We report our experience with Molteno implants in 27 eyes of 20 children with glaucoma. Associated ocular findings included Sturge Weber syndrome (one patient), aniridia and retinoschisis (one patient), aphakia (seven patients, eight eyes), rubella syndrome (two patients), Peters anomaly (one patient), and Treacher Collins syndrome (one patient). Molteno implants are gaining wide acceptance for refractory glaucoma in both children and adults. Such surgery led to vertical strabismus in three patients. Two children developed an inability to elevate the globe after Molteno implants were placed superiorly. One adult, whose Molteno implant was placed inferiorly, could not fully depress the globe postoperatively, and experienced vertical diplopia. The authors describe their experience with Molteno implants in 21 patients. The most frequent types of glaucoma were congenital glaucoma--5 times--and glaucoma associated with pseudophakia--5 times. We report four cases in which cyclocryotherapy proceded Molteno implantation for refractory glaucoma. Three of the four cases were complicated by chronic postoperative hypotony, apparently resulting from overfiltration by the seton, even though a single-plate implant, with a limited filtration area, was used. Religation of the filtering tube or removal of the implant should be considered in these cases.

BACKGROUND: In glaucoma eyes in which the pressure regulation was not possible with drugs and surgery or which were blind already, we treated the patients with a Molteno implant by von Denffer.
MATERIALS AND METHODS: In 10 years (1995 to 2004) we treated 19 eyes with a glaucoma drainage device -- the Molteno implant developed by von Denffer. Molteno implant surgery was performed on 33 eyes of 29 patients with complicated and uncontrolled glaucoma. Average preoperative IOP was 34.0 +/- 8.8 mmHg. After a mean postoperative follow-up period of 18.3 months the IOP was 15.8 +/- 8.3 mmHg. Three patients had an IOP over 22 mmHg. Overall, success rate was 73% with IOP and medication as criteria. In 75% of eyes visual acuity improved or remained within two lines of the preoperative value. The highest success rate was 83% (10 of 12 eyes) in congenital and uveitic glaucoma when both IOP, medication and visual acuity were taken into account.

Drainage surgery may be required in patients who have previously demonstrated a corticosteroid-induced rise in intraocular pressure. This report presents three patients who had complicated secondary glaucoma and who demonstrated an intraocular pressure increase in response to topical cortico-steroid drugs, despite the presence of a functioning Molteno implant. Developmental glaucoma, which cannot be controlled by conventional techniques, poses a difficult problem. Twenty-three eyes of 18 patients with developmental glaucoma underwent a two-stage implantation of the double template Molteno valve with follow-up between 12 and 84 months. Eighty-two black patients with refractory glaucoma were treated with a single-plate Molteno implant inserted in a single-stage procedure. Sixty-two single-plate single-stage Molteno implantations for complicated glaucomas were performed between March 1991 and November 1992. The charts of all these patients were reviewed to determine the intraocular pressure (IOP) control success rate (< 21 mm Hg with or without medications), visual success rate (retention or improvement of visual acuity from preoperative level) and the rate of complications encountered. A technique for controlling intraocular pressure in the immediate postoperative period with Molteno implants is described. Filtration tubes were closed with polypropylene sutures in 13 patients. Filtration tubes closed with 7-0 sutures provided the most satisfactory partial to complete lumen closure. The ligatures in six patients later were opened by argon laser.

We performed Molteno implant surgery in one eye each of 41 patients with uncontrolled glaucoma. Intraocular pressure was controlled (intraocular pressure less than or equal to 18 mm Hg) in 32 eyes (78%). The alternatives in the management of infantile glaucoma refractory to conventional treatment are quite limited and unrewarding.
