin F2alpha and a milk-ejection test on pregnancy rate after the timed artificial insemination protocol, Ovsynch.

The objective of this research was to determine if PGF2alpha-induced milk letdown (ML) is an accurate indicator of luteolysis, allowing cows to be synchronized to begin the Ovsynch protocol (GnRH-7d-PGF2alpha-2d-GnRH-24h-AI) at the most beneficial time of the estrous cycle (days 5-9), and determine if this would improve pregnancy rate (PR). Lactating Holstein cows between 55 and 70 days in milk were used to evaluate the ML test and PR after the Ovsynch protocol, when initiated on the basis of the test result (PROSYNCH). PROSYNCH cows (n = 60) had one teat cannulated to test for ML and were tre
